Statement of Appointment (PHS 2271)

This form is used to appoint individuals as trainees to institutional Ruth L. Kirschstein-National Service Research Award (Kirschstein-NRSA) programs (e.g., T32, T34, T35) and applicable non-NRSA individual and institutional research training programs (e.g., the NIH intramural research training award program and T15 training grants). It can also be used to document the appointment of scholars to institutional career development awards (e.g., K12) and individual participants to research education awards (e.g., R25).

The use of the eRA Commons xTrain system is mandatory  to electronically prepare and submit PHS 2271 Statement of Appointment forms.

Individual Fellowship Activation Notice (PHS 416-5)

Termination Notice (PHS 416-7)

This form is used to document the termination of appointments for National Research Service Award (NRSA) recipients as well as for some non-NRSA individual and institutional research training programs, research education awards, and institutional career development awards.

Payback Agreement (PHS 6031)

Payback agreement to be completed by trainees and fellows before beginning the first 12 months of postdoctoral support. 

The agreement must be signed and mailed to the awarding grants management office named in the notice of grant award.

Annual Payback Activities Certification (APAC) (PHS 6031-1)

The Annual Payback Activities Certification form is the formal  communication between former Kirschstein-NRSA recipients and the PHS regarding the status of payback activities.

The certification form must be signed and returned to the NRSA Payback Service center. See form instruction for details.
