There are two biosketch format pages - fellowship biosketch (this page) and non-fellowship biosketch. The NIH biosketch format pages must be used for application due dates and all RPPR, JIT, and Prior Approval submissions on or before January 24, 2026.
Effective January 25, 2026, Common Forms for Biographical Sketch and Current and Pending (Other) Support and NIH Biographical Sketch Supplement forms must be used. See NOT-OD-26-018.
For due dates January 25, 2025 through January 24, 2026, Scholastic Performance is no longer required for Fellowship Biosketches. Enter “N/A” or leave blank. See NOT-OD-24-107.
