Introduction
The Public Health Service (PHS) Policy on Humane Care and Use of Laboratory Animals (Policy), IV.B.1.-2., requires the Institutional Animal Care and Use Committee (IACUC) to review the institution's program for humane care and use of animals and inspect all of the institution's animal facilities at least once every 6 months using the Guide for the Care and Use of Laboratory Animals (Guide) as a basis for evaluation.
- The program review includes a comprehensive administrative and conceptual discussion about the systems, policies, and procedures that make up the animal care and use program.
- The facility inspection includes observation and investigation of “Any and all buildings, rooms, areas, enclosures, or vehicles, including satellite facilities, used for animal confinement, transport, maintenance, breeding, or experiments inclusive of surgical manipulation. A satellite facility is any containment outside of a core facility or centrally designated or managed area in which animals are housed for more than 24 hours.” (PHS Policy III.B. "Animal Facility").
The IACUC is required to report the results of program review and inspection to the Institutional Official (IO) in the Semiannual Report.
Semiannual Program Review and Facility Inspection Checklist
This checklist is a tool to help you conduct thorough semiannual reviews. It covers the major topics of the Guide and the requirements of the PHS Policy. The checklist does not replace the Guide, but you should use it along with the Guide. The Guide provides the standards, recommendations, and descriptions of desired outcomes necessary to evaluate and inspect an animal care and use program.
You are not required to use the checklist, but we encourage you to amend it as necessary to reflect institutional programs and needs, or develop your own checklist. If the checklist is modified, periodic review of the checklist is recommended to ensure relevant topics are considered as the animal care and use program changes.
Suggestions or comments about this checklist should be e-mailed to: [email protected]
Tips
- It is recommended that the Program Review section be completed during an IACUC meeting.
- Because physical aspects of a program require visual observation to evaluate, it is recommended that the Facility Inspection section be completed during an inspection of the facilities, including satellite facilities.

Semiannual Report to the Institutional Official
The PHS Policy requires the IACUC to prepare and submit a report of their semiannual evaluation to the Institutional Official (IO). The IACUC may determine the best means of conducting an evaluation of the institution's programs and facilities and is responsible for the evaluation and report. Final reports of the semiannual evaluations and inspections are considered full committee actions and should be reviewed and endorsed by a majority of the IACUC.
Semiannual reports should be submitted to the IO regularly, but they should only be submitted to OLAW upon request. If the institution is submitting a new or renewal Assurance and is not accredited by AAALAC International, OLAW will request the most recent semiannual report along with the Assurance.
You must include the following information in your report to the IO:
- Description of the nature and extent of the institutions adherence to the Guide for the Care and Use of Laboratory Animals (Guide) and the PHS Policy, including any departures and reason for each departure
- Identification of any deficiencies in the program or facility, including classification as either significant or minor
- Reasonable and specific plan and schedule for correction of deficiencies
- Identification of facilities accredited by AAALAC International
- Any minority views submitted by IACUC members
- Signature of a majority of the IACUC members, if applicable
Regarding signatures, NOT-OD-24-075 and NOT-OD-24-076 explain that "Digital and scanned signatures are acceptable. Alternatively, an email acknowledgement from the member(s) may serve as their acceptance with the semiannual report...These emails should be retained for the record by the IACUC."
