The UH2/UH3 Phase Innovation Awards Cooperative Agreement involves 2 phases. The UH2 is to support a project with specific milestones to be accomplished at the end of the 2-year period. The UH3 is to provide funding for 3 years to a project that successfully completed the milestones set in the UH2. UH2 projects that have met their milestones will be administratively considered by NIDA and prioritized for transition to the UH3 phase. Investigators responding to this FOA must address both UH2 and UH3 phases.

According to the 2013 National Survey on Drug Use and Health, an estimated 24.6 million Americans aged 12 or older used illicit drugs in the past month and 21.6 million were classified with a substance use disorder (SUD). Marijuana was the most commonly used illicit drug, with 19.8 million past-month users. Marijuana was the illicit drug with the largest number of persons with past year dependence or abuse in 2013, followed by pain relievers, then by cocaine. The same survey estimates that of the 6.9 million persons with an illicit drug use disorder, 4.2 million had marijuana, 1.9 million pain reliever, 855,000 cocaine, and 517,000 heroin use disorder. These results show that SUDs are a significant public health problem that needs immediate attention.
Currently, there are medications approved by the Food and Drug Administration (FDA) for the treatment of nicotine and opiate dependence. However, none of these medications provides optimal therapy. There are no FDA approved medications for cocaine, methamphetamine, or cannabis use disorders. Thus, the development of safe and effective medications to treat these disorders is an urgent public health need.
Scientific advances are offering extraordinary opportunities for the development of medications to treat SUDs. These advances include the discovery of genes and proteins, receptors, neurotransmitters, neuromodulators, and brain circuits associated with drug abuse, as well as risk and protective factors for the onset and progression of these disorders. Multiple therapeutic approaches, ranging from small molecules to biologics (e.g., vaccines) may be poised for the next step in the FDA approval process, and concerted efforts are needed to advance these potential pharmacotherapies to approval.

The NIDA Translational Avant-Garde Award complements NIDA s traditional investigator-initiated grant programs by supporting applications focused on the translation of basic scientific discoveries into medications. Research on the neurobiology of addiction has identified an increasing number of genetic, cellular and biological targets where a therapeutic intervention could benefit patients with SUDs. Translation of these scientific discoveries into therapeutic interventions requires deliberate, persistent, and focused commitment and effort.
The NIDA Translational Avant-Garde Award provides this level of support and commitment by offering a scientist the unique opportunity to realize the therapeutic potential of his/her scientific discoveries and advance them one step closer to regulatory approval. Projects responsive to these coupled announcements could be undertaken at any point along the drug development continuum, from late discovery (i.e., lead optimization/early safety) up to Proof of Concept (POC) clinical trials.
Through this FOA, NIDA seeks to attract exceptionally-talented investigators to the mission of expanding the number and breadth of lead compounds in the pipeline for drug addiction treatment, optimizing these leads, and/or advancing them to clinical testing.
Applicants for the NIDA Translational Avant-Garde award are expected to have a robust body of background data in the basic science and early discovery phases to be poised for transition of a lead compound to the pre-clinical and clinical phases of medications development. NIDA expressly seeks to promote research projects that are ready for translation and can accelerate the development of new medications. Accordingly, only those applicants who conduct research studies positioned between late pre-clinical discovery (i.e., lead optimization and beyond) and POC clinical trials are appropriate to apply. That is, for small molecules, the earliest stage of eligibility for this Award is already having small-molecule compounds with proof of desired pharmacological activity. For biologics, the profiling of promising product candidates in animal models of drug addiction will be allowed as the earliest entry point.
Applications focusing solely on novel target identification/validation, generation of new animal models, development/testing of new human laboratory models, or mechanistic studies of the neurobiology of addiction are considered non-responsive for the NIDA Translational Avant-Garde Award.
Applications submitted in response to this FOA may focus on studying 1) new chemical entities (NCEs), 2) already-marketed pharmacotherapies, 3) biologics (e.g., immunotherapies such as vaccines and antibodies, enzymes, and gene therapies), 4) combinations of medications, and/or 5) new delivery devices/technologies.
The medications investigated for SUDs may target one or more of the neuropathological mechanisms, the various clinical stages, and/or the medical/psychiatric complications of one or multiple SUDs.
This FOA encourages, but is not limited to, studies of medications for SUDs in the following areas:
It is expected that all applications involving treatment seeking individuals will provide a behavioral therapy component. However, the scope of this FOA does not include the evaluation of the safety and/or efficacy of psychosocial interventions.
Awardees of this FOA may collaborate with pharmaceutical companies, academic investigators, and NIH scientists. Awardees are encouraged to make their individual datasets and aggregated datasets available for data sharing to other investigators when appropriate.
All studies involving NIH defined clinical trials must have a Data and Safety Monitoring Plan (http://grants.nih.gov/grants/guide/notice-files/NOT-OD-00-038.html). The DSMB is responsible for reviewing interim and final data, and recommending whether the study should be continued, modified, or terminated. The DSMB reviews the protocols, monitors recruitment, adverse events, data quality, outcome data, and overall study performance. For those clinical trials that required by this policy to have a Data and Safety Monitoring Board (DSMB), the investigators have the option of having their study monitored the DSMB of the Division of Pharmacotherapies and Medical Consequences of Drug Abuse (DPMCDA) of NIDA. This DSMB reports to the Director, DPMCDA. The awardees also have the option to use another DSMB. Applications involving one or more clinical trials must state that the clinical trial(s) will be monitored by a DSMB, which could be the DPMCDA sponsored DSMB. The DSMB to be used for each trial will be agreed upon with NIH prior to award.
HIV/AIDS Counseling and Testing Policy for the National Institute on Drug Abuse: In light of recent significant advances in rapid testing for HIV and in effective treatments for HIV, NIDA has revised its 2001 policy on HIV counseling and testing. NIDA-funded researchers are strongly encouraged to provide and/or refer research subjects to HIV risk reduction education and education about the benefits of HIV treatment, counseling and testing, referral to treatment, and other appropriate interventions to prevent acquisition and transmission of HIV. National Advisory Council on Drug Abuse Recommended Guidelines for the Administration of Drugs to Human Subjects: The National Advisory Council on Drug Abuse (NACDA) recognizes the importance of research involving the administration of drugs with abuse potential, and dependence or addiction liability, to human subjects. Potential applicants are encouraged to obtain and review these recommendations of Council before submitting an application that will administer compounds to human subjects. The UH2/UH3 phased innovation cooperative agreement supports investigation of novel scientific ideas or new interventions, model systems, tools, or technologies that have the potential for significant impact on biomedical or behavioral and social sciences research. An UH2/UH3 grant application need not have preliminary data, extensive background material or preliminary information; however, they may be included if available. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Accordingly, reviewers will emphasize the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Reviewers will assign a single impact score for the entire application, which includes both the UH2 and UH3 phases.
Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).
Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Does the project address an important problem or a critical barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field? Has the applicant made a compelling case that proposed project is translational?
Are the PD(s)/PI(s), collaborators, and other researchers well suited to the project? If Early Stage Investigators or New Investigators, or in the early stages of independent careers, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project? In particular, is there sufficient evidence of the investigator’s expertise in drug discovery and development and/or translational research?
Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed? Does the investigator provide sufficient evidence that the compound or compounds to be evaluated represent an innovative pharmacotherapy for SUDs? Is the proposed work likely to advance the development of a compound toward regulatory approval by the FDA for a SUD indication?
Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project? Are potential problems, alternative strategies, and benchmarks for success presented? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed?
If the project involves human subjects and/or NIH-defined clinical research, are the plans to address 1) the protection of human subjects from research risks, and 2) inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion or exclusion of children, justified in terms of the scientific goals and research strategy proposed?
Does the research project clearly identify a treatment outcome of the research efforts? Does the research project represent a significant leap from incremental knowledge-gathering to a practical application (a translational endpoint)? Does the application provide clear entry and exit points on the drug development pathway? Are they well-defined and well-justified? Does the application acknowledge potential problem areas and consider alternative approaches? Does the application provide milestones that can be achieved in the 2-year period of the UH2 grant?
Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?
As applicable for the project proposed, reviewers will evaluate the following additional items while determining scientific and technical merit, and in providing an overall impact score, but will not give separate scores for these items.
For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of children to determine if it is justified in terms of the scientific goals and research strategy proposed. For additional information on review of the Inclusion section, please refer to the Guidelines for the Review of Inclusion in Clinical Research.
The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following five points: 1) proposed use of the animals, and species, strains, ages, sex, and numbers to be used; 2) justifications for the use of animals and for the appropriateness of the species and numbers proposed; 3) adequacy of veterinary care; 4) procedures for limiting discomfort, distress, pain and injury to that which is unavoidable in the conduct of scientifically sound research including the use of analgesic, anesthetic, and tranquilizing drugs and/or comfortable restraining devices; and 5) methods of euthanasia and reason for selection if not consistent with the AVMA Guidelines on Euthanasia. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.
For Resubmissions, the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.

For Revisions, the committee will consider the appropriateness of the proposed expansion of the scope of the project. If the Revision application relates to a specific line of investigation presented in the original application that was not recommended for approval by the committee, then the committee will consider whether the responses to comments from the previous scientific review group are adequate and whether substantial changes are clearly evident.
As applicable for the project proposed, reviewers will consider each of the following items, but will not give scores for these items, and should not consider them in providing an overall impact score.
Reviewers will assess whether the project presents special opportunities for furthering research programs through the use of unusual talent, resources, populations, or environmental conditions that exist in other countries and either are not readily available in the United States or augment existing U.S. resources.
Reviewers will assess the information provided in this section of the application, including 1) the Select Agent(s) to be used in the proposed research, 2) the registration status of all entities where Select Agent(s) will be used, 3) the procedures that will be used to monitor possession use and transfer of Select Agent(s), and 4) plans for appropriate biosafety, biocontainment, and security of the Select Agent(s).
Reviewers will comment on whether the following Resource Sharing Plans, or the rationale for not sharing the following types of resources, are reasonable: 1) Data Sharing Plan; 2) Sharing Model Organisms; and 3) Genomic Wide Association Studies (GWAS) /Genomic Data Sharing Plan.
Reviewers will consider whether the budget and the requested period of support are fully justified and reasonable in relation to the proposed research.

Cooperative Agreement Terms and Conditions of Award
The following special terms of award are in addition to, and not in lieu of, otherwise applicable U.S. Office of Management and Budget (OMB) administrative guidelines, U.S. Department of Health and Human Services (DHHS) grant administration regulations at 45 CFR Parts 74 and 92 (Part 92 is applicable when State and local Governments are eligible to apply), and other HHS, PHS, and NIH grant administration policies.
The administrative and funding instrument used for this program will be the cooperative agreement, an "assistance" mechanism (rather than an "acquisition" mechanism), in which substantial NIH programmatic involvement with the awardees is anticipated during the performance of the activities. Under the cooperative agreement, the NIH purpose is to support and stimulate the recipients' activities by involvement in and otherwise working jointly with the award recipients in a partnership role; it is not to assume direction, prime responsibility, or a dominant role in the activities. Consistent with this concept, the dominant role and prime responsibility resides with the awardees for the project as a whole, although specific tasks and activities may be shared among the awardees and the NIH as defined below.

The PD(s)/PI(s) provide, in concert with the NIH staff, support necessary to ensure that sites and investigators, and NIH and other research partners fully comply with federal regulatory requirements, including but not limited to those relating to human subjects protections, informed consent, and reporting of adverse events.
Awardees and NIH will jointly develop appropriate confidentiality procedures for data collection, processing, storage and analysis to ensure the confidentiality of data on individual health care provider organization patients, health care providers and other institutions.
All awardees and NIH will cooperate to ensure the timely and broad dissemination of lessons learned, to inform researchers and health care systems engaged in research in health care settings.
Dispute Resolution:
Any disagreements that may arise in scientific or programmatic matters (within the scope of the award) between award recipients and the NIH may be brought to Dispute Resolution. A Dispute Resolution Panel composed of three members will be convened. It will have three members: a designee of the investigator chosen without NIH staff voting from among the PI of the FOA, one NIH designee, and a third designee with expertise in the relevant area who is chosen by the other two; in the case of individual disagreement, the first member may be chosen by the individual awardee. This special dispute resolution procedure does not alter the awardee's right to appeal an adverse action that is otherwise appealable in accordance with PHS regulation 42 CFR Part 50, Subpart D and DHHS regulation 45 CFR Part 16.

When multiple years are involved, awardees will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.
A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.
The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000.
