A recent report from the Institute of Medicine (IOM) identifies the epidemic of chronic condition as the nation’s leading health challenge and calls for cross-cutting, coordinated public health actions for living well with chronic illness . This Funding Opportunity Announcement (FOA) addresses that recommendation by describing an initiative that focuses on self-management as a mainstream science in order to reduce the burden of chronic illnesses/conditions. Self-management is the ability of the individual, in conjunction with family, community, and healthcare professionals, to manage symptoms, treatments, lifestyle changes, and psychosocial, cultural, and spiritual consequences associated with one or more chronic illnesses or conditions.
The challenge is that self-management science for chronic conditions must be both age- and culturally-appropriate, embrace health equity, include individuals with chronic conditions and those around them, account for who will benefit from self-management, and incorporate biobehavioral measures and new innovative designs that capture both the psychosocial and economic aspects of a chronic condition. The IOM report states that despite existing national initiatives to help patients with chronic conditions, the health care enterprise is not doing enough for patients. Chronic conditions are steadily moving toward crisis proportions with medical care costs of people with chronic conditions representing 75 percent of the $2 trillion in U.S. annual health care spending. However, maintaining or enhancing quality of life has not been given the attention it deserves by health care funders, health systems, policy makers, and public health programs and agencies. The report highlights key chronic conditions arthritis, cancer survivorship, chronic pain, dementia, depression, diabetes, posttraumatic disabling conditions, and vision and hearing loss as exemplars to illustrate their diverse sequelae, including emotional distress, sleep and pain symptoms, physical impairments, and age-related disabilities, all of which detract from living well. The report indicates that conditions tend to cluster, particularly among older adults, requiring individuals to manage a complex set of symptoms and treatments.
Disparities in the occurrence of chronic conditions and self-management according to race/ethnicity, income, and geography are also prevalent. Self-management is intended to improve well-being and strengthen self-determination and health care participation, while reducing hospitalization and other types of health care utilization. Self-management reinvents traditional health care as the patient becomes engaged as an active participant in care decisions. Patients who self-manage become informed about their condition and take an active role in treatment, including decisions about whether they adhere to treatments recommended by clinicians, whether they wish to undergo diagnostic or surgical procedures, and which health-behavior-related goals they choose to pursue. As a result, self-management promotes adherence to treatment plans, builds individual capacity, improves interaction between patients and caregivers, reduces the use of medical specialists, and optimizes clinical outcomes across the lifespan. Chronic conditions have emerged as major health concern of Americans.
People are increasingly focused not simply on living longer, but on maintaining or even improving their capacity to live well over their entire lives. Optimal self-management of chronic conditions requires a systematic examination of lifestyle strategies, interventions, and technologies targeted across multiple levels, including patients, families, health providers, health care organizations, communities, and populations.
The AREA program will enable qualified scientists to receive support for research projects. These grants are intended to create a research opportunity for scientists at institutions that have not previously been major recipients of NIH grant awards. It is anticipated that investigators supported under the AREA program will benefit from the opportunity to conduct independent research; that the grantee institution will benefit from a research environment strengthened through AREA grants and furthered by participation in the diverse extramural programs of the NIH; and that students at recipient institutions will benefit from exposure to and participation in scientific research in the biomedical and behavioral sciences.

The number of awards is contingent upon NIH appropriations and the submission of a sufficient number of meritorious applications.
Applicants may request up to $300,000 in direct costs plus applicable Facilities & Administrative (F&A)/indirect costs for the entire project period of up to 3 years. Note when a consortium is involved, the $300,000 direct cost limit is exclusive of consortium F&A costs. These can be requested in addition to the $300,000 direct costs limit.
The scope of the proposed project should determine the project period. The maximum project period is 3 years.

2. The applicant organization may not receive research support from the NIH totaling more than $6 million per year (in both direct and F&A/indirect costs) in each of 4 of the last 7 years.
Institutions with Multiple Schools or Colleges
For institutions composed of multiple schools or colleges, the criterion of financial eligibility is based on the amount of NIH research grant monies received, not by the institution (university or college) as a whole, but by the individual school/college or aggregation of "other academic components" (as defined in this section) where the PD(s)/PI(s) has a primary appointment (e.g., School of Medicine, College of Nursing, etc.). Thus, each of the following should be considered independently when determining the financial eligibility of the applicant organization.
To determine the eligibility of an institution, applicants should consult the list of ineligible institutions on the AREA program website. If the name of the institution does not appear on the list, it is likely eligible to apply for AREA grants. Applicants should check with their own institutions if unsure.
An AREA grant is permitted to have a subcontract to a non-AREA-eligible institution. However, applicants should keep the goals of the AREA program in mind when preparing the application, which include strengthening the research environment of the institution and exposing students to research.
Although foreign institutions are not eligible, applications may include a foreign collaboration, component, or consortium.
Non-domestic (non-U.S.) Entities (Foreign Institutions) are not eligible to apply.

Applicant Organizations

The NIH Policy on Late Submission of Grant Applications states that failure to complete registrations in advance of a due date is not a valid reason for a late submission.

The objectives of the R15 program are to (1) provide support for meritorious research, (2) strengthen the research environment of schools that have not been major recipients of NIH support, and (3) expose available undergraduate and graduate students in such environments to meritorious research. Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to make an important scientific contribution to the research field(s) involved, to provide research opportunities to students, and to strengthen the research environment of the institution, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).
Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Significance
Does the project address an important problem or barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field? If funded, will the AREA award have a substantial effect on the school/academic component in terms of strengthening the research environment and exposing students to research?
Investigator(s)
Are the PD(s)/PI(s), collaborators, and other researchers well suited to the project? If Early Stage Investigators or those in the early stages of independent careers, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD(s)/PI(s), do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project? Do the PD(s)/PI(s) have suitable experience in supervising students in research?
Innovation
Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?
Approach
Are the overall strategy, methodology, and analyses
well-reasoned and appropriate to accomplish the specific aims of the project?
Are potential problems, alternative strategies, and benchmarks for success
presented? If the project is in the early stages of development, will the
strategy establish feasibility and will particularly risky aspects be
managed? Does the application provide sufficient evidence that the project
can stimulate the interests of students so that they consider a career in the
biomedical or behavioral sciences?
If the project involves clinical research, are the plans for 1) protection of
human subjects from research risks, and 2) inclusion of minorities and members
of both sexes/genders, as well as the inclusion of children, justified in terms
of the scientific goals and research strategy proposed?
Environment
Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Does the application demonstrate the likely availability of well-qualified students to participate in the research project? Does the application provide sufficient evidence that students have in the past or are likely to pursue careers in the biomedical or behavioral sciences?
As applicable for the project proposed, reviewers will evaluate the following additional items while determining scientific and technical merit, and in providing an overall impact score, but will not give separate scores for these items.
Protections for Human Subjects
For research that involves human subjects but does
not involve one of the six categories of research that are exempt under 45 CFR
Part 46, the committee will evaluate the justification for involvement of human
subjects and the proposed protections from research risk relating to their
participation according to the following five review criteria: 1) risk to
subjects, 2) adequacy of protection against risks, 3) potential benefits to the
subjects and others, 4) importance of the knowledge to be gained, and 5) data
and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or
more of the six categories of research that are exempt under 45 CFR Part 46,
the committee will evaluate: 1) the justification for the exemption, 2) human
subjects involvement and characteristics, and 3) sources of materials. For

Inclusion of Women, Minorities, and Children
When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of children to determine if it is justified in terms of the scientific goals and research strategy proposed. Vertebrate Animals
The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following five points: 1) proposed use of the animals, and species, strains, ages, sex, and numbers to be used; 2) justifications for the use of animals and for the appropriateness of the species and numbers proposed; 3) adequacy of veterinary care; 4) procedures for limiting discomfort, distress, pain and injury to that which is unavoidable in the conduct of scientifically sound research including the use of analgesic, anesthetic, and tranquilizing drugs and/or comfortable restraining devices; and 5) methods of euthanasia and reason for selection if not consistent with the AVMA Guidelines on Euthanasia. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.

Progress reports for multi-year funded awards are due annually on or before the anniversary of the budget/project period start date of award. The reporting period for multi-year funded award progress report is the calendar year preceding the anniversary date of the award. Information on the content of the progress report and instructions on how to submit the report are posted at https://grants.nih.gov/grants/policy/myf.htm.
A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.
The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. See the NIH Grants Policy Statement for additional information on this reporting requirement.
