Funding Opportunity Purpose

This limited competition Funding Opportunity Announcement (FOA) is to support the Data Coordinating Center (DCC) for the Pediatric Heart Network (https://www.pediatricheartnetwork.org/). Only the current Pediatric Heart Network (PHN) DCC awardee is eligible to apply in response to this FOA. The DCC and the Clinical Research Centers (CRCs; see companion FOA RFA-HL-24-001)) will support and conduct research to improve the health and quality of life for children and adults with congenital heart disease, and children with acquired heart disease through multi-center collaborative clinical research. Collaboration among the CRCs, the DCC, and other stakeholders is expected to facilitate multi-center evaluation of medical, interventional, and surgical therapies; support development of novel treatment techniques and methodologies; identify and attempt to address equity gaps in outcomes; provide a training platform for fellows, junior faculty, and nurses; and promote dissemination and implementation of study results to improve the scientific basis for the care of affected individuals.

Section I. Funding Opportunity Description

Purpose

This limited competition Funding Opportunity Announcement (FOA) is to support the Data Coordinating Center (DCC) for the Pediatric Heart Network (PHN) (https://www.pediatricheartnetwork.org/), whose mission is to improve the health of children and adults with structural congenital heart defects, as well as children with acquired heart disease through multi-center clinical research. A companion FOA for Clinical Research Centers (CRCs) is available; see RFA-HL-24-001 Pediatric Heart Network Clinical Research Centers (UM1 Clinical Trial Optional).

Background and Objectives

Congenital cardiac malformations affect about 1% of live births and are the primary cause of death under age 1 due to a birth defect. Due to improved surgical and management techniques, survival has increased significantly in recent decades, and the number of adults living with congenital heart disease is now approximately twice the number of affected children. Acquired heart disease affects thousands of additional children each year.

The PHN has been a research enterprise since 2001 with established best practices for multi-center research in pediatric cardiology. During this funding cycle, the PHN will retain its infrastructure consisting of a DCC and up to nine CRCs. The PHN FOAs will support this infrastructure for seven years, including a number of trials and other studies as permitted by the budget. PHN investigators will be encouraged to seek funding for additional trials and studies through investigator-initiated grants, industry participation, and foundation grants.

The DCC will work with NHLBI, the CRCs, and other stakeholders to improve patient outcomes through clinical trials and other clinical studies of congenital heart disease in children and adults and of pediatric acquired heart disease.

The objectives for this limited competition FOA are to address a comprehensive plan to provide overall project coordination, administration, data management, and bolster biostatistical support, while emphasizing patient engagement, equity, and collaboration with multiple sites and other research entities through data science strategies. In particular, this FOA seeks an application, from the current PHN DCC, that proposes activities to meet the following broad goals:

  • Overall management of scientific activities related to study development, trial launch, and results dissemination
  • Development and implementation of a plan for diversity, equity, and inclusion for the PHN, both in terms of the science and of the investigators involved in the PHN
  • Development and implementation of a plan for patient and family engagement in PHN studies
  • Development and implementation of a data science approach to research questions across the PHN
  • Management of PHN core administration activities
  • Coordination of logistical and other support services

Organization and Governance

The PHN is a cooperative Network of CRCs, a DCC, an independent Protocol Review Committee, a Data and Safety Monitoring Board (DSMB) and NHLBI Project Scientists.The CRCs are supported by a separate award, described in detail in RFA-HL-24-001.

The DCC is expected to participate in a cooperative and interactive manner with the CRCs and NHLBI. CRCs are responsible for proposing, developing, and refining protocols; identifying, recruiting and retaining study participants including those NIH-designated health disparity populations such as American Indians/Alaska Natives, Asian Americans, Blacks/African Americans, Hispanics/Latinos, Native Hawaiians and other Pacific Islanders, sexual and gender minorities, socioeconomically disadvantaged populations, and underserved rural populations ; entering data promptly and accurately into the electronic data capture system; contributing to manuscripts and otherwise disseminating research findings; and contributing to PHN governance through committee participation.

The Executive Committee is the main governing body of the PHN and is composed of the Principal Investigators (PIs) of the core CRCs and the DCC, the chairs of select PHN Committees, a Study Coordinator and the NHLBI Project Scientists.Each member has one vote; the NHLBI staff has one collective vote.

The Steering Committee consists of PHN investigators from all sites (the 9 main CRCs funded through this FOA and auxiliary sites funded through subcontracts with the DCC to support recruitment in specific studies) , the DCC, and NHLBI staff. PHN investigators, study coordinators, DCC statisticians and staff, and NHLBI staff meet in person an average of twice a year and by teleconference on a monthly basis, as needed.

The DCC supports protocol development; provides sample size calculations, sophisticated statistical advice, and data analysis; supports manuscript preparation; and provides overall study coordination and quality assurance, including coordination of the activities of the DSMB and other standing committees. Funds to support the patient care costs associated with the protocols will be part of the DCC award and will be distributed to the CRCs by the DCC on a per-patient basis and according to the approved protocol budgets.

The PHN's scientific and administrative work is assisted by a variety of committees whose membership is drawn from the CRCs, the DCC, and the NHLBI.Committees may include protocol development, publications, finance, core lab, ancillary studies, nursing research, biospecimens, health services research, and other committees as needed to support PHN activities.

NHLBI is responsible for strategic direction and support for the PHN. The NHLBI Program Office and Office of Grants Management are responsible for the overall management. In addition to regular grant stewardship, the NHLBI Project Scientists will be involved substantially with the awardees as a scientific partner, consistent with the Cooperative Agreement mechanism.

The PHN’s NHLBI-appointed DSMB monitors patient safety and reviews performance of each study. As a part of its monitoring responsibility, the DSMB submits recommendations to the NHLBI regarding the conduct and continuation of each protocol.

Participation in/Selection of Trials

Multiple trials and other clinical studies will be conducted during the project period. Once the CRCs are convened in the new funding cycle, the PHN Executive and Steering Committees will consider proposed protocols and prioritize them. It is expected that each approved protocol will be implemented in all of the core CRCs. The ability to conduct specific protocols will depend on scientific priority and the availability of funds. Sites will be reimbursed to conduct the protocols on a capitated basis. The DCC will manage approval of clinical protocols by the PHN’s single IRB.

The number of protocols supported in the program will depend on the nature and extent of the investigations proposed and on available funds. Both short and long-term projects will be considered.

This FOA requires a Plan for Enhancing Diverse Perspectives (PEDP) as part of the application (see further below). The NIH Policy on Late Submission of Grant Applications states that failure to complete registrations in advance of a due date is not a valid reason for a late submission.

  • System for Award Management (SAM) Applicants must complete and maintain an active registration, which requires renewal at least annually. The renewal process may require as much time as the initial registration. SAM registration includes the assignment of a Commercial and Government Entity (CAGE) Code for domestic organizations which have not already been assigned a CAGE Code.
    • NATO Commercial and Government Entity (NCAGE) Code Foreign organizations must obtain an NCAGE code (in lieu of a CAGE code) in order to register in SAM.

Plan for Enhancing Diverse Perspectives (PEDP)

In an "Other Attachment" entitled "Plan for Enhancing Diverse Perspectives," all applicants must include a summary of strategies to advance the scientific and technical merit of the proposed project through expanded inclusivity. The PEDP should provide a holistic and integrated view of how enhancing diverse perspectives is viewed and supported throughout the application and can incorporate elements with relevance to any review criteria (significance, investigator(s), innovation, approach, and environment) as appropriate. Where possible, applicant(s) should align their description with these required elements within the research strategy section. The PEDP will vary depending on the scientific aims, expertise required, the environment and performance site(s), as well as how the project aims are structured. The PEDP may be no more than 1-page in length and should include a timeline and milestones for relevant components that will be considered as part of the review. Examples of items that advance inclusivity in research and may be part of the PEDP can include, but are not limited to:

Research Strategy: The DCC will have substantial responsibility for the performance of PHN research and awardees. Describe the planned approach to achieving the following:

  • Direct multi-center collaboration in pediatric and adult congenital and pediatric acquired heart disease clinical trials and studies;
  • Develop community engagement strategies for the PHN that encourage diversity, equity, and inclusion amongst the research and study participants.;
  • Manage and collaborate in the development, implementation, and monitoring of PHN protocols from inception through data dissemination;
  • Serve as a state-of-the-art research data management center, including developing and maintaining robust systems for electronic data capture, quality control, monitoring of protocol and site performance, and reimbursement of sites;
  • Support data integration activities for data from PHN studies combined with data from other sources, such as non-PHN registries and mobile health platforms;

  • Prepare and file all regulatory documents required for studies conducted under FDA or EMA auspices and hold INDs or IDEs as needed for specific trials;
  • Provide periodic recruitment data through the NHLBI Clinical Dashboard for all ongoing studies at intervals appropriate to the study;
  • Organize and provide necessary training for PHN protocols;
  • Conduct all necessary monitoring and quality assurance activities, including site visits as needed, and remote monitoring consistent with FDA guidance;
  • Provide a system for active monitoring of all PHN publications in progress, including regular reminders for authors and writing groups to ensure that PHN publication metrics are achieved;
  • Efficiently prepare public-use data sets and manage requests and inquiries related to data; and
  • Maintain and update the PHN website to include information about studies and summaries of study results.

Manage Core PHN operations

  • Organize and manage meetings and calls for the Steering Committee, Executive Committee, Data and Safety Monitoring Board, Protocol Development Committees and other committees essential to the conduct of the PHN;
  • Maintain the infrastructure necessary to support all PHN activities, including virtual meeting capabilities, electronic billing and payment capabilities, and maintenance of the public and administrative PHN websites and electronic data capture systems;
  • Describe flexible staffing approaches to respond to the changing needs and seasonal variation in work effort of the PHN;
  • Execute master site services agreements and protocol agreements with all CRCs including non-PHN auxiliary sites when needed;
  • Handle travel arrangements for consultants as needed;
  • Issue Requests for Proposals, coordinate selection and execute subcontracts for core labs and other resources as required by each study.

Applications are encouraged to describe special or unique strengths that may be relevant to the research conducted by the PHN. These can include systems, tools, or other resources that can be brought to bear to develop and expand the scientific productivity of the PHN. In addition, special administrative strengths or experience as well as participation in aspects of clinical research (IRB, DSMB, scientific board for clinical research, clinical research committees, etc.) for the PD/PI and additional staff members should be highlighted. Past levels of support for clinical trials should be described.

4. Submission Dates and Times

Part I. Overview Information contains information about Key Dates and times. Applicants are encouraged to submit applications before the due date to ensure they have time to make any application corrections that might be necessary for successful submission. When a submission date falls on a weekend or Federal holiday, the application deadline is automatically extended to the next business day.

Organizations must submit applications to Grants.gov (the online portal to find and apply for grants across all Federal agencies). Applicants must then complete the submission process by tracking the status of the application in the eRA Commons, NIH’s electronic system for grants administration. NIH and Grants.gov systems check the application against many of the application instructions upon submission. Errors must be corrected and a changed/corrected application must be submitted to Grants.gov on or before the application due date and time. Protections for Human Subjects

For research that involves human subjects but does not involve one of the categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.

2. Review and Selection Process

As part of the scientific peer review, all applications will receive a written critique.

Appeals of initial peer review will not be accepted for applications submitted in response to this FOA.

Applications will be assigned to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this FOA. Following initial peer review, recommended applications will receive a second level of review by the National Heart, Lung and Blood Advisory Council. The following will be considered in making funding decisions:

  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.

Section VI. Award Administration Information

1. Award Notices

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the recipient's business official.

Recipients must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

Any application awarded in response to this FOA will be subject to terms and conditions found on the Award Conditions and Information for NIH Grants website. This includes any recent legislation and policy applicable to awards that is highlighted on this website.

Institutional Review Board or Independent Ethics Committee Approval: Recipient institutions must ensure that protocols are reviewed by their IRB or IEC. In accordance with the statutory provisions contained in Section 872 of the Duncan Hunter National Defense Authorization Act of Fiscal Year 2009 (Public Law 110-417), NIH awards will be subject to the Federal Awardee Performance and Integrity Information System (FAPIIS) requirements. FAPIIS requires Federal award making officials to review and consider information about an applicant in the designated integrity and performance system (currently FAPIIS) prior to making an award. Cooperative Agreement Terms and Conditions of Award

The following special terms of award are in addition to, and not in lieu of, otherwise applicable OMB administrative guidelines, HHS grant administration regulations at 45 CFR Part 75 and other HHS, PHS, and NIH grant administration policies.

The administrative and funding instrument used for this program will be the cooperative agreement, an "assistance" mechanism (rather than an "acquisition" mechanism) in which substantial NIH programmatic involvement with the awardees is anticipated during the performance of the activities. Under the cooperative agreement, the NIH purpose is to support and stimulate the recipients' activities by involvement in and otherwise working jointly with the award recipients in a partnership role; it is not to assume direction, prime responsibility, or a dominant role in the activities. Consistent with this concept, the dominant role and prime responsibility resides with the awardees for the project as a whole, although specific tasks and activities may be shared among the awardees and the NIH as defined below.

The PD(s)/PI(s) will have the primary responsibility for:

Protocol development and subsequent amendments, recruitment materials, preparing protocol budgets in collaboration with the CRCs, monitoring recruitment of study participants, assuring the quality of study participant protocol adherence, assuring the accurate and timely transmission of data, analyzing and interpreting data, preparing publications and public-use datasets, working with the CRCs and NHLBI to disseminate research findings, and activities as outlined in the Research Strategy (Section IV).

Monitoring and managing support or other involvement of industry or any other third party in a PHN study, e.g., including contracting and data use agreements, any required regulatory activities, required reporting, and any other requirements.

NIH staff have substantial programmatic involvement that is above and beyond the normal stewardship role in awards, as described below:

NIH Project Scientists will have substantial programmatic involvement that is above and beyond the normal stewardship role in awards, as described below.

The NHLBI Project Scientists will assist with development of research protocols, monitor patient recruitment and study progress, ensure disclosure of conflicts of interest, and ensure adherence to NHLBI policies. NHLBI is responsible for appointments to the PHN’s standing Data and Safety Monitoring Board (DSMB).

The NHLBI Project Scientists will serve on the Executive and Steering Committees and other study committees, when appropriate, and will have one collective vote. The NHLBI Project Scientists will work with recipients on issues coming before the Steering Committee such as recruitment, protocol development, follow-up, quality control, adherence to protocol, possible changes to the protocol, interim data and safety monitoring, final data analysis and interpretation, preparation of publications, and development of solutions to major problems.

In addition to the Project Scientist, a separate NHLBI Program Official will be responsible for the normal program stewardship of the cooperative agreement, and will be named in the Notice of Award. Final decision-making authority on matters of budgetary and funding actions, grants management actions, and management of intellectual property issues is assigned to NHLBI staff other than the Project Scientist. The responsibility for final decision making may reside with Senior Institute management.

The NHLBI reserves the right to phase-out or curtail the study (or an individual award) in the event of (a) failure to develop or implement mutually agreeable collaborative protocols; (b) substantial shortfall in participant recruitment, follow-up, data reporting, or quality control; (c) major breach of the protocols or substantive changes in the agreed-upon protocols with which NHLBI cannot concur; (d) attaining of a major study endpoint before schedule with persuasive statistical significance; or (e) human subject ethical issues that may dictate a premature termination.

Areas of Joint Responsibility include:

Governance of the PHN through the Executive Committee. This Committee has primary responsibility for identification of priority areas for research, management of the PHN’s policies and procedures, oversight of the PHN budget, and general PHN operations. Executive Committee membership consists of all Principal Investigators, the chairs of select PHN Committees, a Study Coordinator, and the NHLBI Project Scientists. Each member has one vote; the NHLBI staff has one collective vote.

Negotiation of mutually agreed upon performance and accrual milestones during the development of each clinical research protocol.

Support of the PHN’s DSMB. The DSMB has been appointed by NHLBI to provide overall monitoring of interim data and safety issues. An NHLBI scientist, other than the NHLBI Project Scientist, serves as Executive Secretary to the DSMB, and is responsible for on-boarding new members, managing conflict of interest, and providing a summary of the recommendations from each meeting to the DCC. The DCC is responsible for preparing concise DSMB reports, and all DSMB meeting support. The lead principal investigator of each PHN protocol presents updates to the DSMB a minimum of twice a year.

Dispute Resolution:

Any disagreements that may arise in scientific or programmatic matters (within the scope of the award) between award recipients and the NIH may be brought to Dispute Resolution. A Dispute Resolution Panel composed of three members will be convened. It will have three members: a designee of the Steering Committee chosen without NIH staff voting, one NIH designee, and a third designee with expertise in the relevant area who is chosen by the other two; in the case of individual disagreement, the first member may be chosen by the individual awardee. This special dispute resolution procedure does not alter the awardee's right to appeal an adverse action that is otherwise appealable in accordance with PHS regulations 42 CFR Part 50, Subpart D and HHS regulations 45 CFR Part 16.

3. Data Management and Sharing

Note: The NIH Policy for Data Management and Sharing is effective for due dates on or after January 25, 2023.

Consistent with the NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described.

4. Reporting

When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.

A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement. NIH FOAs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 45 CFR Part 75.301 and 2 CFR Part 200.301.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for recipients of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All recipients of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. See the NIH Grants Policy Statement for additional information on this reporting requirement.

In accordance with the regulatory requirements provided at 45 CFR 75.113 and Appendix XII to 45 CFR Part 75, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313). As required by section 3010 of Public Law 111-212, all information posted in the designated integrity and performance system on or after April 15, 2011, except past performance reviews required for Federal procurement contracts, will be publicly available.
