Section I. Funding Opportunity Description
1.
Research Objectives
Under this Funding Opportunity Announcement (FOA), the
National Institute on Drug Abuse requests research grant applications to
examine the mechanisms of interaction between licit
or illicit drugs of abuse, processes of addiction, or medications to treat drug
abuse, and events related to the etiology and pathogenesis of HIV-associated
neurologic complications. Successful research projects would utilize in vitro, non-human primate, small animal, or human studies and
would contribute to a better understanding of the role of drugs of abuse in HIV
neuropathogenesis and disease progression, including the relationship(s) of
immunologic, virologic, host, pharmacologic, and
environmental factors to HIV-associated central nervous system (CNS)
dysfunction underlying HIV-Associated Dementia, minor cognitive/motor disorder
(MCMD), and subclinical impairment. Collaboration among researchers in
different fields relevant to these interdisciplinary
topics is highly encouraged.
Background
Despite reduced incidence of HIV-associated dementia (HAD), the prevalence of neurologic complications of AIDS continues to increase as patients taking highly-active antiretroviral therapy (HAART) are living longer with HIV infection. Intravenous drug use accounts for 25% of HIV-1 infections in the U.S., and non-injection drug use (including stimulants, club drugs and pain medications) is believed to be highly prevalent in the HIV-infected population. Although there is controversy about whether drug use affects the progression of AIDS, there is emerging evidence suggesting a relationship between drug use, particularly stimulants, and accelerated and/or more severe HIV-associated neurologic complications. Whether this interaction is mediated by effects on viral entry into the CNS, changes in the neural environment that affect viral replication, chronic processes that contribute to disease progression in the brain, or altered susceptibility of neural cells to damage is unknown. Certain coinfections or opportunistic infections common among HIV-infected intravenous drug users also may exacerbate neurologic impairment or damage compared to HIV alone in some patients. Given the wide range of individual differences in HIV-associated neurological disease progression, there is a need to understand both viral and host factors that determine whether the outcome will be pathogenic vs. non-pathogenic in the presence or absence of various illicit or licit drugs of abuse and HAART.
A combination of in vitro or in vivo animal and human neuroscience and neuroimmunology research is needed to explore the underlying basic mechanisms of chronic HIV-associated neurological diseases in the context of drug abuse (e.g. neuroinflammation, changes in blood brain barrier permeability, glial activation or dysfunction, neurotoxicology, neuroprotection, or drug-drug interactions). In particular, NIDA encourages the development or validation of appropriate in vitro or animal models that allow the concurrent study of HIV, SIV or other lentivirus infection in the brain and specific patterns of chronic or acute drug exposure, in ways that reflect issues of clinical relevance, including gender differences.
Hypothesis-driven research examining the biological impact of drugs of abuse on chronic HIV-associated neurologic disease has been limited in part due to complex confounding issues in human studies of HIV-infected drug users, limited availability and high costs of non-human primates for research, and a paucity of suitable small animal or in vitro model systems that sufficiently model HIV/AIDS. In addition, until recently there has been little scientific interaction among researchers in the areas of HIV/AIDS, drug abuse, neurology, neurobiology, pharmacology, toxicology, glial inflammation, immunology, molecular genetics/genomics, and behavior, each of which can bring perspective to this overarching theme. NIDA expects that a better understanding of the basic mechanisms of HIV-associated neuropathogenesis and neuronal/glial dysfunction in the context of substance use/abuse ultimately will lead to the development of CNS-targeted treatments for neurological complications of AIDS in patients who use drugs.
The primary objective for this Funding Opportunity Announcement (FOA) is to encourage a broad range of creative, innovative studies that will increase collaboration among researchers in areas such as drug abuse, virology, neurology, neurobiology, immunology, and/or neuroinflammation, in ways that will:
1. define biological interactions between exposure to addictive substances (focusing on signaling pathways, neural circuits, and neural adaptations) with viral replication and entry into the CNS, HIV-associated neuropathogenesis, glial activation and inflammation, subsequent neuronal dysfunction and damage, and the functional, structural, and neurochemical consequences of these neuronal perturbations;
2. identify viral or host factors or signaling pathways in humans or animals that prevent or exacerbate neurological consequences of HIV/SIV infection, and address how the effects of various substances, separately or combined, might modulate these processes;
3. utilize existing resources (such as genomic/proteomic databases, animal repositories, brain banks that include confirmed information on neuropsychological status and drug abuse history for HIV-infected individuals, pre-established research networks for platform studies, or data from networks for secondary analysis) in novel ways to address interactions between drug abuse and HIV infection; and
4. advance the understanding of basic mechanisms of HIV-associated neurological disease progression and CNS degeneration and dysfunction in HIV-infected persons who use or abuse various substances.
Examples of research questions to be addressed by multidisciplinary approaches include but are not limited to:
Mechanisms of addictive drug actions on blood-brain barrier integrity and cell phenotype, as releted to leukocyte trafficking, HIV/SIV neuroinvasion, viral replication, and/or progression of neurologic disease, using in vivo as well as in vitro approaches.
Mechanisms by which the combination of HIV/AIDS and substance abuse (and/or related neuropeptide/neurotransmitter systems) may lead to dysregulation of glial cell functions, neural progenitor cell function, glial-neuronal signaling and homeostasis (e.g. glutamate transport, cytokine release, growth factor release, or reactive oxygen/nitrogen species production).
Changes in gene transcription, translation and function in neurons and glia resulting from HIV/SIV infection in the brain in the context of substance abuse, as well as genetic differences contributing to enhanced/repressed transcription or translation in affected host cells.
Mechanistic studies of neurologic disease progression in the presence of licit or illicit substances of abuse and HIV/SIV, particularly in the context of gender differences, co-infections or opportunistic infections (e.g. HCV, TB or T. gondii).
Studies to evaluate or model interactions of chronic substance abuse, HIV infection, and pathogenesis of comorbid pain syndromes (e.g. due to coinfections or peripheral neuropathy) with respect to pain treatment strategies.
Studies to determine how drug administration affects immune responses that lead to neurotoxicity or neuroprotection in the context of HIV infection (e.g. Immune Reconstitution and Inflammation Syndrome (IRIS)).
Studies that define pharmacokinetic, pharmacodynamic, and pharmacogenetic interactions between CNS-active AIDS treatments (e.g., neuroactive antiretrovirals, minocycline) and licit or illicit substances of abuse.
Structural, functional and/or neurochemical imaging studies, or other types of clinical or basic neuroscience studies, designed to identify brain regions and systems that exhibit relatively greater or lesser sensitivity to HIV infection as well as to assess the impact of co-occurring substance abuse on disease progression, including neuropsychological or cognitive impairment, and/or response to antiviral treatment of HIV in the context of drug abuse.

II. Award Information
1. Mechanism of Support
This Funding Opportunity Announcement (FOA)
will use the R21 award
mechanism. The applicant will be
solely responsible for planning, directing, and executing the proposed project.
This FOA uses Just-in-Time information concepts. It also uses the modular and non-modular budget formats (see the Modular Applications and Awards section of the NIH Grants Policy Statement. Specifically, if you are submitting an application with direct costs in each year of $250,000 or less (excluding consortium Facilities and Administrative [F&A] costs), use the PHS398 Modular Budget component provided in the SF424 (R&R) Application Package and SF424 (R&R) Application Guide (see specifically Section 5.4, Modular Budget Component, of the Application Guide).

2. Funds Available
Because the nature and scope of the proposed research will vary from
application to application, it is anticipated that the size and duration of
each award will also vary. Although the financial plans of the Institutes and
Centers (ICs) provide support for this program, awards pursuant to this funding
opportunity are contingent upon the availability of funds and the submission of
a sufficient number of meritorious applications.
The total project period for an application submitted in response to this funding opportunity may not exceed 2 years. Although the size of award may vary with the scope of research proposed, it is expected that applications will stay within the budgetary guidelines for an exploratory/developmental project. Direct costs are limited to $275,000 over a two-year period, with no more than $200,000 in direct costs allowed in any single year. Applicants may request direct costs in $25,000 modules, up to the total direct costs limitation of $275,000 for the combined two-year award period.
The participating organization(s) NIDA intends to commit approximately $3 million dollars in FY2007 to fund 5-10 applications for this FOA and its companion R01 FOA.

Pre-Award Costs are allowable. A grantee may, at
its own risk and without NIH prior approval, incur obligations and expenditures
to cover costs up to 90 days before the beginning date of the initial budget
period of a new award if such costs: are necessary to conduct the project, and
would be allowable under the grant, if awarded, without NIH prior approval. If
specific expenditures would otherwise require prior approval, the grantee must
obtain NIH approval before incurring the cost. NIH prior approval is required
for any costs to be incurred more than 90 days before the beginning date of the
initial budget period of a new award.
The incurrence of pre-award costs in anticipation of a
competing or non-competing award imposes no obligation on NIH either to make
the award or to increase the amount of the approved budget if an award is made
for less than the amount anticipated and is inadequate to cover the pre-award
costs incurred. NIH expects the grantee to be fully aware that pre-award costs
result in borrowing against future support and that such borrowing must not
impair the grantee's ability to accomplish the project objectives in the

The goals of NIH supported research are to advance our understanding of biological systems, to improve the control of disease, and to enhance health. In their written critiques, reviewers will be asked to comment on each of the following criteria in order to judge the likelihood that the proposed research will have a substantial impact on the pursuit of these goals. Each of these criteria will be addressed and considered in assigning the overall score, weighting them as appropriate for each application.
Note that an application does not need to be strong in all
categories to be judged likely to have major scientific impact and thus deserve
a high priority score. For example, an investigator may propose to carry out
important work that by its nature is not innovative but is essential to move a
field forward.
Significance: Does this study address an
important problem? If the aims of the application are achieved, how will
scientific knowledge or clinical practice be advanced? What will be the effect
of these studies on the concepts, methods, technologies, treatments, services,
or preventative interventions that drive this field?
Approach: Are the conceptual or clinical
framework, design, methods, and analyses adequately developed, well integrated,
well reasoned, and appropriate to the aims of the project? Does the applicant
acknowledge potential problem areas and consider alternative tactics? For applications designating
multiple PDs/PIs, does the Leadership Plan ensure that there will be sufficient
coordination and communication among the PDs/PIs? Are the administrative
plans for the management of the research project appropriate, including plans
for resolving conflicts?
Innovation: Is the project original and innovative? For example: Does
the project challenge existing paradigms or clinical practice; address an
innovative hypothesis or critical barrier to progress in the field? Does the
project develop or employ novel concepts, approaches, methodologies, tools, or
technologies for this area?
Investigators: Are the investigators appropriately trained and well
suited to carry out this work? Is the work proposed appropriate to the
experience level of the principal investigator and other researchers? Does the
investigative team bring complementary and integrated expertise to the project
(if applicable)? This FOA encourages collaborations among
PD/PIs in different disciplines that contribute to an understanding of interactions between drug exposure and

Protection of Human Subjects from Research Risk: The involvement of human subjects and protections from research risk relating

Inclusion of Women, Minorities and Children in Research: The adequacy of
plans to include subjects from both genders, all racial and ethnic groups (and
subgroups), and children as appropriate for the scientific goals of the

of the subjects or the purpose of the research. This policy results from the
