Through this Notice of Funding Opportunity (NOFO), the National Cancer Institute intends to support basic and translational research that will guide the development of a prophylactic or therapeutic Kaposi sarcoma herpesvirus (KSHV) vaccine. Section I. Notice of Funding Opportunity Description

Purpose

Through this Notice of Funding Opportunity (NOFO), the National Cancer Institute (NCI) intends to support basic and translational research that will guide the development of a prophylactic or therapeutic Kaposi sarcoma herpesvirus (KSHV) vaccine. A prophylactic KSHV vaccine could prevent primary KSHV infection, transmission, and subsequent development of KS and other KSHV-associated syndromes such as multicentric Castleman disease (MCD), primary effusion lymphoma (PEL), and KSHV inflammatory cytokine syndrome (KICS), or ameliorate the severity of disease. A therapeutic KSHV vaccine could be helpful in preventing or treating KSHV disease in people already infected with KSHV. This NOFO builds on the research of, and leverages the gains made through, the NOFO, Investigation of the Transmission of Kaposi Sarcoma-Associated Herpesvirus (KSHV) (RFA-CA-18-013 and RFA-CA-20-046).

Clinical trials may be proposed that forward the development of a prophylactic or therapeutic KSHV vaccine.

The U01 research projects funded through this cooperative agreement NOFO will comprise the KSHV Vaccine Development Consortium (KVDC) between investigators and NCI program staff. Since the funding instrument is a cooperative agreement, there will be substantial NCI programmatic involvement with the recipients. Activities will include the formation of a coordination committee composed of PIs/MPIs/co PIs of all U01s and NCI program staff. There will be occasional conference calls to assist ongoing research efforts and an annual meeting to present results and discuss key research findings (see Section VI.2 form more information).

Background

Kaposi sarcoma (KS) remains one of the most common malignancies worldwide in people living with HIV. Kaposi sarcoma herpesvirus (KSHV), a gammaherpesvirus that is also called human herpesvirus-8 (HHV-8), is the necessary causal agent for KS. Other factors, such as immunosuppression, have a profound effect on the likelihood that a KSHV-infected individual will develop KS. In people with HIV (PWH), KSHV infection is much more likely to lead to the development of KS, and it can also cause several other KSHV-associated diseases, including a form of multicentric Castleman disease (MCD), primary effusion lymphoma (PEL), and KSHV inflammatory cytokine syndrome (KICS).

Specific mechanisms of KSHV transmission, and the means for person-to-person spread in various settings are incompletely understood. KSHV is often found in oral fluids, and this is believed to be the most common conduit of transmission. In sub-Saharan Africa, where KSHV seroprevalence remains high, children are often infected early in life and KSHV is thought to be mainly transmitted through oral fluid exposure, often from close family members. In Europe and North America, KSHV seroprevalence is low (<5%) in the general population. However, men who have sex with men (MSM) and MSM with HIV (MSMH) have high rates (20-30% and 30-65%, respectively). Again, specific mechanisms of transmission are unclear, but likely involve oral fluid transmission through the oral-anal or oral-oral routes. To better understand modes of KSHV transmission, NCI developed a funding opportunity, Investigation of the Transmission of KSHV (RFA-CA-18-013; RFA-CA-20-046). Advances from this initiative and previous NCI investments in KSHV research are yielding valuable insights that can inform development of a KSHV vaccine.

Currently, there is no vaccine for KSHV, nor a coordinated effort to develop a KSHV vaccine. However, the rapid development of the SARS-CoV-2 vaccine, success in cancer prevention with vaccines against human papillomaviruses (HPV) and hepatitis B virus (HBV), lessons learned from the Epstein-Barr virus (EBV) vaccine development process, and continued progress in our understanding of KSHV transmission, infection, and KSHV-associated immunology, put the field at an important inflection point. Prior research has yielded important data on several key biological aspects of KSHV that predict that development of a vaccine against this gammaherpesvirus is feasible: First, KSHV is not common in some populations, and prevalence is restricted to certain geographic regions and populations at high risk for infection and transmission, which would allow for targeted vaccine implementation. Second, KSHV has evolved highly conserved structural genes that are susceptible to immune surveillance. Third, the KSHV genome is evolutionarily stable. Given these attributes, it is anticipated that a single vaccine, based on the fully sequenced KSHV genome, could be effective against all variants in different geographic regions.

KSHV’s predominant presence and proposed transmission in oral fluids suggests pursuit of a vaccine that will generate mucosal immunity. Basic and clinical KSHV research in the context of HIV-coinfection have revealed invariant KSHV antigens capable of eliciting potent neutralizing antibodies and/or cell-mediated immune responses. Existing paradigms of vaccine development and platforms of vaccine delivery allow rapid assessment of product profiles that best fit the KSHV/KS requirements. Finally, while a vaccine to prevent KSHV infection and transmission is a primary goal, prevention of secondary tumorigenesis after infection with KSHV would certainly be a desirable endpoint.

All awarded U01s will become part of the KVDC, which will serve to build a stronger community of investigators working toward development of a KSHV vaccine. The consortium has been proposed to foster dissemination of new advances and discussion of challenges and opportunities for collaboration on shared goals aligned with development of a KSHV vaccine.

Objectives and Scope of this NOFO

This NOFO is designed to support basic and translational research that will guide the development of a prophylactic or therapeutic KSHV vaccine through a cooperative group, the KSHV Vaccine Development Consortium (KVDC). The key objectives of prophylactic vaccine development would be to prevent primary KSHV infection, transmission and subsequent development of KS and other KSHV-associated syndromes (MCD, PEL and KICS). Therapeutic KSHV vaccine development could prevent or treat KSHV disease in people already infected with KSHV.

In considering meritorious applications, an attempt will be made to fund a portfolio that addresses the spectrum of issues related to KSHV vaccine development, with both prophylactic vaccines and/or therapeutic vaccines considered appropriate approaches. Applications that expand the current knowledge base to adequately inform strategies for development, evaluation, and targeted implementation and distribution of vaccine candidates in marginalized, stigmatized and socio-economically disadvantaged populations, and communities who tend to bear the greatest burden of preventable disease, are strongly encouraged.

Projects proposed in response to this NOFO must address research on KSHV, KS and other KSHV-associated diseases and must be focused on supporting basic and translational research that will guide the development of a prophylactic or therapeutic KSHV vaccine.

Examples of research areas that are appropriate for the projects to be proposed under this NOFO include the following:

  • Studies to develop a prototype KSHV vaccine and test its activity in relevant cellular or animal model systems.
  • Studies to identify and evaluate KSHV structural and non-structural targets for a potential KSHV vaccine.
  • Research to develop animal models to study a prototype KSHV vaccine or vaccines.
  • Studies to assess how the efficacy of a promising KSHV vaccine can be optimized for people with HIV (PWH).
  • Research to better define the initial steps of infection with KSHV and the primary means of person-to-person transmission in different populations that can be targeted with a vaccine.
  • Research to optimize and/or standardize KSHV detection methods.

Non-responsive Applications

The following types of activities remain outside the scope of this NOFO, and applications proposing them are non-responsive to this NOFO and will not be reviewed.

  • Projects that fail to choose a specific topic that addresses KSHV or KS basic and translational research that will guide development of a vaccine.
  • Projects that focus solely on KSHV virology, cellular transformation, and disease pathogenesis and are not related to KSHV vaccine development.
  • Projects that are not based on substantial preliminary data.

Funding Instrument

Cooperative Agreement: A support mechanism used when there will be substantial Federal scientific or programmatic involvement. NCI intends to commit an estimated total of $3,000,000 in each FY (2024 and 2025) to fund 3 to 4 awards for each receipt date.

It is anticipated that most application budgets should not exceed $500,000 direct costs per year. However, with a strong justification, a budget can be up to $750,000 direct costs per year. A proposed Clinical Trial application may include study design, methods, and intervention that are not by themselves innovative but address important questions or unmet needs. Additionally, the results of the clinical trial may indicate that further clinical development of the intervention is unwarranted or lead to new avenues of scientific investigation.

Overall Impact

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).

Reviewers will consider each of the review criteria below in the determination of scientific merit and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. Protections for Human Subjects

2. Review and Selection Process

Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s) convened by NCI, in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications will receive a written critique.

Applications may undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.

Cooperative Agreement Terms and Conditions of Award

The following special terms of award are in addition to, and not in lieu of, otherwise applicable U.S. Office of Management and Budget (OMB) administrative guidelines, U.S. Department of Health and Human Services (HHS) grant administration regulations at 2 CFR Part 200, and other HHS, PHS, and NIH grant administration policies.

The administrative and funding instrument used for this program will be the cooperative agreement, an "assistance" mechanism (rather than an "acquisition" mechanism), in which substantial NIH programmatic involvement with the recipients is anticipated during the performance of the activities. Under the cooperative agreement, the NIH purpose is to support and stimulate the recipients' activities by involvement in and otherwise working jointly with the recipients in a partnership role; it is not to assume direction, prime responsibility, or a dominant role in the activities. Consistent with this concept, the dominant role and prime responsibility resides with the recipients for the project as a whole, although specific tasks and activities may be shared among the recipients and NIH as defined below.

The PD(s)/PI(s) will have the primary responsibility to define objectives and approaches, and to plan, conduct, analyze and publish results, interpretations, and conclusions of studies conducted under this program. The PD(s)/PI(s) assume responsibility and accountability to the applicant organization officials and to the NCI for the performance and proper conduct of the research supported by the KSHV Vaccine Development Consortium (KVDC) program in accordance with these terms and conditions of the award.

Specific PD(s)/PI(s) responsibilities and rights will be to:

  • Define research goals, determine and/or approve experimental approaches, and set project milestones and timelines;
  • Oversee the conduct of research including such aspects as experimental activities, data collection, quality control, interim data, safety monitoring, final data analysis and interpretation and preparation of results for publication;
  • Oversee, direct, and coordinate scientific and administrative activities of the KVDC through participation on the KVDC Coordinating Committee;
  • Serve as a voting member on the KVDC Coordinating Committee;
  • Coordinate and encourage inter-consortium collaborations;
  • Participate in various task-oriented working groups together with members from other KVDC projects that might be formed, as needed, e.g., as subcommittees of the KVDC Coordinating Committee;
  • Accept and implement all KVDC Coordinating Committee-approved scientific and KVDC network policy recommendations to the extent consistent with applicable grant regulations;
  • Ensure the timely sharing of KVDC generated data across the entire KVDC program as appropriate, and upon request from NCI Program Staff;
  • Prepare for annual administrative evaluations by KVDC Team Members and NCI Program Staff;
  • Participate in external evaluation processes of the KVDC program coordinated by NCI Program Staff;
  • Provide other information as might be requested by Project Scientist (e.g., on annual Center milestones, goals for throughput, procedures, etc.)
  • Monitor the completion of established goals and benchmarks within the time-frame and budget proposed;
  • Participate in the annual, in person, PD/PI meeting organized by the KVDC;
  • Work closely with the NCI Program Officials or Project Scientist (see below) on the coordination and management of KVDC program activities. These actions involve (but will not be limited to) participation in meetings, working groups, and teleconferences with KVDC scientific and program staff and other awarded programs and investigators, as needed;
  • Ensure that proper process management methods are executed for planning, monitoring, and managing the workload over the award period, and are expected to provide update reports to NCI Program Staff upon request.

Recipients will retain custody of and have primary rights to the data and software developed under these awards, subject to Government rights of access consistent with current HHS, PHS, and NIH policies.

NIH staff have substantial programmatic involvement that is above and beyond the normal stewardship role in awards, as described below. The substantially involved NCI program staff member(s), acting as Project Scientist(s), will coordinate, in a centralized fashion, various activities of the recipients.

Specific responsibilities of the NCI Project Scientist(s) will include the following aspects:

  • Serve as voting members of the KVDC Coordinating Committee and its subcommittees as appropriate;
  • In collaboration with the PDs/PIs and the KVDC Coordinating Committee, ensure the KVDC program functions as a unified whole to achieve the goals of the program;
  • Assist in avoiding unwarranted duplications of effort across the KVDC recipients;
  • Review the progress of individual KVDC recipients and specific activities shared among the recipients;
  • Assist the recipients as a resource in stimulating their broader interaction with other NCI and NIH programs, including non-NIH programs if appropriate, to disseminate results and outcomes from the KVDC program and effectively leverage existing NIH/NCI resources and infrastructure;
  • Co-organize and participate in the annual, in person, meeting of KVDC investigators;
  • Organize and conduct regular meetings to share progress either by teleconference, videoconference, or face-to-face, as needed among the recipients;
  • Ensure the availability of data and related resources developed during the course of the KVDC program to the scientific community at large;
  • Participate in data analyses, interpretations, and co-authorship of KVDC publications as appropriate and jointly agreed through the KVDC Coordinating Committee;
  • Ensure that KVDC Coordinating Committee-approved decisions, recommendations, and KVDC network policies are consistent with applicable grant regulations;
  • Provide technical assistance and advice to the recipients as appropriate (e.g., interpretation and reporting of the collected information);
  • Conduct periodic evaluations, including site visits if appropriate, for scientific and organizational discussions with the recipient research teams, observation of field data collection and management techniques, and other resource-related matters;
  • Facilitate interactions/collaboration between the recipients and other NCI/NIH-supported programs, investigators, or organizations that may contribute to the KVDC goals; Serve as a liaison between the KVDC recipients and NCI/NIH staff members and investigators that may provide additional expertise and/or resources to the program.

Additionally, an NCI Program Staff member acting as a Program Official will be responsible for the normal scientific and programmatic stewardship of the award and will be named in the award notice.

The NCI reserves the right to adjust funding, withhold, suspend, or terminate support to the recipient if the team is unable to meet the performance requirements set forth in the required policies and procedures.

The KVDC Coordinating Committee will serve as the primary governing body of the KVDC program. The Coordinating Committee will be jointly established by all the awarded PDs/PIs of the KVDC and the NCI Program Staff members. The KVDC Coordinating Committee will provide strategic coordination for the activities of the KVDC recipients and the KVDC program overall.

  • The PDs/PIs of each awarded KVDC project (or senior investigator as proxy when applicable) will collectively have one vote; and
  • The NCI Project Scientists will collectively have one vote for NCI.

Non-Voting Members:

  • The KVDC Coordinating Committee may decide to add non-voting, ancillary members as needed, e.g., representatives from NCI/NIH-supported research resources, scientific experts, and/or patient advocates.

A PD/PI representing one of the projects will be selected by the Committee to serve as a chairperson on a rotating basis following award issuance. All Coordinating Committee decisions and recommendations that require voting will be based on a majority vote.

It is anticipated that the KVDC Coordinating Committee will formulate strategic decisions and policies for program-wide activities. The KVDC recipients will be required to accept and implement these decisions and policies to the extent consistent with applicable grant regulations. The activities of the KVDC Coordinating Committee are expected to include the following:

  • Establishing operational guidelines, quality control procedures, and policy consistency to meet the goals of the KVDC program, and monitoring the implementation of these guidelines and policies;
  • Developing quality assurance goals for all partnership functions, specimens, technology, and data for the KVDC program;
  • Following up on action items from the KVDC Coordinating Committee meetings in a timely fashion;
  • Organizing annual, in person meetings of KVDC investigators;
  • Establishing subcommittees, if needed, composed of a set number of members of the KSHV Coordinating Committee or their nominees, NIH/NCI Program Staff members, and additional scientific experts as deemed necessary and appropriate to accomplish KVDC goals.

Dispute Resolution:

Any disagreements that may arise in scientific or programmatic matters (within the scope of the award) between recipients and NIH may be brought to Dispute Resolution. A Dispute Resolution Panel composed of three members will be convened: a designee of the Steering Committee chosen without NIH staff voting, one NIH designee, and a third designee with expertise in the relevant area who is chosen by the other two; in the case of individual disagreement, the first member may be chosen by the individual recipient. This special dispute resolution procedure does not alter the recipient's right to appeal an adverse action that is otherwise appealable in accordance with PHS regulation 42 CFR Part 50, Subpart D and HHS regulation 45 CFR Part 16.

3. Data Management and Sharing

Note: The NIH Policy for Data Management and Sharing is effective for due dates on or after January 25, 2023.

Consistent with the NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described.

4. Reporting

When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.

A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement. NIH NOFOs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 45 CFR Part 75.301 and 2 CFR Part 200.301.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for recipients of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All recipients of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. See the NIH Grants Policy Statement for additional information on this reporting requirement.

In accordance with the regulatory requirements provided at 45 CFR 75.113 and Appendix XII to 45 CFR Part 75, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313). As required by section 3010 of Public Law 111-212, all information posted in the designated integrity and performance system on or after April 15, 2011, except past performance reviews required for Federal procurement contracts, will be publicly available. Full reporting requirements and procedures are found in Appendix XII to 45 CFR Part 75 Award Term and Conditions for Recipient Integrity and Performance Matters.
