Section I. Funding Opportunity Description

Purpose

Through this Funding Opportunity Announcement (FOA), the National Cancer Institute (NCI) invites applications to support planning and feasibility studies to integrate health disparity research into immuno-oncology studies. The P20 mechanism enables complementary, multi-disciplinary research teams to address interdisciplinary research questions at the nexus of cancer health disparities and cancer immunology. It is anticipated that these feasibility or pilot studies will enable the exploration of novel or high-risk research hypotheses.

Key Definitions for this FOA

  • Cancer Health Disparities: Adverse differences among certain population groups in cancer measures, such as: incidence, prevalence, morbidity, mortality, survivorship, quality of life, the burden of cancer or related health conditions, screening rates, and stage at diagnosis. The causes of cancer disparities are multifactorial and complex with direct influence by structural inequalities and societal injustices. These population groups may be characterized by gender, age, race, ethnicity, ancestry, gender and sexual identity, education, income, social class, disability, geographic location, or other characteristics.
  • Immuno-Oncology (IO): Cancer immunology, cancer immunotherapy, and/or cancer immunoprevention. This includes basic biological mechanisms underpinning anti-tumor immune responses, immune regulation of the development and spread of tumors, and approaches to improve immune targeting and destruction of cancer cells.

Background

Integrating cancer health disparities research throughout the cancer research continuum is a high priority for both the NCI's Equity and Inclusion Program and the NIH’s UNITE Initiative. NCI has established a robust developmental pipeline of cancer health disparities research programs broadly ranging from basic research in cancer biology through translational health disparities research, including the generation of novel, authenticated animal models appropriate for advancing disparities research. While access to care and quality of treatment are known drivers of cancer health disparities, there is a complementary and critical need to investigate how biology interacts with the many complex and interrelated contributors to cancer health disparities. This research priority will advance health disparities research toward more specific disciplines in cancer research.

Despite recent advances in immuno-oncology (IO) research to utilize the immune system to eliminate multiple types of cancer, there remains an unmet potential to address cancer health disparities research throughout the IO research spectrum. Although cancer immunotherapy holds tremendous promise for improving patient outcomes, including cancers with a higher burden among underserved populations, this initial research has been predominantly focused on individuals of European ancestry, with a significant lack of data from racial and ethnic minorities. The pilot research studies invited through this FOA, aim to unite health disparity and IO research communities by supporting the integration of cancer health disparities research with the growing immuno-oncology and immunotherapy research fields.

Research Objectives, Scope, and Requirements

Research Scope

The overarching scope of this FOA is to integrate cancer health disparities research throughout the NCI immuno-oncology research continuum by encouraging basic research focused at the intersection of health disparities and immuno-oncology research. This is a particularly challenging research area requiring multi-disciplinary approaches that can incorporate (a) the complex and overlapping biological and immunological factors underlying disparities; (b) difficulties to access sufficiently powered and/or well-curated tissues; (c) lower numbers of underrepresented groups recruited into clinical trials; and (d) overall incidence and mortality gaps that persist between racial and ethnic populations across different cancer types despite accounting for socioeconomic and access to care factors - into basic cancer immunology studies. The research scope includes but is not limited to a) pilot or feasibility studies to test exploratory or novel hypotheses on underlying biological differences in immune mechanisms, immune response, and/or treatment response/resistance, b) initial studies to establish sufficiently powered and/or well-curated specimens from groups under-represented in clinical trials, and/or c) planning studies to build collaborations/teams, generate resources (e.g., tools or reagents), or other collaborative research infrastructure.

Research Objectives

The scientific research objectives are to integrate health disparities research into immuno-oncology research. This research will require elucidating underlying genetic, inflammatory, metabolic, and immune profiles, and/or distinct tumor immune microenvironments across racial/ethnic minorities or other underserved populations. There is a breadth of scientific opportunities to characterize disparities in immunotherapy response, resistance, and immune-related adverse events. Specific research project examples may include (a) investigating differential immune signatures in the tumor microenvironment among racial/ethnic populations; (b) studying innate immune variants that differ across racial/ethnic populations conferring cancer risk or protection for different cancers; (c) characterizing unique pseudogene expression and tumor immune infiltration among population groups; (d) quantifying distinct antigen levels and cancer risk across multi-ethnic populations; and (e) developing innate immune response-related animal models that capture the diversity and are appropriate to investigate cancer health disparities.

The overall scientific objectives of the P20 projects are to integrate health disparities into immuno-oncology research through multi-disciplinary research projects with complementary strengths in both cancer health disparities and IO research. The P20 projects specifically require 1) investigation of under-represented, under-served, and/or racial/ethnic minority populations (e.g., American Indian/Alaska Natives, Asians, African Americans, Native Hawaiians/Pacific Islanders, Hispanic/Latinos, and/or sex and gender minorities); and 2) multi-disciplinary teams with complementary expertise in both cancer health disparities and immuno-oncology research. Given that the causes of cancer health disparities are multifactorial and complex with influence by structural inequalities and societal injustices, it is anticipated the P20 research projects will address the intersectionality of these many, multi-layered biological factors with social determinants of health in understanding and interpreting immuno-oncology research.

Research may include, but is not limited to:

  • Understanding inflammatory, metabolic, and immune profiles of immunotherapy treatment response across under-represented populations;
  • Investigating genetic, immune signatures, immune infiltrates, and/or distinct tumor immune microenvironments that may contribute to cancer health disparities;
  • Characterizing unique gene expression and immune infiltration in cancer subtypes across under-represented populations;
  • Profiling differential genetic and immune signatures in distinct tumor microenvironments that may be associated with cancer racial disparities;
  • Quantifying distinct genetic, inflammatory, metabolic, and immune profiles with cancer risk across multi-ethnic populations;
  • Characterizing disparities in immunotherapy response, resistance, and immune-related adverse events;
  • Developing animal models that recapitulate immune responses across under-represented populations;
  • Developing patient-derived cancer models that are appropriate for investigating immune responses across under-represented populations; and
  • Integrating diverse racial/ethnic representation in multi-omic and/or multi-modal immune monitoring studies.

Required Components

A P20 application must consist of three components: 1) Overall, 2) Administrative Core, and 3) Research Project. The scope of each component is defined below:

  1. The Overall section should describe the overall research goals of the P20 research project.  Describe the specific aims toward integrating health disparities research into immuno-oncology research.  The Overall section to introduce how the administrative core and research aims will be integrated.
  2. The Administrative Core should provide all organizational, administrative, and scientific management of the P20 project. The P20 project should be comprised of complementary, multi-disciplinary research teams with strengths in both cancer health disparities and immuno-oncology research.
  • The Administrative Core is expected to have effective administrative and organizational capabilities to support multi-disciplinary research teams and foster synergy and integration across the P20. The Administrative Core should coordinate and monitor publications and other metrics of collaboration. Given that the causes of cancer health disparities are multifactorial and complex, it is anticipated that the Administrative Core will coordinate data sharing and data interpretation to ensure the research will address the intersection of the many, multi-layered biological factors and social determinants of health, contributing to the understanding cancer health disparities in the proposed immuno-oncology research.
  • It is anticipated these P20 planning and feasibility grants will lead to future submissions of well-integrated multi-disciplinary research project applications (e.g., P01s or R01s). Similarly, the Administrative Core should plan future multi-disciplinary, collaborative research projects beyond initial planning and feasibility studies.

3. The Research Project. All applications must propose one research project that should have the following characteristics:

  • The proposed research project must be focused and align with the cross-cutting theme of addressing cancer health disparities.
  • The proposed research project must be focused on immuno-oncology research, which includes cancer immunology, cancer immunotherapy, cancer immunoprevention, immuno-engineering, immune-related adverse events, cancer immune profiling, etc. This includes basic biological mechanisms underpinning anti-tumor immune responses, immune regulation of the development and spread of tumors, and approaches to improve immune targeting and destruction of cancer cells.
  • The proposed research project must address the topics described above in the “Research Objectives” section.

Non-Responsive Applications

The following types of activities remain outside of the scope of this FOA, and are considered non-responsive and non-responsive applications will not be reviewed:

  • Applications not focused on cancer health disparities research and immuno-oncology; and
  • Applications that do not have a multi-disciplinary team with complementary expertise in both cancer health disparities research and immuno-oncology research.

  • System for Award Management (SAM) – Applicants must complete and maintain an active registration, which requires renewal at least annually. The renewal process may require as much time as the initial registration. SAM registration includes the assignment of a Commercial and Government Entity (CAGE) Code for domestic organizations which have not already been assigned a CAGE Code.
    • NATO Commercial and Government Entity (NCAGE) Code – Foreign organizations must obtain an NCAGE code (in lieu of a CAGE code) in order to register in SAM.
    • Unique Entity Identifier (UEI) - A UEI is issued as part of the SAM.gov registration process. The same UEI must be used for all registrations, as well as on the grant application.
  • eRA Commons - Once the unique organization identifier is established, organizations can register with eRA Commons in tandem with completing their Grants.gov registration; all registrations must be in place by time of submission. eRA Commons requires organizations to identify at least one Signing Official (SO) and at least one Program Director/Principal Investigator (PD/PI) account in order to submit an application.

Number of Applications

Applicant organizations may submit more than one application, provided that each application is scientifically distinct.

The NIH will not accept duplicate or highly overlapping applications under review at the same time, per 2.3.7.4 Submission of Resubmission Application. This means that the NIH will not accept:

Sub-section B. Innovation:
Highlight any innovative aspects in which the P20 program will integrate health disparities into immuno-oncology research through multi-disciplinary research projects with complementary strengths in both cancer health disparities and immuno-oncology (IO) research.

Sub-section C. Approach:
Summarize the research strategies, methodologies, and analyses that will be used to accomplish the overall specific aims and scientific objectives of the P20 project. Address potential problems, alternative strategies, and benchmarks for success in achieving the aims of the overall P20 project. Discuss the multi-disciplinary team with complementary strengths in both cancer health disparities and IO research.

Letters of Support: Attach letters of support here that are relevant to the overall application. The letter of commitment from the host institution should describe the integration and synergies between the institutional resources and those of the overall P20 project. Specifically, institutional leaders are expected to provide detailed statements of the long-term research commitments and describe the specific resources, space, etc. as applicable to the P20 research.

The Resource Sharing Plan should be provided only under the Overall component, but it should cover all the activities (Administrative Core and Research Project) of the P20 project. Applicants must address plans for sharing research resources (e.g., methods and measures, assessment and intervention protocols, technology, common data elements, data analytic strategies, de-identified data collected in research studies, evidence-based approaches, etc.) with the clinical care and cancer control communities.

Research Strategy: The Administrative Core is expected to have effective administrative and organizational capabilities to support multi-disciplinary research teams, and foster synergy and integration across the P20. The Administrative Core should coordinate and monitor publications and other metrics of collaboration. Given that the causes of cancer health disparities are multifactorial and complex, it is anticipated that the Administrative Core will coordinate data sharing and data interpretation to ensure the research will address the intersection of the many, multi-layered biological factors and social determinants of health, contributing to the understanding of cancer health disparities in the proposed immuno-oncology research.

The proposed research project must be focused on immuno-oncology research, which includes cancer immunology, cancer immunotherapy, cancer immunoprevention, immuno-engineering, immune-related adverse events, cancer immune profiling, etc. This includes basic biological mechanisms underpinning anti-tumor immune responses, immune regulation of the development and spread of tumors, and approaches to improve immune targeting and destruction of cancer cells.

Part I. Overview Information contains information about Key Dates and times. Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s) convened by NCI, in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications will receive a written critique.

Applications may undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.

Appeals of initial peer review will not be accepted for applications submitted in response to this FOA .

Applications will be assigned to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this FOA . Following initial peer review, recommended applications will receive a second level of review by the National Cancer Advisory Board. The following will be considered in making funding decisions:

  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.
  • Relevance of the proposed project to program priorities.

3. Anticipated Announcement and Award Dates

After the peer review of the application is completed, the PD/PI will be able to access his or her Summary Statement (written critique) via the eRA Commons. Refer to Part 1 for dates for peer review, advisory council review, and earliest start date.

Section VI. Award Administration Information

1. Award Notices

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the recipient's business official.

Recipients must comply with any funding restrictions described in Section IV.6. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

Any application awarded in response to this FOA will be subject to terms and conditions found on the Award Conditions and Information for NIH Grants website. This includes any recent legislation and policy applicable to awards that is highlighted on this website.

Institutional Review Board or Independent Ethics Committee Approval: Grantee institutions must ensure that protocols are reviewed by their IRB or IEC. To help ensure the safety of participants enrolled in NIH-funded studies, the recipient must provide NIH copies of documents related to all major changes in the status of ongoing protocols.

2. Administrative and National Policy Requirements

All NIH grant and cooperative agreement awards include the NIH Grants Policy Statement as part of the NoA. If a recipient is successful and receives a Notice of Award, in accepting the award, the recipient agrees that any activities under the award are subject to all provisions currently in effect or implemented during the period of the award, other Department regulations and policies in effect at the time of the award, and applicable statutory provisions.

Should the applicant organization successfully compete for an award, recipients of federal financial assistance (FFA) from HHS will be required to complete an HHS Assurance of Compliance form (HHS 690) in which the recipient agrees, as a term and condition of receiving the grant, to administer their programs in compliance with federal civil rights laws that prohibit discrimination on the basis of race, color, national origin, age, sex and disability, and agreeing to comply with federal conscience laws, where applicable. HHS recognizes that research projects are often limited in scope for many reasons that are nondiscriminatory, such as the principal investigator’s scientific interest, funding limitations, recruitment requirements, and other considerations. Thus, criteria in research protocols that target or exclude certain populations are warranted where nondiscriminatory justifications establish that such criteria are appropriate with respect to the health or safety of the subjects, the scientific study design, or the purpose of the research. In accordance with the statutory provisions contained in Section 872 of the Duncan Hunter National Defense Authorization Act of Fiscal Year 2009 (Public Law 110-417), NIH awards will be subject to the Federal Awardee Performance and Integrity Information System (FAPIIS) requirements. FAPIIS requires Federal award making officials to review and consider information about an applicant in the designated integrity and performance system (currently FAPIIS) prior to making an award. An applicant, at its option, may review information in the designated integrity and performance systems accessible through FAPIIS and comment on any information about itself that a federal agency previously entered and is currently in FAPIIS. The Federal awarding agency will consider any comments by the applicant, in addition to other information in FAPIIS, in making a judgement about the applicant’s integrity, business ethics, and record of performance under Federal awards when completing the review of risk posed by applicants as described in 45 CFR Part 75.205 and 2 CFR Part 200.206 “Federal awarding agency review of risk posed by applicants.” This provision will apply to all NIH grants and cooperative agreements except fellowships.”

3. Data Management and Sharing

Note: The NIH Policy for Data Management and Sharing is effective for due dates on or after January 25, 2023.

Consistent with the NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described. If additional Data Management and Sharing requirements need to be added, please insert what requirements are desired.

4. Reporting

When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.

A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement. NIH FOAs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 45 CFR Part 75.301 and 2 CFR Part 200.301.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for recipients of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All recipients of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over the threshold. See the NIH Grants Policy Statement for additional information on this reporting requirement.

In accordance with the regulatory requirements provided at 45 CFR 75.113and 2 CFR Part 200.113 and Appendix XII to 45 CFR Part 75 and 2 CFR Part 200, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM)about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313). As required by section 3010 of Public Law 111-212, all information posted in the designated integrity and performance system on or after April 15, 2011, except past performance reviews required for Federal procurement contracts, will be publicly available. Full reporting requirements and procedures are found in Appendix XII to 45 CFR Part 75and 2 CFR Part 200 – Award Term and Condition for Recipient Integrity and Performance Matters.
