Funding Opportunity Purpose

This Notice of Funding Opportunity (NOFO) establishes an accelerated review/award process to support time-sensitive research to evaluate a new policy or program that is likely to influence diet, physical activity, sleep, and/or weight and thus has the potential to lower rates of chronic diseases, particularly in populations at risk for health disparities. This NOFO is intended to support research where opportunities for empirical study are, by their very nature, only available through expedited review and funding. All applications submitted to this NOFO must demonstrate that the evaluation of the policy or program of interest offers an uncommon and scientifically-compelling research opportunity that will only be available if the research is initiated with minimum delay. For these reasons, applications submitted to this time-sensitive NOFO are not eligible for re-submission. It is intended that eligible applications selected for funding will be awarded within 4 months of the application due date; however, administrative requirements and other unforeseen circumstances may delay issuance dates beyond that timeline.

Section I. Notice of Funding Opportunity Description

Background

Unhealthy behaviors such as poor dietary intake, inadequate physical activity, excessive sedentary behavior, and lack of sleep are major contributors to many serious chronic diseases and conditions that are associated with increased morbidity and mortality, higher healthcare expenses, and reduced quality of life. The prevalence of obesity and other chronic diseases that are associated with these unhealthy behaviors has dramatically increased in both children and adults in the United States over the past four decades with disproportionately larger increases observed in many populations that are affected by health disparities. For NIH, populations that experience health disparities include racial and ethnic minority groups (see OMB Directive 15), people with lower socioeconomic status (SES), underserved rural communities, sexual and gender minority (SGM) groups, and people with disabilities. Factors that hamper healthy behaviors are more likely to affect these populations owing to larger social and structural influences such as racism and discrimination, lack of access to healthcare, and economic inequality. Adverse social and structural determinants are likewise strongly linked with food and nutrition insecurity, inadequate housing, lack of safe spaces for physical activity, and unfavorable environmental factors that influence sleep among others. 

Across the United States, many localities, states, and the Federal government are taking action to address obesity and chronic disease rates through policies and programs that have the potential to improve health behaviors. While helping people achieve and maintain healthy behaviors and potentially lowering the rates of chronic diseases are critical public health goals, there are many gaps in knowledge regarding the effectiveness of large-scale policies and programs that could help achieve these goals at the population level or within sub-populations, including populations at risk for health disparities. Rigorous scientific evaluation of these policies and programs can help build an evidence base to better inform policy and public health approaches to prevent chronic diseases.

Purpose and Objectives

This notice of funding opportunity (NOFO) is intended to encourage and support research in which a unique and time sensitive opportunity has arisen to collect baseline data and then prospectively assess effectiveness of an imminent policy or program that is likely to influence diet, physical activity, sleep, and/or weight and thus has the potential to lower rates of chronic diseases, particularly in populations at risk for health disparities. 

For the purposes of this NOFO:

  • Policy is broadly defined to include both formal public policies at local, state, and Federal levels of government, and organizational-level policies, such as those implemented by large organizations, worksites, or school districts, that are expected to influence diet, physical activity, sedentary behavior, and/or sleep. Examples include, but are not limited to, the development of supermarkets in underserved areas, calorie labeling requirements, taxes on foods and/or beverages, Food is Medicine programs, changes in health insurance/system reimbursement policies, after-school and summer programs, or modification of the built (or human-made) environments to encourage walking or cycling for transportation or leisure.
  • Program is defined as a set of activities initiated by governmental or other organizational bodies to enhance chronic disease prevention and control. Examples might include programs implemented at worksites, healthcare organizations, after-school or summer programs, or in a community that can be expected to improve behaviors such as energy intake, sleep, and activity levels.

This NOFO is not intended to support the initiation and delivery of new programs; rather, it is intended to support evaluation of the effectiveness of programs and/or policies that are being or will be implemented independent of NIH grant funding.

Applications submitted to this NOFO should demonstrate that measures or outcomes to be collected and evaluated will allow for meaningful and scientifically valid conclusions to be made about the effects of the policy or program on the target behaviors and chronic disease outcomes. Examples of appropriate studies include, but are not limited to, the following:

  • Introduction of food or beverage taxes/subsidies/price changes/other incentives;
  • Infrastructure initiatives such as retailers or other public venues offering healthier food options in underserved areas;
  • Changes to workplace food and/or physical activity environment;
  • Polices expected to influence available dietary options and/or purchasing behaviors, such as calorie labeling in restaurants, produce prescriptions, meal delivery, menu or food product reformulation, and supermarket layout or pricing strategies;
  • Modification of school system policies, including food, physical activity, or starting time;
  • Significant changes in policy or practice in large healthcare organizations that are expected to improve weight outcomes, such as changes in reimbursement for services or pharmaceutical agents, incentives, or wide scale implementation of prevention or treatment services;
  • Policies or programs designed to improve access to or quality of housing and the surrounding built or food environment;
  • Modifications to the built environment to encourage active transportation or leisure physical activity, such as the implementation of bike lanes in urban areas, multi-use trails, subsidies for public transit, upgrades of sidewalks, or improved access to parks and recreation facilities.

There are several distinguishing features of time-sensitive obesity program and/or policy research evaluations:

  • The research question must clearly offer a unique and scientifically compelling research opportunity that will only be possible if the evaluation research component is initiated with minimum delay, and
  • The plan for collection of baseline and follow-up data must be feasible.

Primary outcomes under study must be assessed using objective measures or gold standard assessment methods. Examples of acceptable primary outcomes include rigorously measured behavior change, such as 24-hour dietary recalls or purchasing behaviors, use of resources intended for physical activity, food or beverage purchasing data, energy intake with a focus on lowered calories or lower calorie substitutions, activity changes such as reduced sedentary behavior or increased physical activity, assessment of sleep with actigraphy, weight-related variables (e.g., BMI, body composition), and/or relevant chronic disease outcomes. Other self-reported measures of dietary intake and physical activity can be included but should not be the primary outcome measure(s).

Where possible or relevant, applications should include secondary outcomes that evaluate potential unintended consequences of a policy or program, degree of implementation, and an assessment of barriers and facilitators associated with implementation. This includes measures that will help identify why the policy or program does or does not succeed.

This NOFO strongly encourages equitable scientific partnerships between researchers and public partners (e.g., community-based organizations, local governments, school districts, employers, etc.). Where appropriate, agreements must also be in place that allow for unrestricted publication of findings regardless of study outcomes.

The Program Director/Principal Investigator of the evaluation study application may not be the Director and/or initiator of the policy or program that will be evaluated. Given the possibility for changes in policy or program implementation that are beyond the control of the grantee, grant awards may be terminated early if these changes limit the possibility to collect meaningful outcome data.

Applications requesting research support for the evaluation or retrospective analysis of existing policies or programs that are not time-sensitive should not be submitted under this NOFO. Such applications will be returned to the investigators without review. The focus of this FOA is on research in humans, not animals.

National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK): NIDDK is interested in the evaluation of large-scale programs or policies expected to influence diet, physical activity, sleep, and/or weight with relevance to the development, progression and/or management of diseases and conditions within its mission, including diabetes, other endocrine and metabolic diseases; gastrointestinal, liver and pancreatic diseases; nutritional disorders; obesity; kidney diseases; and urologic conditions. NIDDK encourages research teams that include diverse perspectives to promote inclusive excellence and enhance the scientific process, including meaningful partnership with individuals who are living with NIDDK-relevant diseases/conditions as well as family and community members. NIDDK encourages applications that demonstrate consideration of the Tips for Pursuing Competent Health Equity Research (see Appendix G of Pathways to Health for All). This includes applications with interdisciplinary research teams comprised of individuals with a breadth of lived experiences and biomedical scientific expertise deemed necessary to positively inform the study design, research conduct, and applicability of research findings for the populations of interest. NIDDK has many research areas within its mission, but all research related to relevant diseases/conditions may not be appropriate for NIDDK. Applicants to this NOFO are strongly encouraged to contact NIDDK staff as soon as possible in the development of the application, so that NIDDK staff can help the applicant understand whether the proposed study is within the goals and mission of the Institute and is appropriate for this NOFO.

National Cancer Institute (NCI): NCI is particularly interested in the evaluation of programs or policies that may affect diet, physical activity, sleep, and/or weight, and studies incorporating economic research on such programs and policies. Prospective applicants are strongly urged to contact the NCI Scientific/Research Contacts prior to preparation of such proposals.

National Institute of Child Health and Human Development (NICHD): NICHD has particular interest in research on time-sensitive events, policies, programs, or infrastructure changes which may impact health behaviors (e.g., physical activity, sedentary behavior, dietary intake, and/or sleep) in populations within NICHD's scientific mission areas. The NICHD Strategic Plan outlines high priority areas for the Institute.

Applications assigned to NICHD must address one or more NICHD priority populations.  These include:

  • Infants, children and adolescents, including individuals transitioning from pediatric to adult health care providers
  • Pregnant, peripartum, postpartum and lactating persons
  • Individuals of reproductive age in regard to gynecologic and reproductive health care
  • Individuals of any age with intellectual and developmental disabilities
  • Individuals of any age with physical disabilities

Office of Disease Prevention (ODP): The ODP is interested in supporting research that evaluates food environment, diet and physical activity behaviors, the effect of housing interventions and policies on risk and protective factors for the leading causes of disease and health conditions across the lifespan. The ODP does not award grants; therefore, applications must be relevant to the objectives of at least one of the participating NIH ICs listed in this announcement. The ODP only accepts co-funding requests from NIH ICs. The following types of applications are not responsive to this NOFO. Applications not responsive to this NOFO will not be reviewed.

  • Applications that do not clearly describe the time-sensitivity of the project(s),
  • Applications that do not propose objective measures for the primary outcome(s),
  • Applications that are not specific to policies/programs that may affect dietary intake, physical activity, sedentary behavior, and/or sleep,
  • Applications that independently propose initiation and/or delivery of new policies or programs,
  • Applications exclusively proposing to conduct analyses of existing data, including data in the electronic medical record,
  • Applications proposing to assess policies or programs implemented outside of the United States or US territories,
  • Applications proposing research with animals, or
  • Applications where the Program Director/Principal Investigator is the Director and/or initiator of the policy or program to be evaluated.

Plan for Enhancing Diverse Perspectives (PEDP)

The NIH recognizes that teams comprised of investigators with diverse perspectives working together and capitalizing on innovative ideas and distinct viewpoints outperform homogeneous teams. There are many benefits that flow from a scientific workforce rich with diverse perspectives, including: fostering scientific innovation, enhancing global competitiveness, contributing to robust learning environments, improving the quality of the research, advancing the likelihood that underserved populations participate in, and benefit from research, and enhancing public trust.

To support the best science, the NIH encourages inclusivity in research guided by the consideration of diverse perspectives. Broadly, diverse perspectives can include but are not limited to the educational background and scientific expertise of the people who perform the research; the populations who participate as human subjects in research studies; and the places where research is done.

This NOFO requires a Plan for Enhancing Diverse Perspectives (PEDP), which will be assessed as part of the scientific and technical peer review evaluation.  Assessment of applications containing a PEDP are based on the scientific and technical merit of the proposed project. Consistent with federal law, the race, ethnicity, or sex of a researcher, award participant, or trainee will not be considered during the application review process or when making funding decisions.  Applications that fail to include a PEDP will be considered incomplete and will be administratively withdrawn before review.

  • System for Award Management (SAM) – Applicants must complete and maintain an active registration, which requires renewal at least annually. The renewal process may require as much time as the initial registration. SAM registration includes the assignment of a Commercial and Government Entity (CAGE) Code for domestic organizations which have not already been assigned a CAGE Code.
    • NATO Commercial and Government Entity (NCAGE) Code – Foreign organizations must obtain an NCAGE code (in lieu of a CAGE code) in order to register in SAM.
    • Unique Entity Identifier (UEI) - A UEI is issued as part of the SAM.gov registration process. The same UEI must be used for all registrations, as well as on the grant application.
  • eRA Commons - Once the unique organization identifier is established, organizations can register with eRA Commons in tandem with completing their Grants.gov registrations; all registrations must be in place by time of submission. eRA Commons requires organizations to identify at least one Signing Official (SO) and at least one Program Director/Principal Investigator (PD/PI) account in order to submit an application.
  • Grants.gov – Applicants must have an active SAM registration in order to complete the Grants.gov registration.

Applicant organizations may submit more than one application, provided that each application is scientifically distinct.

The NIH will not accept duplicate or highly overlapping applications under review at the same time, per NIH Grants Policy Statement Section 2.3.7.4 Submission of Resubmission Application. This means that the NIH will not accept:

  • A new (A0) application that is submitted before issuance of the summary statement from the review of an overlapping new (A0) or resubmission (A1) application.
  • A resubmission (A1) application that is submitted before issuance of the summary statement from the review of the previous new (A0) application.

Plan for Enhancing Diverse Perspectives (PEDP)

  • In an "Other Attachment" entitled "Plan for Enhancing Diverse Perspectives," all applicants must include a summary of actionable strategies to advance the scientific and technical merit of the proposed project through expanded inclusivity. 
  • Applicants should align their proposed strategies for PEDP with the research strategy section, providing a holistic and integrated view of how enhancing diverse perspectives and inclusivity are buoyed throughout the application.
  • The PEDP will vary depending on the scientific aims, expertise required, the environment and performance site(s), as well as how the project aims are structured.
  • The PEDP may be no more than 2 pages in length and should include:
    • Actionable strategies using defined approaches for the inclusion of diverse perspectives in the project;
    • Description of how the PEDP will advance the scientific and technical merit of the proposed project;
    • Anticipated timeline of proposed PEDP activities;
    • Evaluation methods for assessing the progress and success of PEDP activities.

Examples of items that advance inclusivity in research and may be appropriate for a PEDP can include, but are not limited to:

  • Partnerships with different types of institutions and organizations (e.g., research-intensive; undergraduate-focused; HBCUs; emerging research institutions; community-based organizations).
  • Project frameworks that enable communities and researchers to work collaboratively as equal partners in all phases of the research process.
  • Outreach and planned engagement activities to enhance recruitment of individuals from diverse groups as human subjects in clinical trials, including those from underrepresented backgrounds.
  • Description of planned partnerships that may enhance geographic and regional diversity.
  • Outreach and recruiting activities intended to diversify the pool of applicants for research training programs, such as outreach to prospective applicants from groups underrepresented in the biomedical sciences, for example, individuals from underrepresented racial and ethnic groups, those with disabilities, those from disadvantaged backgrounds, and women.
  • Plans to utilize the project infrastructure (i.e., research and structure) to enhance the research environment and support career-advancing opportunities for junior, early- and mid-career researchers.
  • Transdisciplinary research projects and collaborations among researchers from fields beyond the biological sciences, such as physics, engineering, mathematics, computational biology, computer and data sciences, as well as bioethics.

Examples of items that are not appropriate in a PEDP include, but are not limited to:

  • Selection or hiring of personnel for a research team based on their race, ethnicity, or sex.
  • A training or mentorship program limited to certain researchers based on their race, ethnicity, or sex.

4. Submission Dates and Times

Part I. contains information about Key Dates and times. Applicants are encouraged to submit applications before the due date to ensure they have time to make any application corrections that might be necessary for successful submission. When a submission date falls on a weekend or Federal holiday, the application deadline is automatically extended to the next business day.

Organizations must submit applications to Grants.gov (the online portal to find and apply for grants across all Federal agencies). Applicants must then complete the submission process by tracking the status of the application in the eRA Commons, NIH’s electronic system for grants administration. NIH and Grants.gov systems check the application against many of the application instructions upon submission. Errors must be corrected and a changed/corrected application must be submitted to Grants.gov on or before the application due date and time.  If a Changed/Corrected application is submitted after the deadline, the application will be considered late. Applications that miss the due date and time are subjected to the NIH Grants Policy Statement Section 2.3.9.2 Electronically Submitted Applications.

Use of Common Data Elements in NIH-funded Research

Many NIH ICs encourage the use of common data elements (CDEs) in basic, clinical, and applied research, patient registries, and other human subject research to facilitate broader and more effective use of data and advance research across studies. CDEs are data elements that have been identified and defined for use in multiple data sets across different studies. Use of CDEs can facilitate data sharing and standardization to improve data quality and enable data integration from multiple studies and sources, including electronic health records. NIH ICs have identified CDEs for many clinical domains (e.g., neurological disease), types of studies (e.g. genome-wide association studies (GWAS)), types of outcomes (e.g., patient-reported outcomes), and patient registries (e.g., the Global Rare Diseases Patient Registry and Data Repository). NIH has established a “Common Data Element (CDE) Resource Portal" (https://cde.nlm.nih.gov/home (https://cde.nlm.nih.gov/home)) to assist investigators in identifying NIH-supported CDEs when developing protocols, case report forms, and other instruments for data collection. The Portal provides guidance about and access to NIH-supported CDE initiatives and other tools and resources for the appropriate use of CDEs and data standards in NIH-funded research. Investigators are encouraged to consult the Portal and describe in their applications any use they will make of NIH-supported CDEs in their projects.

Section V. Application Review Information

1. Criteria

Only the review criteria described below will be considered in the review process. Applications submitted to the NIH in support of the NIH mission are evaluated for scientific and technical merit through the NIH peer review system.

Overall Impact

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following scored review criteria and additional review criteria (as applicable for the project proposed).  An application does not need to be strong in all categories to be judged likely to have a major scientific impact. As part of the overall impact score, reviewers should consider and indicate how the Plan for Enhancing Diverse Perspectives affects the scientific merit of the project.

Scored Review Criteria

Reviewers will consider Factors 1, 2 and 3 in the determination of scientific merit, and in providing an overall impact score. In addition, Factors 1 and 2 will each receive a separate factor score. 

 

Significance

  • Evaluate the importance of the proposed research in the context of current scientific challenges and opportunities, either for advancing knowledge within the field, or more broadly. Assess whether the application addresses an important gap in knowledge in the field, would solve a critical problem, or create a valuable conceptual or technical advance.
  • Evaluate the rationale for undertaking the study, the rigor of the scientific background for the work (e.g., prior literature and/or preliminary data) and whether the scientific background justifies the proposed study.

Innovation

  • Evaluate the extent to which innovation influences the importance of undertaking the proposed research. Note that while technical or conceptual innovation can influence the importance of the proposed research, a project that is not applying novel concepts or approaches may be of critical importance for the field.
  • Evaluate whether the proposed work applies novel concepts, methods or technologies or uses existing concepts, methods, technologies in novel ways, to enhance the overall impact of the project.

Specific to this NOFO: Evaluate whether the research project offers an uncommon and scientifically significant research opportunity. Assess whether the results of the evaluation research, if successful, will have the potential to meaningfully contribute to the advancement of public health.

 

Approach

  • Evaluate the scientific quality of the proposed work. Evaluate the likelihood that compelling, reproducible findings will result (rigor) and assess whether the proposed studies can be done well and within the timeframes proposed (feasibility).

Rigor:

  • Evaluate the potential to produce unbiased, reproducible, robust data.
  • Evaluate the rigor of experimental design and whether appropriate controls are in place.
  • Evaluate whether the sample size is sufficient and well-justified.
  • Assess the quality of the plans for analysis, interpretation, and reporting of results.
  • Evaluate whether the investigators presented adequate plans to address relevant biological variables, such as sex or age, in the design, analysis, and reporting.
  • For applications involving human subjects or vertebrate animals, also evaluate:
    • the rigor of the intervention or study manipulation (if applicable to the study design).
    • whether outcome variables are justified.
    • whether the results will be generalizable or, in the case of a rare disease/special group, relevant to the particular subgroup.
    • whether the sample is appropriate and sufficiently diverse to address the proposed question(s).
  • For applications involving human subjects, including clinical trials, assess the adequacy of inclusion plans as appropriate for the scientific goals of the research. Considerations of appropriateness may include disease/condition/behavior incidence, prevalence, or population burden, population representation, and/or current state of the science.

Feasibility:

  • Evaluate whether the proposed approach is sound and achievable, including plans to address problems or new challenges that emerge in the work. For proposed studies in which feasibility may be less certain, evaluate whether the uncertainty is balanced by the potential for major advances.
  • For applications involving human subjects, including clinical trials, evaluate the adequacy and feasibility of the plan to recruit and retain an appropriately diverse population of participants. Additionally, evaluate the likelihood of successfully achieving the proposed enrollment based on age, racial, ethnic, and sex or gender categories.
  • For clinical trial applications, evaluate whether the study timeline and milestones are feasible.

Specific to this NOFO: Evaluate whether an important scientific opportunity will be lost if the work is not initiated with minimum delay. If the PD/PI is involved as an advisor to the program during development and/or implementation, evaluate whether the methods to minimize bias are described and whether they are appropriate for the project.

 

Investigator(s)

Evaluate whether the investigator(s) have demonstrated background, training, and expertise, as appropriate for their career stage, to conduct the proposed work. For Multiple Principal Investigator (MPI) applications, assess the quality of the leadership plan to facilitate coordination and collaboration.

Evaluate whether the institutional resources are appropriate to ensure the successful execution of the proposed work.

For research that involves human subjects but does not involve one of the categories of research that are exempt under 45 CFR Part 46, evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects; 2) adequacy of protection against risks; 3) potential benefits to the subjects and others; 4) importance of the knowledge to be gained; and 5) data and safety monitoring for clinical trials.

For research that involves human subjects and meets the criteria for one or more of the categories of research that are exempt under 45 CFR Part 46, evaluate: 1) the justification for the exemption; 2) human subjects involvement and characteristics; and 3) sources of materials. For additional information on review of the Human Subjects section, please refer to the Guidelines for the Review of Human Subjects.

 

When the proposed research includes Vertebrate Animals, evaluate the involvement of live vertebrate animals according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. 2. Review and Selection Process

Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s) convened by NIDDK, in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications will receive a written critique.

Applications may undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.

Applications will be assigned on the basis of established PHS referral guidelines to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this NOFO. Following initial peer review, recommended applications will receive a second level of review by the appropriate national Advisory Council or Board. The following will be considered in making funding decisions:

  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.
  • Relevance of the proposed project to program priorities.

Section VI. Award Administration Information

1. Award Notices

A Notice of Award (NoA) is the official authorizing document notifying the applicant that an award has been made and that funds may be requested from the designated HHS payment system or office. The NoA is signed by the Grants Management Officer and emailed to the recipient’s business official.

In accepting the award, the recipient agrees that any activities under the award are subject to all provisions currently in effect or implemented during the period of the award, other Department regulations and policies in effect at the time of the award, and applicable statutory provisions.

Recipients must comply with any funding restrictions described in Section IV.6. Funding Restrictions. Institutional Review Board or Independent Ethics Committee Approval: Recipient institutions must ensure that protocols are reviewed by their IRB or IEC. To help ensure the safety of participants enrolled in NIH-funded studies, the recipient must provide NIH copies of documents related to all major changes in the status of ongoing protocols.

2. Administrative and National Policy Requirements

3. Data Management and Sharing

Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 2 CFR Part 200.301.
