The INvestigation of Co-occurring conditions across the Lifespan to Understand Down syndromE (INCLUDE) Project encourages exploratory and innovative research to develop, characterize, or improve animal models and related biological materials for Down syndrome (DS) related research and to improve access to information about or from the use of animal models for DS research. The animal models and related biological materials developed must have utility in targeted or basic science studies in areas highly relevant to DS. With this funding opportunity announcement, the INCLUDE Project encourages, but is not limited to, projects focusing on development of various animal models, genetic resources, atlases at a single cell or subcellular level, advanced informatics technologies including artificial intelligence (AI) or machine learning (ML), and integration of multiple animal models and technology platforms for enhancing rigor and reproducibility of preclinical DS research.

Section I. Notice of Funding Opportunity Description

The NIH INvestigation of Co-occurring conditions across the Lifespan to Understand Down syndromE (INCLUDE) Project seeks to improve health and quality-of-life for individuals with Down syndrome (DS). This Funding Opportunity Announcement (FOA) is intended to solicit requests for exploratory and innovative research to foster the development of and expand the access to animal models for DS-related research and related biological materials and information that would advance preclinical and basic science studies related to DS.

Background

DS is the most common genetic cause of intellectual disability, the most common autosomal trisomy, and one of the most visible and universally recognized genetic syndromes. Each year there are approximately 5300 babies born in the United States with DS. Within the past 25 years, the average lifespan for a person with DS has doubled, from 30 to 60 years. Despite this increase in lifespan, individuals with DS and their families face significant and changing health challenges with age, and they have often been excluded from participation in research that could improve their health outcomes and quality of life. While all people with DS are connected by the common feature of a complete or partial copy of chromosome 21 (trisomy 21), there are significant physical and cognitive differences among them, indicating that inter-individual variability exists.

DS is associated with an increased prevalence of autism and epilepsy. About 75% of individuals experience cognitive decline in a syndrome that resembles Alzheimer’s disease but has its onset a decade or two earlier than typical Alzheimer’s disease. Individuals with DS also have high rates of hearing loss, eye abnormalities, congenital heart defects, sleep apnea, pulmonary hypertension, gastrointestinal malformations, thyroid disease, leukemia, and other autoimmune or immune dysregulation disorders including celiac disease. However, people with DS infrequently develop solid tumors such as breast or prostate cancer, and despite multiple risk factors for coronary artery disease and high rates of obesity, sleep apnea, and type 1 diabetes, they rarely develop atherosclerosis or have myocardial infarctions. Understanding this unique combination of risk and resiliencies will inform medical advances for individuals with DS, and for individuals who do not have DS but share these co-occurring conditions.

This FOA is one of several NIH-wide research initiatives created in response to Fiscal Year 2018, 2019, and 2020 Omnibus Appropriations Reports, which encourage NIH to expand its current efforts on DS and common co-occurring conditions also seen in the general population, while increasing the pipeline of DS investigators. Together, the initiatives are called the INCLUDE Project (INvestigation of Co-occurring conditions across the Lifespan to Understand Down syndromE). Information about projects that were funded in prior years, as well as the NIH INCLUDE Down Syndrome Research Plan, are available on the INCLUDE Project website at https://www.nih.gov/include-project. The INCLUDE Project has three components:

  • Component 1: Targeted high risk - high reward basic science studies in areas highly relevant to Down syndrome
  • Component 2: Assembly of a large cohort of individuals with Down syndrome across the lifespan to perform deep phenotyping and study co-existing conditions
  • Component 3: Inclusive clinical trials of existing and future treatments and interventions for co-occurring conditions in individuals with Down syndrome

The INCLUDE Project sponsored a virtual workshop titled “Down Syndrome Research: The Intersection of Basic Science and Clinical Cohort Development” on November 9-10, 2020. Participants, including basic science investigators and clinical experts as well as members of the DS communities, discussed the current status and gaps in DS research. With respect to basic and preclinical science, participants identified the need for developing and phenotyping various animal models and enhancing rigor and reproducibility of animal research.

Research Objectives

This FOA addresses Component 1 of the INCLUDE Project. With this FOA, the INCLUDE Project intends to address the gaps identified from the aforementioned workshop by fostering development of and expanding access to animal models for DS research and related biological materials and information that will advance preclinical and basic science studies related to DS. Sharing of resources and effective communication of scientific advancements to broader communities are a high priority of the INCLUDE Project.

Examples of projects for developing animal models and related biological materials highly relevant to DS research include, but are not limited to:

  • New or improved animal models for studying fundamental biological mechanisms underlying DS.
  • Genetic resources, antibodies, nanobodies, functional antibodies, or other reagents for quantifying or characterizing macromolecules or cells in animal models for DS research.
  • Development of resources or tools for high throughput genetic and therapeutic screens in appropriate animal species, such as C. elegans, Drosophila and zebrafish, for DS research.
  • AI, ML or other deep phenotyping tools for integrating multi-omics, biochemical, physiological, morphological, and behavioral data within or among animal models for DS research; generating novel hypotheses; fostering communication between basic science and clinical researchers; and improving rigor, reproducibility, and translatability of animal models for DS research.
  • Development of resources or tools for facilitating development of whole-organism or whole-organ atlases of DS animal models at single cell or subcellular levels for comparative studies and for studying cells and cellular networks underlying DS.
  • Animal models or resources for detecting and understanding in vivo protein-protein interactions in cells and organelles for DS research at a large scale. Of particular interest are projects taking advantage of structural information generated from AI or ML tools.
  • Complementary approaches to the use of animals, such as animal-tissue-on-chip models and organoids, that can simulate physiological and pathophysiological processes and capture the complex dynamics of interacting molecules, cells, tissues, and organs for DS research outside the context of whole organisms.
  • Comparison of existing DS animal models to better understand the health conditions and co-occurring conditions with DS. While several useful animal models are available to the community, a careful comparison of their distinct phenotypes is critical to the integration of findings from those models. For example, with the mouse strains currently in use, qualitative and quantitative validation of relevant phenotypes would contribute to a better understanding of the penetrance of those phenotypes. In addition, a better understanding of the influences of the genetic background and colony maintenance on relevant phenotypes in mouse strains could improve the translatability of those models for DS research.
  • Development of rat, non-rodent vertebrate, and nonhuman primate models of DS that are better suited than currently available mouse models for answering specific questions concerning the complex neurological, behavioral, and morphological traits and other phenotypes and comorbidities that affect individuals with DS.
  • Resources or tools for integration of multiple evolutionarily divergent animal models and technology platforms with different capabilities to improve the rigor, reproducibility, and translatability of preclinical DS research.
  • Improvement of animal DS models for regenerative medicine, which will better emulate physiological, cellular, and molecular manifestations underlying DS seen in humans. Development of methods to access stem cell-based treatment potential regarding effectiveness, major complications, safety, and off target effects. Demonstration of functionality of specific stem cells or their derivatives and effectiveness of achieving specific results in improved animal disease models.
  • Studies on testing the potential of somatic cell genome editing technology in animal models to resolve elements of DS conditions.
  • Development and refinement of animal models or tools for studying the impact of newly emerging human pathogens on individuals with DS and in response to recent active trans-NIH public health initiatives, such as those in response to pandemic(s).

The R21 grant activity is intended to encourage exploratory research projects that foster a constant infusion of new ideas, techniques, and points of view into DS-related research. Such projects could assess a new experimental system or propose an innovative use of an existing methodology, system or model to enhance DS-related research. These studies may involve considerable risk but may lead to a breakthrough in DS research through development of novel techniques, reagents, methodologies and models. The INCLUDE project is particularly interested in development of animal models with relevance to the multiple organ systems affected in individuals with DS co-occurring conditions, such as neurodevelopment, immune system dysregulation, Alzheimer’s disease, cancer, cardiovascular disease, and autism.

Program Directors/Principal Investigators (PD/PIs) planning to submit applications in response to this FOA are strongly encouraged to contact the scientific contacts of this FOA prior to submission to be advised on appropriateness of the intended resource and research plans for this program, competitiveness of a potential application, and alignment with program priorities of the INCLUDE initiative.

Section V. Application Review Information

1. Criteria

Only the review criteria described below will be considered in the review process. Applications submitted to the NIH in support of the NIH mission are evaluated for scientific and technical merit through the NIH peer review system.

For this particular announcement, note the following: 

The R21 exploratory/developmental grant supports investigation of novel scientific ideas or new model systems, tools, or technologies that have the potential for significant impact on biomedical or biobehavioral research. An R21 grant application need not have extensive background material or preliminary information. Accordingly, reviewers will emphasize the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Preliminary data are not required for R21 applications; however, they may be included if available. 

Overall Impact

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following scored review criteria and additional review criteria (as applicable for the project proposed). An application does not need to be strong in all categories to be judged likely to have a major scientific impact.

Scored Review Criteria

Reviewers will evaluate Factors 1, 2 and 3 in the determination of scientific merit, and in providing an overall impact score. In addition, Factors 1 and 2 will each receive a separate criterion score. 

 

Significance

  • Evaluate the importance of the proposed research in the context of current scientific challenges and opportunities, either for advancing knowledge within the field, or more broadly. Assess whether the application addresses an important gap in knowledge in the field, would solve a critical problem, or create a valuable conceptual or technical advance.
  • Evaluate the rationale for undertaking the study, the rigor of the scientific background for the work (e.g., prior literature and/or preliminary data) and whether the scientific background justifies the proposed study.

Innovation

  • Evaluate the extent to which innovation influences the importance of undertaking the proposed research. Note that while technical or conceptual innovation can influence the importance of the proposed research, a project that is not applying novel concepts or approaches may be of critical importance for the field.
  • Evaluate whether the proposed work applies novel concepts, methods or technologies or uses existing concepts, methods, technologies in novel ways, to enhance the overall impact of the project.

Specific to this NOFO: 

  • Evaluate the project’s alignment with research priorities of the INCLUDE Project.
  • Evaluate the need for the proposed model(s), technology(ies), or biological material(s) to address any scientific hypothesis of interest to the DS research community.
  • Evaluate the proposed animal model(s), biological material(s), or tool(s) used to address a gap(s) in DS research.
 

Approach

  • Evaluate the scientific quality of the proposed work. Evaluate the likelihood that compelling, reproducible findings will result (rigor) and assess whether the proposed studies can be done well and within the timeframes proposed (feasibility).

Rigor:

  • Evaluate the potential to produce unbiased, reproducible, robust data.
  • Evaluate the rigor of experimental design and whether appropriate controls are in place.
  • Evaluate whether the sample size is sufficient and well-justified.
  • Assess the quality of the plans for analysis, interpretation, and reporting of results.
  • Evaluate whether the investigators presented adequate plans to address relevant biological variables, such as sex or age, in the design, analysis, and reporting.
  • For applications involving human subjects or vertebrate animals, also evaluate:
    • the rigor of the intervention or study manipulation (if applicable to the study design).
    • whether outcome variables are justified.
    • whether the results will be generalizable or, in the case of a rare disease/special group, relevant to the particular subgroup.
    • whether the sample is appropriate and sufficiently diverse to address the proposed question(s).
  • For applications involving human subjects, including clinical trials, assess the adequacy of inclusion plans as appropriate for the scientific goals of the research. Considerations of appropriateness may include disease/condition/behavior incidence, prevalence, or population burden, population representation, and/or current state of the science.

Feasibility:

  • Evaluate whether the proposed approach is sound and achievable, including plans to address problems or new challenges that emerge in the work. For proposed studies in which feasibility may be less certain, evaluate whether the uncertainty is balanced by the potential for major advances.
  • For applications involving human subjects, including clinical trials, evaluate the adequacy and feasibility of the plan to recruit and retain an appropriately diverse population of participants. Additionally, evaluate the likelihood of successfully achieving the proposed enrollment based on age, racial, ethnic, and sex/gender categories.
  • For clinical trial applications, evaluate whether the study timeline and milestones are feasible.

Specific to this NOFO:

  • If applicable, evaluate if the application describes a clear plan on how to distribute developed animal model(s), biological material(s), or informatic tool(s) to the scientific community after the termination of the grant cycle.
 

Investigator(s)

Evaluate whether the investigator(s) have demonstrated background, training, and expertise, as appropriate for their career stage, to conduct the proposed work. For Multiple Principal Investigator (MPI) applications, assess the quality of the leadership plan to facilitate coordination and collaboration.

Environment

Evaluate whether the institutional resources are appropriate to ensure the successful execution of the proposed work.

Additional Review Criteria

As applicable for the project proposed, reviewers will consider the following additional items while determining scientific and technical merit, but will not give criterion scores for these items, and should consider them in providing an overall impact score.

 

For research that involves human subjects but does not involve one of the categories of research that are exempt under 45 CFR Part 46, evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects; 2) adequacy of protection against risks; 3) potential benefits to the subjects and others; 4) importance of the knowledge to be gained; and 5) data and safety monitoring for clinical trials.

For research that involves human subjects and meets the criteria for one or more of the categories of research that are exempt under 45 CFR Part 46, evaluate: 1) the justification for the exemption; 2) human subjects involvement and characteristics; and 3) sources of materials. When the proposed research includes Vertebrate Animals, evaluate the involvement of live vertebrate animals according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. When the proposed research includes Biohazards, evaluate whether specific materials or procedures that will be used are significantly hazardous to research personnel and/or the environment, and whether adequate protection is proposed.

 

As applicable, evaluate the full application as now presented.

 

As applicable, evaluate the progress made in the last funding period.

As applicable, evaluate the appropriateness of the proposed expansion of the scope of the project.

Additional Review Considerations

As applicable for the project proposed, reviewers will consider each of the following items, but will not give scores for these items, and should not consider them in providing an overall impact score.

 

For projects involving key biological and/or chemical resources, evaluate the brief plans proposed for identifying and ensuring the validity of those resources.

 

Evaluate whether the budget and the requested period of support are fully justified and reasonable in relation to the proposed research.

2. Review and Selection Process

Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s) convened by the Center for Scientific Review , in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications will receive a written critique.

Applications may undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.

Applications will be assigned on the basis of established PHS referral guidelines to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this NOFO. Following initial peer review, recommended applications will receive a second level of review by the appropriate national Advisory Council or Board. The following will be considered in making funding decisions:

  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.
  • Relevance of the proposed project to program priorities.

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement Section 2.5.1. Just-in-Time Procedures. This request is not a Notice of Award nor should it be construed to be an indicator of possible funding.

Prior to making an award, NIH reviews an applicant’s federal award history in SAM.gov to ensure sound business practices. An applicant can review and comment on any information in the Responsibility/Qualification records available in SAM.gov. NIH will consider any comments by the applicant in the Responsibility/Qualification records in SAM.gov to ascertain the applicant’s integrity, business ethics, and performance record of managing Federal awards per 2 CFR Part 200.206 “Federal awarding agency review of risk posed by applicants.” This provision will apply to all NIH grants and cooperative agreements except fellowships.

3. Anticipated Announcement and Award Dates

After the peer review of the application is completed, the PD/PI will be able to access his or her Summary Statement (written critique) via the eRA Commons. Refer to Part 1 for dates for peer review, advisory council review, and earliest start date.

Section VI. Award Administration Information

1. Award Notices

A Notice of Award (NoA) is the official authorizing document notifying the applicant that an award has been made and that funds may be requested from the designated HHS payment system or office. The NoA is signed by the Grants Management Officer and emailed to the recipient’s business official.

In accepting the award, the recipient agrees that any activities under the award are subject to all provisions currently in effect or implemented during the period of the award, other Department regulations and policies in effect at the time of the award, and applicable statutory provisions.

Recipients must comply with any funding restrictions described in Section IV.6. Funding Restrictions. Any pre-award costs incurred before receipt of the NoA are at the applicant's own risk.  For more information on the Notice of Award, please refer to the NIH Grants Policy Statement Section 5. Institutional Review Board or Independent Ethics Committee Approval: Recipient institutions must ensure that protocols are reviewed by their IRB or IEC. To help ensure the safety of participants enrolled in NIH-funded studies, the recipient must provide NIH copies of documents related to all major changes in the status of ongoing protocols.

2. Administrative and National Policy Requirements

The following Federal wide and HHS-specific policy requirements apply to awards funded through NIH:

All federal statutes and regulations relevant to federal financial assistance, including those highlighted in NIH Grants Policy Statement Section 4 Public Policy Requirements, Objectives and Other Appropriation Mandates.

Recipients are responsible for ensuring that their activities comply with all applicable federal regulations.  NIH may terminate awards under certain circumstances.  See 2 CFR Part 200.340 Termination and NIH Grants Policy Statement Section 8.5.2 Remedies for Noncompliance or Enforcement Actions: Suspension, Termination, and Withholding of Support

3. Data Management and Sharing

Consistent with the 2023 NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described.

4. Reporting

When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement Section 8.4.1 Reporting. To learn more about post-award monitoring and reporting, see the NIH Grants & Funding website, see Post-Award Monitoring and Reporting.

A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement Section 8.6 Closeout. NIH NOFOs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 2 CFR Part 200.301.
