Wearable technology shows promise in providing information that can be used to create biomarkers of various diagnostic groups that have relevance to mental illness. However, there are barriers related to multiple and sometimes proprietary data standards that make data aggregation difficult from existing devices. The purpose of this Notice of Funding Opportunity (NOFO) is to support the creation of standards for data as well as metadata and related coordination activities that will allow researchers to easily access data from personal tracking devices and integrate those data for subsequent data analysis. The recipients will be expected to partner with device manufacturers, the researcher community, and other interested parties such as ethics experts if the effort is going to be successful. It is expected that the recipient will create standards for personal tracking device data that serve the same purpose as the Digital Imaging and Communications in Medicine (DICOM) or Neuroimaging Informatics Technology Initiative (NIfTI) formats and standards do for medical image files. Those medical image file standards have allowed secondary data analysis and have resulted in many software packages and workflows for secondary data analysis.

Background and Rationale
Investigator-initiated applications administered by the National Institute of Mental Health (NIMH) that involve human research participants increasingly propose collecting data from wearable devices or from cell phones. Such devices have the potential to provide information that may turn out to be very useful in creating biomarkers for groups of people who have a mental illness. However, wearable devices and cell phones are created by a number of different manufacturers each of whom deliver analyzed data (for example about sleep quality) to consumers. In some cases, the raw data are also made available to the consumer, but the data format for either raw or derived data streams differs from manufacturer to manufacturer. This makes it difficult or impossible to link data sets coming from different devices.
Other areas of data collection have had similar issues and have found solutions to those problems. For example, in the human magnetic resonance imaging field, each of the device manufacturers collect data using different protocols and have different internal data formats from the other manufacturers. However, that community has developed the Digital Imaging and Communications in Medicine (DICOM) data standard that allows each manufacturer to export data using the same standard. In turn, this has allowed researchers to develop analysis tools and pipelines that begin with data in DICOM format. Such standardization has been essential for data analysis in the field.
DICOM is not the only standard that is useful in the magnetic resonance space. Other standards such as the Neuroimaging Informatics Technology Initiative (NIfTI) and its derivatives have been very useful to the research community.
For standards pertaining to wearable devices and cell phones, it may also be appropriate to have specialized standards in some areas as well as a larger general standard. The overall goal of this Notice of Funding Opportunity (NOFO) is to develop standards that include data formats, data organization, device descriptions, and related information that will promote re-use and integration of data from wearable devices and cell phones. The standards and data formats can be aimed at raw data, derived data, or both.
A recent meeting held at UCLA, Digital Sensing for Mental Health on February 23 to March 2, 2023, discussed many of these issues.
Purpose and Objectives
The specific goal of this NOFO is to develop standards and data formats that will promote re-use of data from wearable devices and cell phones. Examples of wearable devices include jewelry, activity trackers, watches, glasses. For this NOFO, cell phones are also included. Sensors that monitor activity in rooms or other defined locations but do not have a wearable component are not responsive to this NOFO.
One important issue related to data from wearable devices is the definition of what information should be considered data and what information should be considered metadata. In many fields, header records are used to hold information about the image (for example), and there is a clear distinction between metadata and data. For wearable devices and cell phones, the difference may be less clear. For the purpose of this NOFO, any activity that makes it easier to reuse data from wearable devices and cell phones is allowable for both data as well as for metadata. The applicant should define how to standardize both types of information in their application.
Issues that will have to be addressed in proposed standards include:
The use of data from wearable devices can raise ethical and privacy issues. Such issues should not be dealt with in the proposed standards but will be dealt with by the archive holding the data in collaboration with appropriate Institutional Review Boards. However, the applicant can propose to organize a meeting or meetings related to these ethical issues and to make the recommendations coming from such meetings widely available to the wearable device research community.
The development of standards can take many different pathways, but for this NOFO there is a particular pathway that is recommended. That pathway includes:
Applicants are strongly encouraged to extend existing standards where possible.
Standards that can be widely used (either in the number of different devices, manufacturers, or customers who would use devices with those standards) are strongly preferable to narrower standards.
The wearable device field is changing rapidly. Because of the speed of developments, it would not be surprising if the initial standard(s) developed under this NOFO will need to be updated. Applicants should include a plan for updating the initial standard. The timelines or triggers that would indicate a need for an updated standard should be included in the application. The ownership of the initial standard should also be discussed in this section.
Finally, members of the research community will require training in the contents of the new standard as well as how to use the standard in their research programs. Applicants should propose appropriate training activities.
Applications Not Responsive to this NOFO
Applications that are focused on developing standards for anything other than derived or raw data from wearable devices or cell phones are non-responsive and will not be reviewed. Applications focused on other aspects of measuring or using data from wearable devices are non-responsive and will not be reviewed.

Institutional Review Board or Independent Ethics Committee Approval: Recipient institutions must ensure that protocols are reviewed by their IRB or IEC. If a recipient is successful and receives a Notice of Award, in accepting the award, the recipient agrees that any activities under the award are subject to all provisions currently in effect or implemented during the period of the award, other Department regulations and policies in effect at the time of the award, and applicable statutory provisions.

HHS recognizes that NIH research projects are often limited in scope for many reasons that are nondiscriminatory, such as the principal investigators scientific interest, funding limitations, recruitment requirements, and other considerations. Thus, criteria in research protocols that target or exclude certain populations are warranted where nondiscriminatory justifications establish that such criteria are appropriate with respect to the health or safety of the subjects, the scientific study design, or the purpose of the research. In accordance with the statutory provisions contained in Section 872 of the Duncan Hunter National Defense Authorization Act of Fiscal Year 2009 (Public Law 110-417), NIH awards will be subject to the Federal Awardee Performance and Integrity Information System (FAPIIS) requirements. FAPIIS requires Federal award making officials to review and consider information about an applicant in the designated integrity and performance system (currently FAPIIS) prior to making an award. An applicant, at its option, may review information in the designated integrity and performance systems accessible through FAPIIS and comment on any information about itself that a federal agency previously entered and is currently in FAPIIS. The Federal awarding agency will consider any comments by the applicant, in addition to other information in FAPIIS, in making a judgement about the applicants integrity, business ethics, and record of performance under Federal awards when completing the review of risk posed by applicants as described in 2 CFR Part 200.206 Federal awarding agency review of risk posed by applicants. This provision will apply to all NIH grants and cooperative agreements except fellowships.

Consistent with the 2023 NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described.
When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.
A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement. NIH NOFOs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 2 CFR Part 200.301.
The Federal Funding Accountability and Transparency Act of 2006, as amended (FFATA), includes a requirement for recipients of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All recipients of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over the threshold. See the NIH Grants Policy Statement for additional information on this reporting requirement.
In accordance with the regulatory requirements provided 2 CFR Part 200.113 and Appendix XII to 2 CFR Part 200, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313).
