REVISED PHS 398 AND PHS 2590 NOW AVAILABLE, REVISED PHS 416-1 AND 416-9 IN PRODUCTION Release Date: November 12, 1998 P.T. National Institutes of Health The revised (4/98) PHS 398 and PHS 2590 grant application forms and instructions are now available, and may now be used by applicants. For the January/February 1999 application receipt dates, use of the revised forms is encouraged. The new forms must be used for receipt dates of May 10, 1999 and thereafter. The new PHS 398 and PHS 2590 application instructions and forms are available on the NIH grants Web site at https://grants.nih.gov/grants/forms.htm. Applicants are encouraged to access the application instructions and forms via the Internet; however, single hard copies will be available from Offices of Sponsored Programs at most research organizations. The Offices of Sponsored Programs may order bulk quantities by e-mail, anticipating their needs for six to twelve months, from Grants Information, Division of Extramural Outreach and Information Resources. Applicants who do not have an Office of Sponsored Programs, or access to the Internet, may also order copies from Grants Information. The revised PHS 416-1 and PHS 416-9, Applications for an Individual National Research Service Award, have been approved and are in the production process. When the revised PHS 416-1 and PHS 416-9 application instructions and forms arrive, a notice will be published in the NIH Guide for Grants and Contracts announcing their availability. The current version (8/95) of the forms should be used until the notice appears. The current version may be used for the December 5, 1998 receipt date. CHANGES TO THE PHS 398 INCLUDE: o A "New Investigator" check box now appears on the Face Page of the application; o Item 3c on the Face Page, the Social Security Number box, is shaded out. The Social Security Number should be provided on Form Page KK only. A space now appears at the top right hand corner of Form Page KK for the Social Security Number; o Research Plan requirements now include a description of the inclusion of children as participants in research involving human subjects; o Additional instructions are provided regarding the submission of proprietary information with the application; o Research Career Award instructions and Table of Contents (Form Page LL) incorporate Just-in-Time requirements. Other support information for the sponsor(s) and co-sponsor(s) should now be provided on a modified Format Page 7- GG; o Page limits for the Research Career Award applications are clarified. The total number of pages for Item 3 (The Candidate) and a.-d. of Item 6 (Research Plan) combined may not exceed 25; o Organization contacts, phone numbers, e-mail and Web addresses are updated; o The label page is updated and includes the phone number for express mail and courier service delivery of an application. The mailing label is now addressed to the "Center for Scientific Review." CHANGES TO THE PHS 2590 INCLUDE: o More detailed instructions are provided for the Streamlined Noncompeting Award Process (SNAP); o Progress Report Summary requirements are modified to include a description of the inclusion of children as participants in research involving human subjects. ERROR IN FIRST PRINTING OF PHS 398 FORM PAGES: There is a correction to be made to the PHS 398 Biographical Sketch, Form Page FF. The last line of instructions should state "DO NOT EXCEED TWO PAGES." The pdf version posted on the NIH web site is correct.
