EXTENSION OF CURRENT 398, 2590 AND 416-1 GRANT APPLICATION FORMS, EXPECTED AVAILABILITY OF NEW GRANT APPLICATION FORMS Release Date: February 20, 1998 P.T. National Institutes of Health The Application for a Public Health Service Grant (PHS 398) and Application for Continuation of a Public Health Service Grant (PHS 2590) currently in use were revised May 1995 and approved for use through February 28, 1998. These forms have again been revised and were submitted to the Office of Management and Budget (OMB) for approval. The National Institutes of Health (NIH) recently received notice that OMB approved the most recent revision. The new forms will be available shortly. In the interim and until further notice, the May 1995 revision is to be used. The NIH will be limiting the number of copies of the May 1995 forms and instructions mailed to requestors in order to avoid reprinting materials that will shortly be outdated. Offices of Sponsored Programs should encourage conservation of the current forms and instructions through sharing, photocopying and/or accessing the materials via the Internet. They are posted on the NIH Office of Extramural Research home page at grants.nih.gov/grants/forms.htm. The NIH will publish an announcement in the NIH Guide for Grants and Contracts when the new forms are available for use. There will be a transition period during which either the new or old forms will be accepted. Applicants should continue to use the May 1995 revision of the PHS 398 and 2590 until further notice. The Application for a Public Health Service Individual National Research Service Award (PHS 416-1) is in the process of being revised. Revised forms should be available by the fall of 1998. The current form (August 1995 revision) has been extended to June 1998. Again, applicants should continue to use the current form until further notice.
