Purpose

This notice informs the community of an issue with the Fellowship administrative supplement package associated with PA-18-591 - Administrative Supplements to Existing NIH Grants and Cooperative Agreements (Parent Admin Supp Clinical Trial Optional).

The application package with Competition ID "FORMS-E-ADMINSUPP-FELLOWSHIP" included the PHS Inclusion Enrollment Report form in error. A corrected application package with Competition ID "FORMS-E-ADMINSUPP-FELLOWSHIP-REVISED" is now available and must be used for administrative supplements to Fellowship awards effective immediately.

Note: ASSIST, Workspace and many institutional system-to-system solutions allow application data to be easily copied from one application package to another. Contact appropriate support team for assistance (ASSIST - eRA Service Desk; Workspace - Grants.gov Contact Center).
