Final Progress Report Instructions

Final Progress Report Instructions are now posted at:, and are effective for awards with project end dates commensurate with the date of this Notice and beyond. There are no changes to the Final Progress Report instructions for non-SBIR/STTR Phase II awardees. Only the location of the instructions, formerly part of the PHS 2590, is different.

SBIR/STTR Phase II Final Progress Report instructions include a new format that clarifies the information requested on commercialization activities. There are two changes to the information required: (a) the All Personnel Report is no longer requested as part of the final report, and (b) the CMS reimbursement approval status of the product, process, or service, is requested. An optional sample format is posted at: and is strongly recommended to facilitate accurate and complete reports, and to enable more effective agency management of the SBIR/STTR program.
