REVISED PHS 2271 NOW AVAILABLE

EFFECTIVE DATE: The revised "Statement of Appointment Form" (PHS 2271,
Rev. 5/01) is now available and will be accepted immediately. For
appointments beginning on or after June 1, 2002, the new form is
required. Use of the previous version (Rev. 4/98) will be permitted
for appointments that begin on or before May 31, 2002. Please note,
the National Institutes of Health (NIH) must implement use of this
revised form in order to maintain compliance with the Office of
Management and Budget (OMB) standards for the collection of
race/ethnicity data.
The major change in this version of the form is the ability to capture
race/ethnicity data using a two-tiered reporting system. Individual
trainees will first indicate ethnicity and then indicate one or more
races. Another change is a new lexicon for describing the actual area
of research training for each trainee.
The revised instructions and form are available at
http://grants.nih.gov/grants/forms.htm#training. The form available on the
website has been modified to enable the fields to be filled in directly
using Adobe Acrobat Reader Software. Free Adobe Software may be
accessed at http://www.adobe.com/products/acrobat/readstep.html.
