Purpose

This Notice of Special Interest (NOSI) is issued by the National Institute of Neurological Disorders and Stroke (NINDS) to outline NINDS priorities for applications in the field of headache and migraine research in Fiscal Year 2022. Recognizing the urgent need to increase the study of neurological conditions associated with pain such as headache and migraine, the Fiscal Year 2022 draft budgets from the President, House, and Senate each include proposals to increase funding for pain research within NINDS mission. With this NOSI, NINDS wishes to inform the science community of the headache and migraine research opportunities that will present themselves, if funds become available. To be considered for funding through this initiative in Fiscal Year 2022, applications will need to be submitted to corresponding due dates starting in January or February 2022 so they can undergo Scientific Merit and NINDS Advisory Council review for the September 2022 Council round.

For funding consideration, applicants must include NOT-NS-22-051 (without quotation marks) in the Agency Routing Identifier field (box 4B) of the SF424 R&R form. Applications without this information in box 4B will not be considered for this initiative. Applications deemed nonresponsive to the terms of this notice will not be considered for this initiative.

Background:

Although there is still some uncertainty about the final availability of funds, we are excited by the potential to accelerate discovery and therapy development in this critical area, and we hope to encourage investigators to consider submitting applications that can advance our knowledge of the nature of pain and its treatment. Despite the prevalence of headache and migraine and the recent success in treatment opportunities, many questions remain regarding the pathophysiology, progression, and the multitude of endogenous and exogenous factors that influence headache and migraine. The following topic areas are not intended to be comprehensive or exhaustive. Synergistic studies that reach across two or more of these areas are encouraged. Interdisciplinary and multidisciplinary research is especially encouraged, as is research that involves specific cooperation between basic, translational, and clinical scientists.

Research areas of interest include:

Basic Science and Mechanistic Headache Research

  • Hypothesis-driven research that will elucidate mechanistic differences between pediatric and adult headache (developmental biology)
  • Given the variation in sex differences in adult, children and adolescents, investigations of the role of sex hormones in headache and migraine pathogenesis
  • Studies of Tension Type Headache
  • Studies of Trigeminal autonomic cephalalgias (TACs) such as cluster headache, paroxysmal hemicrania and short-lasting unilateral neuralgiform headache
  • Studies of Post traumatic headache in adult, children and adolescents
  • Exploration of mechanisms of onset and progression of headache in pediatric population
  • Studies of pain associated with light and sound in individuals with headache and migraine
  • Exploration of mechanisms of nervous system hypersensitivity including hyperalgesia, photophobia, phonophobia, and allodynia associated with migraine
  • Exploration of inflammatory processes related to headache and migraine
  • Basic research on the mechanisms and treatments of medication overuse headache (MOH)

Headache Research Tool Development

  • Age and developmentally appropriate tools to monitor headaches, including characterization and responses to therapy
  • Development and validation of animal models of headache and migraine phases and genetic models, including the evaluation of sex differences in those animal models

Basic Science and Clinical Headache Research in Humans

  • Differences in headache-relevant developmental pharmacology (esp. pharmacodynamics) from infancy to adolescence
  • Exploratory clinical trials of promising assets (drugs, biologics, devices, and/or other non-pharmacological treatments) for the treatment of headache in adults, children, and adolescents
  • Comparative effectiveness studies of established headache treatment approaches in adult and pediatric populations
  • Comparative effectiveness studies of established headache treatment approaches during early childhood, middle childhood, and adolescence
  • Evaluation of the influence of sex/gender in symptomatic presentation of migraine and nervous system hypersensitivity as it relates to pain
  • Basic and clinical research on the mechanisms and treatments of medication overuse headache

Studies of other primary headache such as nummular headache, hypnic headache and new daily persistent headache (NDPH) are also responsive to this NOSI.
