Through this Notice of Funding Opportunity (NOFO), the National Cancer Institute (NCI) invites research grant applications (R01) for the development and application of current and emerging molecular imaging methods to gain fundamental insights into cancer inflammation in vivo.
The motivation for this initiative is the recognition that much of the current imaging research on inflammation in cancer relies heavily on in vitro and ex vivo methods. These approaches have limited potential to provide significant insights into the dynamic interactions between cancer and inflammation. Utilizing molecular imaging probes in pre-clinical and clinical investigations allows for precise temporal resolution at molecular and cellular levels. This information is valuable for identifying and characterizing in vivo inflammatory cellular physiology in cancer and for detecting molecular changes in response to treatment.
This NOFO encourages applications focused on developing integrated imaging approaches to investigate the role of inflammation in cancer through strong cross-discipline collaboration between cancer basic science researchers and imaging scientists. These collaborations are expected to advance the science and understanding of cancer inflammation interactions.

Through this Notice of Funding Opportunity (NOFO), the National Cancer Institute (NCI) intends to support R01 projects that leverage cutting-edge in vivo imaging technologies to monitor specific cell populations or signaling pathways, aiming to elucidate the relationship between inflammation and tumor behavior. The insights gained are expected to provide novel mechanistic understandings of the dynamic changes in cellular physiology related to inflammation in cancer. This knowledge will be utilized to reduce disease burden by accelerating tumor-specific therapeutic approaches and improving the monitoring of therapeutic responses. Because of the multidisciplinary nature of the NOFO, applicants must be led by Multiple Principal Investigators in imaging and cancer biology who will contribute unique expertise and scientific insights toward the successful completion of the proposed research.
Several molecules, signaling pathways, and cellular infiltrates play crucial roles in the biological behavior of tumors. These include cytokines, chemokines, growth factors, inflammasomes, tumor-infiltrating immune cells, tumor stroma, and immune checkpoints. Given the initial successes of immunotherapies, further research is needed to understand the targets and mechanisms involved in cancer-related inflammation and immunity. The complex network of inflammatory pathways offers multiple diverse targets for gaining new insights into cancer biology.
Currently, clinical evaluations for cancer-associated inflammation rely largely on immunological assays of biological fluids and tissue samples. While these diagnostic tools are specific and sensitive, they do not reveal the dynamic tumor immune cellular physiology or the inter- and intra-lesion heterogeneity in these interactions. Non-invasive methods that can assess the multiple dimensions of the dynamic processes in tumors are needed to gain a more in-depth understanding of the role inflammation plays in shaping tumor-host interactions in human cancer. Imaging technology has the capability to monitor these tumor-inflammation interactions in vivo and in real-time. Applying these approaches will advance our understanding of cancer biology and therapeutic effects.
Tremendous opportunities exist for the application of in vivo imaging at the molecular and cellular levels to provide information on how immune cells traffic and interact throughout the entire organism. The need for in vivo imaging methods to study cancer inflammation dynamics at these levels was identified at a 2019 Trans-NIH Workshop on Imaging Inflammation in Health and Disease. The conference highlighted multiple technological advances, challenges, and needs in visualization tools that can generate high-resolution spatiotemporal images to capture the heterogeneity of cancer-associated inflammation biology. It also found that imaging techniques used effectively in other disease research areas could be applied to analyzing the role of cancer-associated inflammation in vivo. This NOFO provides research support to develop or implement innovative molecular imaging technologies that can explore well-characterized inflammatory targets in real-time, aiming to better understand the crucial physiological influence of inflammation on cancer cell behavior.
This NOFO aims to stimulate cancer inflammation imaging research by combining the efforts of imaging- and biology-based scientists as Multiple Principal Investigators, fostering interdisciplinary interactions among Cancer Biology, Immunology, and Imaging researchers. The Cancer Biology and Immunology component, which investigates and validates known cancer inflammation targets using state-of-the-art molecular and cellular assays, may serve as the foundation for developing molecular imaging probes (e.g., small molecules, peptides, aptamers, antibodies, engineered proteins, and nanoparticles) and approaches capable of monitoring relevant cancer inflammation signatures.
It is anticipated that in vivo measurements, including the development of imaging probes with target specificity, sensitivity, stability, optimal biodistribution, and low toxicity, conducted in the Imaging component, will provide deeper insights into cellular signaling, pro- versus anti-inflammatory mediators, and feedback to the Cancer Biology and Immunology component for a better understanding of cancer behavior mechanisms. Key personnel should demonstrate a willingness to collaborate outside their own disciplines. The number of project lead partners and the nature of these interactions will be determined by the applicants to establish creative, productive, and synergistic collaborations. Collaborative activities should go beyond merely providing reagents and materials; they are expected to involve the integration and exchange of ideas, knowledge, data, and methodologies to contribute to hypothesis-driven projects. Applications with a single PD/PI will be considered non-responsive and will be administratively withdrawn without peer review.
Under this NOFO, research proposing the combination of in vivo whole organism imaging studies with multi-omics approaches or with in vitro and ex vivo imaging methods (e.g., multi-omics mass spectrometry imaging, multiplex spatial imaging) to address mechanistic hypotheses is highly encouraged. The use of quantitative molecular imaging approaches at the multimodal and multiscale levels to study and monitor in vivo inflammatory responses throughout the tumor lifespan is within the scope of this NOFO. The translation of existing imaging agents and quantitative tools developed for other inflammation-associated diseases to investigate the biology of cancer-associated inflammation is also supported by this NOFO.

Specific to this NOFO: Evaluate whether the underlying premise and scientific foundation of the project and the experimental design support a team science approach. For studies that involve design and development of a new molecular imaging probe for a known cancer associated inflammation target, evaluate whether it describe the preliminary analytical performance of the chosen biomarker assay(s) (e.g., laboratory validation studies demonstrating the accuracy, precision, sensitivity, specificity, reportable range of test results, reference intervals (normal values) with controls and calibrators, and reproducibility) that demonstrate suitability for in vivo imaging and validation testing. For each stage of validation proposed, evaluate whether the proposed data acquisition results achieve statistical significance.

For research that involves human subjects but does not involve one of the categories of research that are exempt under 45 CFR Part 46, evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects; 2) adequacy of protection against risks; 3) potential benefits to the subjects and others; 4) importance of the knowledge to be gained; and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the categories of research that are exempt under 45 CFR Part 46, evaluate: 1) the justification for the exemption; 2) human subjects involvement and characteristics; and 3) sources of materials. When the proposed research includes Vertebrate Animals, evaluate the involvement of live vertebrate animals according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. For projects involving key biological and/or chemical resources, evaluate the brief plans proposed for identifying and ensuring the validity of those resources.

Applications will compete for available funds with all other recommended applications submitted in response to this NOFO. Following initial peer review, recommended applications will receive a second level of review by the appropriate national Advisory Council or Board. If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement Section 2.5.1. A Notice of Award (NoA) is the official authorizing document notifying the applicant that an award has been made and that funds may be requested from the designated HHS payment system or office. The NoA is signed by the Grants Management Officer and emailed to the recipients business official.
In accepting the award, the recipient agrees that any activities under the award are subject to all provisions currently in effect or implemented during the period of the award, other Department regulations and policies in effect at the time of the award, and applicable statutory provisions.

Institutional Review Board or Independent Ethics Committee Approval: Recipient institutions must ensure that protocols are reviewed by their IRB or IEC. To help ensure the safety of participants enrolled in NIH-funded studies, the recipient must provide NIH copies of documents related to all major changes in the status of ongoing protocols.
The following Federal wide and HHS-specific policy requirements apply to awards funded through NIH:
All federal statutes and regulations relevant to federal financial assistance, including those highlighted in NIH Grants Policy Statement Section 4 Public Policy Requirements, Objectives and Other Appropriation Mandates.
Recipients are responsible for ensuring that their activities comply with all applicable federal regulations. NIH may terminate awards under certain circumstances. See 2 CFR Part 200.340 Termination and NIH Grants Policy Statement Section 8.5.2 Remedies for Noncompliance or Enforcement Actions: Suspension, Termination, and Withholding of Support.

Consistent with the 2023 NIH Policy for Data Management and Sharing, when data management and sharing is applicable to the award, recipients will be required to adhere to the Data Management and Sharing requirements as outlined in the NIH Grants Policy Statement. Upon the approval of a Data Management and Sharing Plan, it is required for recipients to implement the plan as described.
When multiple years are involved, recipients will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement Section 8.4.1 Reporting. A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement Section 8.6 Closeout. NIH NOFOs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 2 CFR Part 200.301.
