The Office of Extramural Research (OER) is revising the Ruth L. Kirschstein National Research Service Award Individual Fellowship Application (PHS 416-1) and Ruth L. Kirschstein National Research Service Award Individual Fellowship Progress Report for Continuation Support PHS 416-9). The Application and Progress Report are used to request all individual fellowship support under the auspices of the Ruth L. Kirschstein National Research Service Award authority. The forms and instructions are used by the National Institutes of Health (NIH) and Agency for Healthcare Research and Quality (AHRQ). This Notice solicits suggestions to improve these instructions and form pages. Submit suggestions by February 18, 2005 to: FellowshipRevision@mail.nih.gov.

Primary goals of the revision process will be to enhance quality and clarity, with special emphasis on streamlining and plain language efforts.

The current PHS 416-1 and PHS 416-9 maintain OMB Clearance through 6/2005. The NIH will publish an announcement in the NIH Guide for Grants and Contracts when the new forms and instructions are available for use. There will be a transition period when the current or revised forms will be accepted.
