The Agency for Health Care Research and Quality (AHRQ) informs the
research community that applications for AHRQ Independent Scientist 
(K02) and Mentored Clinical Scientist (K08) awards should contain a 
detailed budget for the initial budget period (Form page 4) and a 
categorical budget for the entire proposed period of support (Form page 
5).  A budget justification as described on page 59 of the PHS 398 
Additional Instructions for Preparing Individual Research Career Award 
(RCA) Applications, should also be included.

These form pages are contained in the Application for Public Health 
Service Grant (PHS 398 revised May 2001)

Details relevant to career development award application preparation 
can be found under "Research Career Awards," Section IV, pages 1-7 of 
the PHS 398.  Applicants need to address both the instructions 
contained in PHS 398 and those described under "Application Procedures" 
in the Program Announcements.  The relevant AHRQ program announcements 
are PAR-99-164, Independent Scientist Award and PA-00-010, Mentored 
Clinical Scientist Development Awards.  In instances in which 
instructions differ, instructions in this notice and in the Program 
Announcements take precedence over instructions contained in the PHS 
398 application.   

Specifically, instructions 4 and 5 (p. 59) of the PHS 398 - Additional 
Instructions for Preparing Individual Research Career Award (RCA) 
Applications   do not apply to AHRQ Career Applications.   The 
following statements in the PHS 398 should not be followed:  1. Do not 
complete the Form page 4;  2. Do not complete the categorical budget 
table on Form page 5, and 3. (under budget justification) No specific 
costs for items or categories should be shown.
