Background
Although there are many high-quality opioid treatment programs and office-based prescribers and providers of FDA-approved medications for the treatment of opioid use disorder (OUD), the overall quality of treatment for OUD in the United States could be improved. One-size-fits all, episode-based, non-patient-centered models of care are common. FDA-approved medications are not routinely used when indicated and often are discontinued too quickly because of factors at the patient, clinician, and health care reimbursement levels. Patients may not be assessed sufficiently by qualified personnel for comorbidities, use of other medications and drugs, or other factors that may inform the provision of and sufficient engagement and retention in safe, effective, treatment. Patients are admitted to residential rehabilitation programs or inpatient withdrawal management programs but then not continued in evidence-based treatment with medications for OUD after discharge. Programs may discharge patients for resuming substance use without determining if additional treatment or a different approach might enhance patient success. Programs may impose rules or maintain schedules that make it difficult for patients to comply with the treatment regimen and are not necessary for the provision of high-quality treatment. Payers may impose strict prior authorization requirements and limit coverage for treatment with OUD medications for what is often a chronic, life-threatening disorder. Objective information that can help patients and families select a provider is rarely available.
For decades, quality measurement has been promoted as a way to improve the effectiveness and safety of health care services. Theoretically, quality measures can highlight areas of weakness and track progress toward improvement. Quality measures can be used by patients, families, and payers to select and reward providers, promoting enrollment in high-quality providers and improvement among those of lower quality. Most measure development has been based on Donabedian's Structure, Process and Outcome model. Structural measures indicate a health care provider's capacity to provide high-quality treatment, such as the qualifications of its staff or availability of evidence-based practices. Process measures indicate provider adherence to generally accepted recommendations for clinical practices, and such as the proportion of patients who receive blood pressure screening. Outcome measures reflect the effect of the health care service or intervention on patient outcomes. While criteria such as importance, reliability, validity, comprehensibility, scientific acceptability, and usability have been delineated to promote the development of strong measures, too often the practice guidelines from which they are derived are insufficiently supported by scientific evidence. Also, while use of available data is an important consideration in current practice, it has limited innovation in measurement. While there have been some notable examples of quality measures developed under this paradigm fostering improvement, there have also been some notable failures. Given the explosion in the number of measures and concomitant investments in measuring and reporting, re-thinking the effort within the broader health care sector has begun.
Within the substance use disorder treatment sector, dozens of quality measures, mostly process measures, have been generated and there are repeated calls to develop more. Examples of existing measures include the widely-used HEDIS treatment initiation and engagement measures, the SAMHSA National Outcome Measures, measures developed and implemented by various federal and state authorities or by professional organizations, and measures used in the recent British National Health Service Payment for Results pilot. Those that exist mostly were developed by expert consensus panels. The measures were then carefully specified in claims or other administrative data to select the appropriate numerator and denominator codes, determine if there was a need for improvement, plausible benchmarks, etc. Often the measures were developed in response to changes in the health care system, such as the expansion of managed behavioral health organizations in the 1990s or the Meaningful Use provisions of the Health Information Technology for Economic and Clinical Health (HITECH) Act in the late 2000s. Measures have been adopted by states and other payers and included pay-for-performance initiatives. Sometimes, the measures improved, but, more often there was little or no change. Although a substantial literature exists that reports on the statistical associations of several of these measures with patient outcomes, there has been little rigorous research establishing a causal link between efforts to improve the measures and improved patient outcomes.
There is a need for rigorous, scientific research to develop and test means to improve quality measures to support their use in quality improvement initiatives. Such research would do the following:
This FOA solicits applications for conducting such research for OUD treatment.
Research Goals
Applications proposing developmental work with possible transition to a randomized, controlled clinical trial to determine the effects of strategies for measuring and improving the quality OUD treatment are sought.
To be responsive to this FOA, applicants must propose the following activities in addition to any other activities that may be necessary for the success of their proposed project:
1. Identify high-priority target(s) for improving the safety and effectiveness of OUD treatment.
2. Develop new and/or select existing quality measure(s) to assess improvement in the target(s).
3. Develop a plan for improving the quality measure(s). The plan must include, but need not be limited to, the following:
4. Conduct a randomized, controlled trial. The trial must test the efficacy or effectiveness of the improvement approach on the selected quality measure(s) and on patient health outcomes (e.g. opioid and other drug use, overdose, overdose death, other health outcomes such as infectious diseases, functional status) as primary outcomes. Other outcomes, including criminal justice and social outcomes, may be relevant as secondary outcomes depending on the project.
5. Identify, quantify, and where possible, monetize, the resources used by measured entities to compute and report on the quality measure(s), to conduct the proposed improvement plan, and sustain the improvement. The application may also propose an economic evaluation such as a cost-effectiveness or cost-benefit analysis.
6. Identify and assess unintended consequences. Examples might include effects on service availability, patient casemix, or effects on non-measured outcomes.
7. If the intervention is successful, develop and disseminate materials such as protocols and tools, including appropriate benchmarks and target levels and case-mix adjustment if relevant, to enable other similarly situated entities to implement these measures and improvement strategies.
8. Regardless of whether the intervention is successful, disseminate the results and lessons through scientific publication and other appropriate channels.
Relevant projects may be conducted at any level of the general or specialty health care system where quality measurement and management is being or could be used as a tool to improve the safety and effectiveness of OUD treatment.
Responsive projects must involve at least one organization that intends to adopt the measure(s) for OUD treatment quality monitoring and improvement should the trial yield positive results. Such organizations may include treatment programs, physician practices, government entities charged with regulating, monitoring, and/or paying for treatment using FDA-approved medications for those with opioid use disorders, private insurance companies and health plans, or other organizations that would use quality measurement and management to improve OUD treatment. The appropriate level and manner of organizational involvement likely will vary depending on the project, but must be described in the application and confirmed in a letter of support. Examples of involvement may include, but are not limited to, the following: Organization personnel serving as senior/key personnel, consultants, or other contributors (defined here: https://grants.nih.gov/grants/policy/senior_key_personnel_faqs.htm#1660); resources including data made available to the project; information needed to recruit and randomize subjects; or other contributions to the research.
The focus of this FOA is on improving the clinical quality of OUD care using FDA-approved medications that is provided to patients with OUD. Examples of relevant of quality improvement topics may include, but are not limited to, the following:
Strategies for improvement must be specific to the selected improvement target(s) and measure(s) and must consider specific changes in processes, treatment types and duration, personnel, or other factors that would allow a measured entity to improve on the measure(s).
Applications that do not identify possible targets and approaches are not responsive to this FOA and will be withdrawn without review.
Applications that propose to develop quality improvement plans that do not pre-specify targets and approaches are not responsive to this FOA and will be withdrawn without review.
Applications proposing to improve measures reflecting only the receipt, frequency, and/or timing of vaguely defined services (e.g. office visits or counseling ) are not responsive to this FOA and will be wihtdrawn without review.
Applications that do not describe and include a letter of support confirming the involvement of an organization that does or could use quality measures to monitor and improve OUD treatment and the organization s intention to implement the measures should the trial yield positive results are not compliant with this FOA and will be withdrawn without review.
Phased Innovation Award
This FOA uses a bi-phasic, milestone-driven R61/R33 mechanism. Proposed projects should include both an R61 phase, with milestone-driven developmental activities, and an R33 phase conducting a clinical trial to test the effectiveness of the proposed plan. Applications proposing only either R61 or R33 activities alone will not be accepted.
Preliminary data are not required for this FOA, but may be included if available.

Milestones and R61/R33 Transition
The application must propose at least three milestones, including at least one go/no-go milestone (described below), for transition to the R33 phase. Prior to funding an application, program staff will contact the applicant to discuss the proposed transition milestones. Program staff and the applicant will negotiate and agree on a final set of transition milestones. These will be incorporated into the terms and conditions of the award and will be the basis for judging the success of the R61 work.
Transition to the R33 phase is contingent upon programmatic review that will include, but is not limited to, satisfactory demonstration that the R61 transition milestones have been achieved. Transition to the R33 phase is not guaranteed.
A continuation application package will be required 60-75 days prior to the anticipated start of the R33 phase. The package will include a progress report summary, a section labeled "Milestones" describing the progress made on each milestone, an updated research plan for the R33 phase, and other information relevant to a particular project that will be specified in the terms and condition of the R21 award.

NIDA intends to commit $2.5 million in FY 2019 to fund 5-6 awards.
Direct costs will vary with the scope of the project. The R61 phase direct costs must not exceed $300,000 in any one year. The R33 phase direct costs must not exceed $500,000 in any one year.
The scope of the proposed project should determine the project period. The maximum period of the combined R61 and R33 phases is 5 years, with up to 2 years for the R61 phase and up to 4 years for the R33 phase. Applications with a project period of less than 5 years are encouraged where feasible.

Specific Aims
The application should contain separate Specific Aims sections for the R61 and R33 phases in the single Specific Aims attachment.
Research Strategy
In preparing the R61/R33 application, investigators should consider that the application will be assigned a single overall impact score. Thus, clarity and completeness about specific goals and the feasibility of each phase and the transition milestones are critical.
The Research Strategy should include separate Approach sections for the R61 and R33 phases in the single Research Strategy attachment. Separate Significance and Innovation sections may be included, but may be combined into a single section within the R61 section as appropriate. It is not necessary to repeat any information or details in the R33 section that are described in the R61 section.
Any preliminary data that will support or justify the proposed hypothesis, rationale, or development plan may be included. However, preliminary data are not required for this FOA.
The choice of randomized, controlled trial design will depend on project goals. The statistical analysis and power calculations should reflect the chosen design and the implications for data analysis methods and sample size estimation. For example, some applications may propose cluster-randomized trials, in which groups of participants are randomized to study conditions and interventions are delivered to members of those groups. Interactions occur among participants in the same cluster both pre- and post-randomization. Modifications to standard approaches must be made to address intraclass correlation expected in data obtained from participants in the same groups or clusters. The methods proposed for data analysis and the methods used for sample size calculation should reflect both the extra variation expected in the data and the degrees of freedom available to estimate that extra variation. The application must address each of the 8 factors (high-priority targets, quality measures, plan for improving the measures, randomized trial, resources, unintended consequences, and dissemination) delineated in the Research Goals section.

Transition Milestones
A Transition Milestones section must be included in the Research Strategy attachment. It must propose at least 3 transition milestones for completion of the R61 phase, a discussion of the suitability of the proposed milestones for assessing success in the R61 phase, and a discussion of the implications of successful completion of these milestones for the proposed R33 study. Transition milestones should be specific, quantifiable, and scientifically justified; they should not be simply a restatement of the R61 specific aims. At least one go/no go transition milestone must involve an assessment of the feasibility and value of the proposed R33, using information obtained in the R61 phase.

This FOA supports investigation of novel scientific ideas or new interventions, model systems, tools, or technologies that have the potential for significant impact on biomedical or behavioral and social sciences research. An application to this FOA need not have preliminary data, extensive background material or preliminary information; however, they may be included if available. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Accordingly, reviewers will emphasize the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Reviewers will assign a single impact score for the entire application, which includes both the R61 and R33 phases.
Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).
Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Does the project address an important problem or a critical barrier to progress in the field? Is there a strong scientific premise for the project? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?
Are the scientific rationale and need for a clinical trial to test the proposed hypothesis or intervention well supported by preliminary data, clinical and/or preclinical studies, or information in the literature or knowledge of biological mechanisms? For trials focusing on clinical or public health endpoints, is this clinical trial necessary for testing the safety, efficacy or effectiveness of an intervention that could lead to a change in clinical practice, community behaviors or health care policy? For trials focusing on mechanistic, behavioral, physiological, biochemical, or other biomedical endpoints, is this trial needed to advance scientific understanding?
Are the PD(s)/PI(s), collaborators, and other researchers well suited to the project? If Early Stage Investigators or those in the early stages of independent careers, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?
With regard to the proposed leadership for the project, do the PD/PI(s) and key personnel have the expertise, experience, and ability to organize, manage and implement the proposed clinical trial and meet milestones and timelines? Do they have appropriate expertise in study coordination, data management and statistics? For a multicenter trial, is the organizational structure appropriate and does the application identify a core of potential center investigators and staffing for a coordinating center?
Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?
Does the design/research plan include innovative elements, as appropriate, that enhance its sensitivity, potential for information or potential to advance scientific knowledge or clinical practice?
Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project? Have the investigators presented strategies to ensure a robust and unbiased approach, as appropriate for the work proposed? Are potential problems, alternative strategies, and benchmarks for success presented? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed? Have the investigators presented adequate plans to address relevant biological variables, such as sex, for studies in vertebrate animals or human subjects?
If the project involves human subjects and/or NIH-defined clinical research, are the plans to address 1) the protection of human subjects from research risks, and 2) inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion or exclusion of children, justified in terms of the scientific goals and research strategy proposed?

Study Design
Is the study design justified and appropriate to address primary and secondary outcome variable(s)/endpoints that will be clear, informative and relevant to the hypothesis being tested? Is the scientific rationale/premise of the study based on previously well-designed preclinical and/or clinical research? Given the methods used to assign participants and deliver interventions, is the study design adequately powered to answer the research question(s), test the proposed hypothesis/hypotheses, and provide interpretable results? Is the trial appropriately designed to conduct the research efficiently? Are the study populations (size, gender, age, demographic group), proposed intervention arms/dose, and duration of the trial, appropriate and well justified?
Are potential ethical issues adequately addressed? Is the process for obtaining informed consent or assent appropriate? Is the eligible population available? Are the plans for recruitment outreach, enrollment, retention, handling dropouts, missed visits, and losses to follow-up appropriate to ensure robust data collection? Are the planned recruitment timelines feasible and is the plan to monitor accrual adequate? Has the need for randomization (or not), masking (if appropriate), controls, and inclusion/exclusion criteria been addressed? Are differences addressed, if applicable, in the intervention effect due to sex/gender and race/ethnicity?
Are the plans to standardize, assure quality of, and monitor adherence to, the trial protocol and data collection or distribution guidelines appropriate? Is there a plan to obtain required study agent(s)? Does the application propose to use existing available resources, as applicable?
Data Management and Statistical Analysis
Are planned analyses and statistical approach appropriate for the proposed study design and methods used to assign participants and deliver interventions? Are the procedures for data management and quality control of data adequate at clinical site(s) or at center laboratories, as applicable? Have the methods for standardization of procedures for data management to assess the effect of the intervention and quality control been addressed? Is there a plan to complete data analysis within the proposed period of the award?
Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?
If proposed, are the administrative, data coordinating, enrollment and laboratory/testing centers, appropriate for the trial proposed?
Does the application adequately address the capability and ability to conduct the trial at the proposed site(s) or centers? Are the plans to add or drop enrollment centers, as needed, appropriate?
If international site(s) is/are proposed, does the application adequately address the complexity of executing the clinical trial?
If multi-sites/centers, is there evidence of the ability of the individual site or center to: (1) enroll the proposed numbers; (2) adhere to the protocol; (3) collect and transmit data in an accurate and timely fashion; and, (4) operate within the proposed organizational structure?
As applicable for the project proposed, reviewers will evaluate the following additional items while determining scientific and technical merit, and in providing an overall impact score, but will not give separate scores for these items.
Transition Milestones
Given the critical nature of the milestones for the potential R61 to R33 transition, are the proposed transition milestones well-defined with quantifiable measures that are appropriate for assessing the success of the R61 phase of the application? Is a "go/no go" measure identified? Do the transition milestones have specific quantifiable criteria that will enable clear decisions about their attainment? Is it clear how the R33 phase of the study will develop and expand once the R61 transition milestones are achieved? Given the potential benefits of the proposed research, do the transition milestones support the transition and will the overall project advance the intervention, method, or strategy?
Study Timeline
Is the study timeline described in detail, taking into account start-up activities, the anticipated rate of enrollment, and planned follow-up assessment? Is the projected timeline feasible and well justified? Does the project incorporate efficiencies and utilize existing resources (e.g., CTSAs, practice-based research networks, electronic medical records, administrative database, or patient registries) to increase the efficiency of participant enrollment and data collection, as appropriate?
Are potential challenges and corresponding solutions discussed (e.g., strategies that can be implemented in the event of enrollment shortfalls)?
For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of children to determine if it is justified in terms of the scientific goals and research strategy proposed. For additional information on review of the Inclusion section, please refer to the Guidelines for the Review of Inclusion in Clinical Research.
The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. Reviewers will assess the use of chimpanzees as they would any other application proposing the use of vertebrate animals.
