The diagnosis and treatment of many heart, lung, blood diseases, and sleep (HLBS) disorders are still mainly based on symptoms instead of a molecular understanding of disease pathobiology. To better understand the pathobiological bases of diseases, NHLBI recently launched the Trans Omics for Precision Medicine (TOPMed) program, which is using high throughput omics technologies to characterize molecular abnormalities or signatures associated with HLBS disorders. TOPMed plans to build an information commons with a user interface that will allow members of the research community to integrate clinical and omics datasets and employ state-of-art tools for data analysis. TOPMed will include data from tens of thousands of well-phenotyped individuals, including whole genome sequences (WGS), genome wide methylation (GWM) profiles, RNA-sequencing data, and metabolomic profiles.
Although thousands of genes are expressed in every cell in humans, analyses of genomics data typically focus on one gene or one variant at a time and use simple p-values to assess the biological significance of variations. More advanced analyses that integrate TOPMed trans-omics datasets (DNA, RNA, and metabolite) from the same individuals would provide a tremendous opportunity to build systems level models to more accurately represent the underlying biological networks as the result of gene-gene and gene-environment interactions. This FOA intends to support investigators developing analytic methods that will use TOPMed data to achieve in-depth understanding of disease pathobiology at the system level.
The large and complex data sets from TOPMed raise both challenges and opportunities, examples of which are listed below. This FOA provides investigators the opportunity to pursue their own research interests in relevant areas such as computational biology, data science or population genetics, genetic epidemiology or another genotype-phenotype approach while building a data-analytical capacity in TOPMed that can be utilized by investigators in the research community at large.
Awardees will be expected to join the collaborative TOPMed team and participate in collaborative research with the multidisciplinary teams being assembled through this program. The research proposed by applicants should address both immediate needs in TOPMed and long term challenges and opportunities in integrative omics research and data science.
Applicants are strongly encouraged to use multidisciplinary approaches to identify the major challenges for genomics analysis and design solutions to meet current research needs. TOPMed currently has more than two dozen groups that are experts in heart, lung, blood diseases, and sleep (HLBS) disorders; epidemiology; genomics; and informatics. It is intended that applicants will build collaborations with multiple TOPMed groups and use TOPMed data and other existing data to develop and validate their methods and tools. Awardees will join steering committee activities through regular teleconferences and two annual in-person meetings at NIH.
Applicants are not required to develop completely novel tools or methods but may modify or repurpose existing tools to more effectively and efficiently extract biological information from high-dimensionality data. Applications that emphasize their potential to advance understanding of the biological basis of HLBS disorders through omics analyses are highly encouraged. All software tools developed under this FOA are expected to perform in a public cloud environment. This FOA intends to support applications that show a plan to address portability and interoperability of their software tools in any of the categories applicable, including data portability, application portability, platform portability, application interoperability, and platform interoperability. In addition, a plan for open source software and dissemination to benefit the research community is expected.
Examples of appropriate research objectives include but are not limited to:
Cooperative Agreement: A support mechanism used when there will be substantial Federal scientific or programmatic involvement. Substantial involvement means that, after award, NIH scientific or program staff will assist, guide, coordinate, or participate in project activities. NHLBI intends to commit total costs of $3,100,000 in FY 2017, $3,100,000 in FY 2018, and $3,100,000 in FY 2019 to fund up to 5 awards.
Application budgets may not exceed $400,000 in direct costs per year and need to reflect the actual needs of the proposed project.

If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?
Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Cooperative Agreement Terms and Conditions of Award
The following special terms of award are in addition to, and not in lieu of, otherwise applicable U.S. Office of Management and Budget (OMB) administrative guidelines, U.S. Department of Health and Human Services (DHHS) grant administration regulations at 2 CFR 200), and other HHS, PHS, and NIH grant administration policies.
The administrative and funding instrument used for this program will be the cooperative agreement, an "assistance" mechanism (rather than an "acquisition" mechanism), in which substantial NIH programmatic involvement with the awardees is anticipated during the performance of the activities. Under the cooperative agreement, the NIH purpose is to support and stimulate the recipients' activities by involvement in and otherwise working jointly with the award recipients in a partnership role; it is not to assume direction, prime responsibility, or a dominant role in the activities. Consistent with this concept, the dominant role and prime responsibility resides with the awardees for the project as a whole, although specific tasks and activities may be shared among the awardees and the NIH as defined below.
The PD(s)/PI(s) will have the primary responsibility for:
All aspects of this analytical method development initiative. This is to ensure that every step along the software development life cycle (SDLC) will be fully supported. Specifically, the SDLC will include 1) requirement analysis, 2) design, 3) implementation, 4) testing, 5) improving, and back to step 1 if needed.
Awardees will retain custody of and have primary rights to the results and software developed under these awards, subject to Government rights of access consistent with current DHHS, PHS, and NIH policies. Upon completion of the project, recipients are expected to put all tools developed under this FOA into the public domain and/or make them available to other investigators, according to the approved plan for making data and materials available to the scientific community and the NHLBI for the conduct of research at no charge other than the costs of reproduction and distribution.
One PI for each award will participate as a member in the NHLBI TOPMed consortium (http://www.nhlbi.nih.gov/research/resources/nhlbi-precision-medicine-initiative/topmed) and in the Steering Committee of that consortium.
NIH staff has substantial programmatic involvement that is above and beyond the normal stewardship role in awards, as described below:
An NHLBI Project Scientist (PS) will be responsible for the scientific stewardship of the award and will be named in the award notice. The assigned Program Officer (PO) will be responsible for normal programmatic management and not serve as the NIH Project Scientist. The Project Scientist (PS) may be a member of Steering Committee, Subcommittees, or working groups.
Progress will be administratively reviewed prior to issuance of non-competing awards. NHLBI reserves the right to phase out or curtail an individual award in the event of: (1) significant under performance; (2) inability to work as member in the TOPMed consortium; or (3) a major breach or substantive changes in the agreed-upon data-sharing and open source policy required by this initiative.
Annual continuation and level of funding will be based on NHLBI review of overall performance, determined as part of the NHLBI review of the annual non-competing continuation grant progress reports submitted by the awardees. A Program Officer will be responsible for the programmatic stewardship of the award and will be named in the award notice.
Areas of Joint Responsibility include:
Awardees agree to the governance of the study through TOPMed’s Steering Committee (SC). All PD(s)/PI(s) and a Chairperson, to be appointed by the NHLBI, will comprise the SC. All major scientific decisions will be determined by majority vote of the SC. Each grant will have one vote, the NHLBI representative PS will have one vote; and the Chair will have one vote in case of a tie. Awardee members of the Steering Committee will be required to accept and implement policies approved by the Steering Committee.
Dispute Resolution:
Any disagreements that may arise in scientific or programmatic matters (within the scope of the award) between award recipients and the NIH may be brought to Dispute Resolution. A Dispute
Resolution Panel composed of three members will be convened. It will have three members: a designee of the Steering Committee chosen without NIH staff voting, one NIH designee, and a third designee with expertise in the relevant area who is chosen by the other two; in the case of individual disagreement, the first member may be chosen by the individual awardee. This special dispute resolution procedure does not alter the awardee's right to appeal an adverse action that is otherwise appealable in accordance with PHS regulation 42 CFR Part 50, Subpart D and DHHS regulation 45 CFR Part 16.
When multiple years are involved, awardees will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.
A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.
The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000.
