The purpose of this Funding Opportunity Announcement (FOA) is to provide support for research designed to optimize smoking cessation treatment among people living with HIV (PLWH) in the United States (U.S.). Responsive applications must propose research that will be conducted with PLWH and will inform efforts to reduce the incidence of tobacco-related disease and death among PLWH. Research may address the behavioral and sociocultural factors and conditions that are associated with cigarette smoking among PLWH and may also address smoking-related health disparities among PLWH, considering the heterogeneity across the various subgroups of PLWH. This FOA aims to support research to systematically test existing evidence-based smoking cessation interventions (e.g., combination of behavioral and pharmacological) and/or to develop and test adaptations of evidence-based smoking cessation interventions among PLWH. The principal focus of this initiative is on cigarette smoking cessation; however, studies that address dual/poly tobacco product use as part of a cigarette smoking cessation intervention are acceptable. Proposed projects must include prospective, comparative evaluation(s) of the intervention(s) in terms of the rates of cigarette smoking cessation, including sustained abstinence, among current cigarette smokers.

The goal of this Funding Opportunity Announcement (FOA) is to improve cigarette smoking cessation treatment among people living with HIV (PLWH) in the United States (U.S.), with an emphasis on methodologically rigorous studies that propose to systematically test existing evidence-based smoking cessation interventions (e.g., combination of behavioral and pharmacological) and/or to develop and test adaptations of evidence-based smoking cessation interventions for application to this target population. The long-term goal is to reduce cigarette smoking rates among PLWH, and thus tobacco-related health disparities in this population. Responsive applications should address the highest HIV/AIDS research priorities as identified by NIH (see NOT-OD-15-137) which include research to reduce health disparities in the treatment outcomes of those living with HIV/AIDS.
This FOA will utilize the NIH Exploratory/Developmental Research Grant (R21) mechanism, and is suitable for early phase, pilot, or exploratory/developmental projects. For such projects, preliminary data are not required; however, they may be included if available.
Investigators who are interested in proposing larger scale, later phase projects based upon strong preliminary data should submit applications in response to the partner FOA of identical scientific scope, RFA-CA-18-027, which uses the Research Project Grant (R01) mechanism.
There are an estimated 1 million PLWH in the U.S. Of these, an estimated 40% smoke cigarettes, more than twice the prevalence observed in the U.S. adult population. Consequently, PLWH experience substantial tobacco-related morbidity and mortality, including from tobacco-related cancers. Thus, addressing tobacco cessation as early in life as possible among PLWH is extremely important. For PLWH who smoke, after achieving and maintaining a suppressed viral load, smoking cessation is the next most important intervention that will maximize both quality of life and life expectancy.
HIV/AIDS and Cancer
Antiretroviral therapy (ART) has prolonged the life expectancy among PLWH in the U.S., such that HIV infection is now considered a chronic health condition. With early diagnosis and access to treatment, the life expectancy of PLWH is very similar to that of people without HIV, and death from AIDS-related causes is decreasing. However, the incidence of both AIDS-defining (i.e., non-Hodgkin lymphoma, Kaposi sarcoma, and cervical cancer) and non-AIDS-defining (i.e., anal, colorectal, Hodgkin lymphoma, liver, lung, oropharyngeal, melanoma) cancers are higher among PLWH compared to the general population. Indeed, non-AIDS-defining cancers are now the leading non-AIDS cause of death among PLWH. Impaired immune function, chronic inflammation, and a higher prevalence of risk factors, such as cigarette smoking and viral co-infections, contribute to this substantial cancer burden. The mortality rate from cancer among PLWH has been estimated to be more than three times higher than that of uninfected persons.
Lung cancer is the leading cause of cancer death among PLWH on ART and is one of the leading causes of death overall in this population. The incidence of lung cancer in PLWH appears to be higher than can be explained by smoking alone. Lung infections, chronic inflammation, and other immune stressors may act in synergy with tobacco use to promote lung cancer development among PLWH. Generally, lung cancer is diagnosed a decade or more earlier among PLWH than those without, and it is estimated that 94% of lung cancer diagnoses among PLWH could potentially be prevented by eliminating cigarette smoking in this population.
Cigarette Smoking among PLWH
As noted above, the prevalence of cigarette smoking among PLWH is more than twice that of the general adult U.S. population. It is estimated that life expectancy among HIV-positive smokers is reduced by at least 16 years compared with HIV-positive nonsmokers. Further, cigarette smoking is associated with lower adherence to ART and can influence HIV pathogenesis. It is important to note that PLWH are not a homogeneous group. The Centers for Disease Control and Prevention reports that gay and bisexual men are the most affected by HIV, with 70% of new infections in 2014 occurring within this group; however, they note differences in infection trends by age and race/ethnicity. Sexual and gender minorities (e.g., lesbian, gay, bisexual) also have a higher prevalence of cigarette smoking than the general population. Sociodemographic characteristics associated with current smoking among PLWH center on socioeconomic status, such as lower educational level, poverty, homelessness, and incarceration. Age and race have also been shown to affect adherence to tobacco dependence treatment among PLWH.
The literature points to numerous risk factors associated with cigarette smoking among PLWH, including but not limited to substance use or co-dependence on other substances, alcohol use, marijuana use, mental illness, depression, lack of social support, not achieving HIV viral suppression, and other HIV-related symptoms. Some of these risk factors, particularly chronic stressors (e.g., poverty, racism/discrimination, stigmatization, lack of access to HIV treatment, emotional distress, pain, comorbid substance use), may serve as barriers to cessation. Additionally, given the increased number of tobacco products available to consumers, and the increasingly complex patterns of tobacco product use observed in the general population, it is likely that dual and poly tobacco product use has also increased among PLWH, which may pose another barrier to successful smoking cessation. The diversity of PLWH and risk factors posed by these sub-groups present unique challenges for smoking cessation. Understanding the complex interplay between sociodemographic and behavioral risk factors associated with cigarette smoking among PLWH will contribute to the development of effective smoking cessation interventions, as well as inform and guide the adaptation of existing evidence-based interventions for this population.
Smoking Cessation Interventions for PLWH
The high prevalence of cigarette smoking and the high incidence of tobacco-related cancer and other smoking-related diseases among PLWH call for swift action to intervene with this population to reduce tobacco use rates. However, evidence to guide the care of PLWH smokers is inadequate. PLWH who smoke have substantially lower quit rates compared with the general population. It is not clear how to best structure or deliver evidence-based tobacco cessation treatment that will address the complex and unique needs of PLWH (e.g., risk factors, treatment needs). Tailored cessation studies designed for eventual dissemination and implementation are likely necessary to focus efforts on diverse groups of PLWH who use tobacco. The 2008 U.S. Public Health Service Guideline for Treating Tobacco Use and Dependence reported that, "No long-term RCTs [randomized controlled trials] have examined the effectiveness of interventions in this [HIV-positive] population." (p. 144). In addition, the Guideline identified three topics requiring additional research: 1) effectiveness of medications and counseling/behavioral interventions, including tailored interventions; 2) effectiveness of motivational interviewing and educational approaches in increasing motivation to quit; and 3) effectiveness of community and social support networks in bolstering quitting motivation and improving treatment outcomes. Given the few randomized clinical trials examining smoking cessation treatment for PLWH conducted to date, and the major methodological limitations of many of these studies (e.g., lack of randomization, comparison conditions, treatment fidelity assessments, abstinence verification tests), there is a strong need for additional research in this area.
Key barriers to successful cessation among PLWH include patient-level, provider-level, and systems- or contextual-level factors. The complex interplay among these factors presents unique challenges.
Patient-Level. There is little evidence regarding the optimal smoking cessation treatment (e.g., combination of behavioral and pharmacological) for this population. Thus, it is not clear whether existing evidence-based smoking cessation interventions will be effective for PLWH or if modifications to standard treatments will enhance their effectiveness. PLWH are a diverse group of individuals, and certain subgroups of PLWH with higher tobacco use rates (e.g., sexual and gender minorities, individuals with mental and substance use disorders) may have unique treatment needs. For example, PLWH who have behavioral health conditions (e.g., mental and substance use disorders) have disproportionately high smoking rates and may have unique treatments needs; both from a medication standpoint (nicotine impacts the effectiveness of some psychotropic medications) and a counseling/behavioral standpoint (this subpopulation may need more intensive support). A better understanding of how tobacco cessation interventions can be adapted for different subgroups of PLWH is needed. Finally, few studies have examined relapse to smoking among PLWH. Among the general population, relapse is highest within the first few weeks after a quit attempt; this is also likely to be true among PLWH.
Provider-Level. The health care setting provides opportunities to identify tobacco users and offer evidence-based cessation treatments. However, few studies have focused on understanding barriers and facilitators of smoking cessation treatment delivery by HIV treatment providers (e.g., doctors, nurses, peer educators, social workers). Several studies suggest that these providers may not be adequately addressing tobacco use among their patients. Existing studies indicate that few PLWH are offered cessation treatment, often because it is not viewed as a high priority by patients and providers alike. This emphasizes the need for research to understand how to reduce barriers to delivering smoking cessation treatment among HIV treatment providers and increase motivation for smoking cessation among their patients.
Systems- or Contextual-Level. Although integrating evidence-based smoking cessation interventions into HIV clinical care settings has been shown to be feasible, treating tobacco dependence is not yet the standard of care in these settings. Barriers include lack of provider and patient time, competing demands, and lack of staff training. Integration of evidence-based smoking cessation interventions into the HIV clinical care setting may provide greater continuity of care and opportunities to intervene with this population. Research is also needed to understand how to implement evidence-based tobacco cessation interventions for PLWH in settings beyond the HIV clinical care setting (e.g., community settings, prisons, and other medical care settings).

Annual grantee meetings: Awardees will be expected to participate in an annual investigators' meeting that may be hosted at NCI and/or on a rotating basis at the participating Principal Investigators' institutions. The meetings will provide a forum for presenting scientific findings from each of the funded studies and will facilitate interactions among the community of funded scientists. Applicants should budget for the Principal Investigator and one additional project personnel to attend the planned annual grantee meeting. Investigators will be expected to participate in conference calls for planning purposes biannually or at other appropriate intervals; they may also be asked to participate in committees that meet periodically by conference call.
Standardization and coordination: Funded investigators will, to the extent possible, be expected to collaborate and report key common variables in a standardized manner. Investigators may also be asked to collaborate in the development of formative tools, approaches to dissemination, and other areas of shared investigator interest.

The R21 exploratory/developmental grant supports investigation of novel scientific ideas or new model systems, tools, or technologies that have the potential for significant impact on biomedical or biobehavioral research. An R21 grant application need not have extensive background material or preliminary information. Accordingly, reviewers will emphasize the conceptual framework, the level of innovation, and the potential to significantly advance our knowledge or understanding. Appropriate justification for the proposed work can be provided through literature citations, data from other sources, or, when available, from investigator-generated data. Preliminary data are not required for R21 applications; however, they may be included if available.

A proposed Clinical Trial application may include study design, methods, and intervention that are not by themselves innovative but address important questions or unmet needs. Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).
Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Does the project address an important problem or a critical barrier to progress in the field? Is there a strong scientific premise for the project? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?

Are the scientific rationale and need for a clinical trial to test the proposed hypothesis or intervention well supported by preliminary data, clinical and/or preclinical studies, or information in the literature or knowledge of biological mechanisms? Does the design/research plan include innovative elements, as appropriate, that enhance its sensitivity, potential for information or potential to advance scientific knowledge or clinical practice?

Study Design
Is the study design justified and appropriate to address primary and secondary outcome variable(s)/endpoints that will be clear, informative, and relevant to the hypothesis being tested? Is the scientific rationale/premise of the study based on previously well-designed preclinical and/or clinical research? Given the methods used to assign participants and deliver interventions, is the study design adequately powered to answer the research question(s), test the proposed hypothesis/hypotheses, and provide interpretable results? Is the trial appropriately designed to conduct the research efficiently? Are the study populations (size, gender, age, demographic group), proposed intervention arms/dose, and duration of the trial, appropriate and well justified?
Are potential ethical issues adequately addressed? Is the process for obtaining informed consent or assent appropriate? Is the eligible population available? Are the plans for recruitment outreach, enrollment, retention, handling dropouts, missed visits, and losses to follow-up appropriate to ensure robust data collection? Are the planned recruitment timelines feasible and is the plan to monitor accrual adequate? Has the need for randomization (or not), masking (if appropriate), controls, and inclusion/exclusion criteria been addressed? Are differences addressed, if applicable, in the intervention effect due to sex/gender and race/ethnicity?
Are the plans to standardize, assure quality of, and monitor adherence to, the trial protocol and data collection or distribution guidelines appropriate? Is there a plan to obtain required study agent(s)? Does the application propose to use existing available resources, as applicable?
Data Management and Statistical Analysis
Are planned analyses and statistical approach appropriate for the proposed study design and methods used to assign participants and deliver interventions? Are the procedures for data management and quality control of data adequate at clinical site(s) or at center laboratories, as applicable? Have the methods for standardization of procedures for data management to assess the effect of the intervention and quality control been addressed? Is there a plan to complete data analysis within the proposed period of the award?
Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?
In addition, for applications involving clinical trials:
If proposed, are the administrative, data coordinating, enrollment, and laboratory/testing centers, appropriate for the trial proposed?
Does the application adequately address the capability and ability to conduct the trial at the proposed site(s) or centers? Are the plans to add or drop enrollment centers, as needed, appropriate?
If international site(s) is/are proposed, does the application adequately address the complexity of executing the clinical trial?
If multi-sites/centers, is there evidence of the ability of the individual site or center to: (1) enroll the proposed numbers; (2) adhere to the protocol; (3) collect and transmit data in an accurate and timely fashion; and (4) operate within the proposed organizational structure?
As applicable for the project proposed, reviewers will evaluate the following additional items while determining scientific and technical merit, and in providing an overall impact score, but will not give separate scores for these items.
Specific to applications proposing clinical trials:
Is the study timeline described in detail, taking into account start-up activities, the anticipated rate of enrollment, and planned follow-up assessment? Is the projected timeline feasible and well justified? Does the project incorporate efficiencies and utilize existing resources (e.g., CTSAs, practice-based research networks, electronic medical records, administrative database, or patient registries) to increase the efficiency of participant enrollment and data collection, as appropriate?? Are potential challenges and corresponding solutions discussed (e.g., strategies that can be implemented in the event of enrollment shortfalls)?
For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of children to determine if it is justified in terms of the scientific goals and research strategy proposed. The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. Reviewers will assess the use of chimpanzees as they would any other application proposing the use of vertebrate animals. In accordance with the statutory provisions contained in Section 872 of the Duncan Hunter National Defense Authorization Act of Fiscal Year 2009 (Public Law 110-417), NIH awards will be subject to the Federal Awardee Performance and Integrity Information System (FAPIIS) requirements.
