NIH’s new Application Submission System & Interface for Submission Tracking (ASSIST) is available for the electronic preparation and submission of multi-project applications through Grants.gov to NIH. Section I. Funding Opportunity Description
Background and Rationale

Measurement is the backbone of all scientific discoveries. Without accurate, valid, and reliable measurement in a biological assay, a blood chemistry profile, or a neuropsychological assessment, for example, data yield is problematic and interpretation of results is severely compromised, if not rendered null. For studies of behavioral and neurological function, many researchers/psychometricians have developed assessment tools to measure emotional, sensory, motor or cognitive performance and have delivered to the field a variety of excellent batteries. However, many of these tools are outdated, are sometimes inappropriately used, e.g., attempting to measure normal function with a tool designed to assess dysfunction/disorder, and are not used uniformly. Investigators have expressed the need for brief assessment tools that could be used as a form of common currency across diverse study designs and populations. In response to these declared needs, the NIH awarded a contract in 2006 to establish the NIH Toolbox for Assessment of Neurological and Behavioral Function. The NIH Toolbox was developed to provide to the clinical research community a set of brief, psychometrically sound measures to assess neurological and behavioral function in community-based populations. The now-completed contract delivered measures and normative data for non-disease/disorder populations, ages 3 85 years, and in both Spanish and English versions.

Research into the onset and development of amnestic Mild Cognitive Impairment (aMCI) and Alzheimer’s disease (AD) would benefit from a set of standard measures that could be used uniformly at AD Centers and research sites across the USA and internationally. Currently, NIA-funded AD Centers use the Uniform Data Set (UDS) of measures to monitor change in cognitive function over time and aid in determination of a dementia diagnosis. However, limitations of the UDS include cost of administration, length of administration, lack of computerization, and a narrow range of subdomain assessments.

Most treatment trials for AD now focus on much earlier stages of the disease than ever before, including the pre-dementia stage. However, measures of cognition that are currently used are not, and were not, designed to be sensitive at this stage. The most commonly used outcome measure for AD clinical trials is the Alzheimer’s Disease Assessment Scale Cognitive Subscale (ADAS-Cog), but it does not possess the sensitivity needed to detect subtle cognitive change in the pre-dementia stage. Furthermore, it does not provide for assessment of other non-cognitive domains (sensory, motor or emotional function) for which change in performance may herald developing disease and serve as markers for pre-clinical AD. At an exploratory workshop held by the NIA in 2010, "Sensory and Motor Dysfunction in Aging and Alzheimer's Disease," participants discussed whether change/dysfunction in these domains might herald onset of dementia. Comprehensive measures of sensory, motor and emotional performance are rarely collected in studies of cognition, due in no small part to the lack of instruments that are well-validated and have a short administration time; further, where such domains are sampled, the heterogeneity of measures employed hinders the ability to compare data readily across studies. Building more comprehensive datasets in domains besides cognition will help advance our knowledge of the utility of change in these domains as early predictors of dementia. Furthermore, performance on these non-cognitive domains also may help in differentiating age-related cognitive decline in the oldest-old from aMCI or early AD. In sum, alternative measures to those currently available are urgently needed.

The NIH Toolbox was designed as a 'dynamic' assessment tool so that it can faithfully incorporate modifications/improvements in the future without loss of comparable data collection. In 2015, an iPad version of the NIH Toolbox was developed and is now available through an app in the iTunes App Store. Validation of the NIH Toolbox iPad version in patient populations would be an enormous benefit to clinical research, not only in terms of having brief but comprehensive assessments of neurological and behavioral function in a highly portable format, but importantly to provide an opportunity for the collection of uniform data across studies. The goal of this proposal is to take advantage of the new iPad version of the NIH Toolbox to validate the measures in persons with aMCI and early AD, and in individuals over 85 years of age. The resultant product will provide a valuable resource for the AD and oldest-old research communities.

Specific Research Objectives

Valid and innovative measures are needed by the MCI and AD research community to facilitate detection of disease at the earliest stages, assessment of change in function over time, and ability to track success of preventive and therapeutic interventions. To address these needs, the following elements are particularly sought, although applicants are welcome to include additional features appropriate for extending the NIH Toolbox in this way:

  • Establish and implement the basic governance and administrative structure for collection of data;
  • Create programming and database structures to receive, store, aggregate, analyze and share (as appropriate) data collected with the NIH Toolbox in aMCI and early AD patients and in individuals over 85 years of age;
  • Ensure diversity of race, ethnicity and gender within the patient and oldest-old groups to be evaluated;
  • Establish the psychometric properties of the NIH Toolbox in patients with aMCI and early AD, and in individuals over 85 years of age;
  • Evaluate the performance of the NIH Toolbox measures in the designated groups of interest in the context of previously collected and available AD biomarker data;
  • Collect longitudinal data to evaluate reliability of the instruments (short term) and to evaluate performance over longer intervals (up to 2 years) in the designated groups of interest;
  • Evaluate promising non-NIH Toolbox measures for inclusion in the NIH Toolbox iPad application that would enhance the performance of the measurement battery for detecting disease at the earliest stages and differentiating between age-related cognitive decline and impairment/dementia in the oldest-old;
  • Ensure integration of any promising non-NIH Toolbox measures into the software for the NIH Toolbox app;
  • Provide the operating procedures for administering validated and enhanced measurement batteries; and
  • Prepare documentation to supply to the FDA to qualify the measures as drug development tools for use in FDA clinical trials for MCI and AD therapeutics.

Cooperative Agreement: A support mechanism used when there will be substantial Federal scientific or programmatic involvement. Substantial involvement means that, after award, NIH scientific or program staff will assist, guide, coordinate, or participate in project activities. Required Registrations

Applicant organizations must complete and maintain the following registrations as described in the SF 424 (R&R) Application Guide to be eligible to apply for or receive an award. All registrations must be completed prior to the application being submitted. Registration can take 6 weeks or more, so applicants should begin the registration process as soon as possible. The NIH Policy on Late Submission of Grant Applications states that failure to complete registrations in advance of a due date is not a valid reason for a late submission.

  • Dun and Bradstreet Universal Numbering System (DUNS) - All registrations require that applicants be issued a DUNS number. After obtaining a DUNS number, applicants can begin both SAM and eRA Commons registrations. The same DUNS number must be used for all registrations, as well as on the grant application.
  • System for Award Management (SAM) (formerly CCR) Applicants must complete and maintain an active registration, which requires renewal at least annually. The renewal process may require as much time as the initial registration. SAM registration includes the assignment of a Commercial and Government Entity (CAGE) Code for domestic organizations which have not already been assigned a CAGE Code.
  • NATO Commercial and Government Entity (NCAGE) Code Foreign organizations must obtain an NCAGE code (in lieu of a CAGE code) in order to register in SAM.

They should describe the process for consideration, evaluation and determination of measurement properties for any non-NIH Toolbox measures that could enhance the performance of the measurement battery for detecting disease at the earliest stages and differentiating between age-related cognitive decline and impairment/dementia in the oldest-old. The project may require modification of existing instruments or creation of new ones appropriate to the target groups in this initiative. In the Overall section, applicants should address how the various cores will be coordinated in order to establish the research resource.

Focusing on the project as a whole, describe (i) the importance of the problem or barrier to progress in the field for which the proposed project is focused; (ii) how the resource developed in the proposed project will improve scientific knowledge, assessment capability, and/or clinical practice; and (iii) how assessment and monitoring of early stages of disease will be changed if the proposed aims are achieved.

Specific Aims: Identify general objectives planned for the development and functioning of the Data and Statistical Analysis Core. Objectives should target plans for development of psychometric properties in the defined groups, data interoperability, data access and security and privacy, along with the main benchmarks that would indicate the accomplishment of these objectives.

Research Strategy: The Core should develop a central data and software resource for integrating and facilitating access, analysis, and dissemination of data for the purposes of determining the measurement properties of the instruments in the defined groups.

Applications should describe the following:

  • The overall vision of the Data and Statistical Analysis Core and how this meets the project objectives;
  • The proposed process for creating a data repository with the capability to store/aggregate all primary and processed data;
  • Plans for statistical and psychometric oversight and input;
  • Plans for integration and analysis of diverse data (cognitive, sensory, motor, emotional, biomarker) on the same subjects to achieve development of a valid research resource;
  • Plans to establish a data-submission pipeline, quality-control metrics and provenance checking required to deposit data for analysis;
  • Plans to combine and analyze various formats of data, including previously collected AD biomarker data on the participants in the study;
  • Ability to store and report participant data to the researchers via a secure system compliant with applicable federal data-security standards;
  • Ability to test additional instruments for possible inclusion into an aMCI and/or early AD specific battery;
  • Ability to perform quality control to ensure that any new measures meet relevant standards;
  • Ability to ensure appropriate translation of any additional instruments into Spanish;
  • Ability to ensure data security and maintain confidentiality of personally identifiable information at the highest level;
  • Specific processes and methods for integration of data generated by outside sources; and
  • Description of server maintenance, software maintenance and upgrades, security, backup processes, and data recovery plan.

Specific Aims: Describe a project that collects and determines the measurement properties of the NIH Toolbox and any appropriate adjunct measurement tools for creation of a research resource that facilitates detection of MCI and AD at the earliest stages, that detects change in function over time, and that could be used to track success of preventive and therapeutic interventions.

Research Strategy: Applicants should describe the scientific rationale and process for developing a measurement resource that will facilitate detection of disease at the earliest stages, increase uniformity of data collection and expand ability to interpret and combine results across studies. The plans for collection of data and validation of the measures in aMCI, early AD and over 85 years of age groups should be clearly described and justified. Applicants should clearly justify their sample size, selection of participants and how participants are stratified. The project should include the following:

4. Submission Dates and Times

Part I. Overview Information contains information about Key Dates and times. Applicants are encouraged to submit applications before the due date to ensure they have time to make any application corrections that might be necessary for successful submission. When a submission date falls on a weekend or Federal holiday, the application deadline is automatically extended to the next business day.

Organizations must submit applications to Grants.gov (the online portal to find and apply for grants across all Federal agencies) using ASSIST or other electronic submission systems. Applicants must then complete the submission process by tracking the status of the application in the eRA Commons, NIH’s electronic system for grants administration. NIH and Grants.gov systems check the application against many of the application instructions upon submission. Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).

Scored Review Criteria - Overall

Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.

Significance

Does the proposed plan address the needs of the research resource that it will serve? Is the scope of activities proposed for the plan appropriate to meet those needs? Will successful completion of the aims bring unique advantages or capabilities to the research resource? Does the proposed plan provide for a resource that will allow accessibility of these measurement systems by researchers? Will the proposed plan increase uptake of the measurement systems for use in research and clinical trials? Does the project address an important problem or a critical barrier to progress in outcomes assessment in AD and the oldest-old?

Investigator(s)

Are the PD(s)/PI(s) and other personnel well suited to their roles in the project? Do they have appropriate experience and training, and have they demonstrated experience and an ongoing record of accomplishments in managing technical research? Do the investigators demonstrate significant experience with coordinating collaborative research? If the project is multi-PD/PI, do the investigators have complementary and integrated expertise and skills; are their leadership approach, governance, plans for conflict resolution, and organizational structure appropriate for the project? Does the applicant have experience overseeing selection and management of subawards, if needed? Does the application propose novel organizational concepts, management strategies, or instrumentation in coordinating the research resource the project will serve? Are the concepts, strategies, or instrumentation novel to one type of research program or applicable in a broad sense? Is a refinement, improvement, or new application of organizational concepts, management strategies or instrumentation proposed? What novel approaches will be used by the investigators to capture relevant information for research purposes? Does the proposed plan allow for technological advances (e.g., upgrades to relevant software or development of new software) during the project period?

Approach

Are the overall strategy, operational plan, and organizational structure well-reasoned and appropriate to accomplish the goals of the research resource the project will serve? Will the investigators promote strategies to ensure a robust and unbiased scientific approach across the resource, as appropriate for the work proposed? Are potential problems, alternative strategies, and benchmarks for success presented? If the resource is in the early stages of operation, does the proposed strategy adequately establish feasibility and manage the risks associated with the activities of the resource? Are an appropriate plan for work-flow and a well-established timeline proposed? Have the investigators presented adequate plans to ensure consideration of relevant biological variables, such as sex, for studies of vertebrate animals or human subjects?

Environment

Will the institutional environment in which the project will operate contribute to the probability of success in facilitating the research resource it serves? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the institutional environment, infrastructure, or personnel? Are resources available within the scientific environment to support electronic information handling? Are there sufficient commitments of facilities, infrastructure, and other resources to allow for the full-scale operation?

Vertebrate Animals

The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. Reviewers will assess the use of chimpanzees as they would any other application proposing the use of vertebrate animals. 2. Review and Selection Process

Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s), convened by NIA in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications:

  • May undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.
  • Will receive a written critique.

Appeals of initial peer review will not be accepted for applications submitted in response to this FOA.

Applications will be assigned to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this FOA. Following initial peer review, recommended applications will receive a second level of review by the National Advisory Council on Aging. The following will be considered in making funding decisions:

  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.
  • Relevance of the proposed project to program priorities.

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the grantee’s business official.

Awardees must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. An applicant, at its option, may review information in the designated integrity and performance systems accessible through FAPIIS and comment on any information about itself that a Federal agency previously entered and is currently in FAPIIS. The Federal awarding agency will consider any comments by the applicant, in addition to other information in FAPIIS, in making a judgement about the applicant’s integrity, business ethics, and record of performance under Federal awards when completing the review of risk posed by applicants as described in 45 CFR Part 75.205 Federal awarding agency review of risk posed by applicants. This provision will apply to all NIH grants and cooperative agreements except fellowships.

Cooperative Agreement Terms and Conditions of Award

The following special terms of award are in addition to, and not in lieu of, otherwise applicable U.S. Office of Management and Budget (OMB) administrative guidelines, U.S. Department of Health and Human Services (DHHS) grant administration regulations at 45 CFR Part 75, and other HHS, PHS, and NIH grant administration policies.

The administrative and funding instrument used for this program will be the cooperative agreement, an "assistance" mechanism (rather than an "acquisition" mechanism), in which substantial NIH programmatic involvement with the awardees is anticipated during the performance of the activities. Under the cooperative agreement, the NIH purpose is to support and stimulate the recipients' activities by involvement in and otherwise working jointly with the award recipients in a partnership role; it is not to assume direction, prime responsibility, or a dominant role in the activities. Consistent with this concept, the dominant role and prime responsibility resides with the awardees for the project as a whole, although specific tasks and activities may be shared among the awardees and the NIH as defined below.

The PD(s)/PI(s) will have the primary responsibility for:

  • Defining objectives and approaches for the grant and its components, in collaboration with the steering committee (do not list the names of committee members);
  • Overseeing the planning and conducting of all scientific, organizational, and administrative activities;
  • Promoting and coordinating scientific collaborations across sites (if multiple sites are involved);
  • Overseeing the adherence to the approved plans for timely sharing of resources and data;
  • Participating as voting member(s) in the activities of the Steering Committee and interacting with any sub-committee(s);
  • Accepting and implementing all scientific, organizational, administrative, and policy recommendations approved by the Steering Committee to the extent consistent with applicable grant regulations;
  • Organizing and participating in the periodic Steering Committee Meetings and other activities of the project as relevant;
  • Cooperating in the program evaluation process and interacting with NIH representatives coordinating this process;
  • Overseeing the timely preparation and submission to the NIH of annual progress reports and other information as needed;
  • Conducting periodic self-evaluation of the resource; and
  • Being prepared for administrative site visits by NIH staff members.

Intellectual Property Consideration

Awardees will be responsible for relevant intellectual property oversight that will include:

  • Ensuring the ability to maintain, manage, modify, and distribute data by securing appropriate permissions from other parties involved;
  • Developing criteria for making decisions to include outcome measures and various technology as needed to address intellectual property constraints while ensuring minimal burden to users of these instruments;
  • Notifying users of the existence of third party ownership or reach-through, as appropriate;
  • Ensuring that infrastructure, items, and instruments developed with NIH grant support are made available to users in a manner consistent with NIH sharing policies (see http://sharing.nih.gov/), including the NIH research systems policy and the NIH data sharing policy as applicable;
  • Ensuring that agreements with any party, including users, do not unreasonably limit access by other parties to the NIH Toolbox infrastructure and/or items or instruments; and
  • Establishing appropriate agreements between/among stakeholders for the appropriate sharing of data and appropriate protection of intellectual property.

The PD(s)/PI(s) assumes responsibility and accountability to the applicant organization officials and to the NIH for the performance and proper conduct of research conducted with or through the grant in accordance with terms and conditions of the award.

The grant will be subject to external evaluation (coordinated by the NIH). The investigators will be expected to participate in such evaluations.

NIH staff have substantial programmatic involvement that is above and beyond the normal stewardship role in awards, as described below:

One or more designated NIH Project Scientist(s) will have substantial programmatic involvement that is above and beyond the normal stewardship role in awards. Additional NIH scientific staff members with relevant expertise may also become substantially involved in grant activities as Projects Scientists or Coordinators.

The NIH Project Scientist(s) and other substantially involved NIH program staff members will assist with the grant by:

  • Monitoring the operations of the components and activities;
  • Making recommendations to the Steering Committee on strategic directions and improvements to the components and activities;
  • If appropriate, collaborating scientifically on research projects involving the investigators;
  • Serving as a liaison to facilitate and/or coordinate interactions between the investigators and other entities that may be mutually beneficial;
  • Using the NIH channels to disseminate knowledge about the resource to the scientific community;
  • Facilitate contacts between the grantee(s) and outside researchers interested in potential collaborations and/or the use of the resource; and
  • Coordinating an external evaluation of the research program.

NIH staff members who are substantially involved in the scientific activities (e.g., publications) of the grant will not attend peer review meetings of renewal and/or supplemental applications.

In addition, an NIH Program Official will be responsible for the normal scientific and programmatic stewardship of the award, including reviewing progress, and will be named in the award notice. If this individual becomes substantially involved in the grant activities, he/she will not attend peer review meetings of renewal and/or supplemental applications or will seek NIH waiver if such participation is essential.

The NIH reserves the right to adjust funding, withhold, suspend, or terminate the support to the awardee and/or member institutions that are unable to meet the performance requirements set forth in these Terms and Conditions of Award, or significantly change the level of performance.

Areas of Joint Responsibility include:

Steering Committee. The Steering Committee will serve as the main governing board for the research resource development. The Steering Committee will consist of the following voting members:

  • PD(s)/PI(s), one of whom is expected to chair the Steering Committee;
  • Three to five other PD(s)/PIs and/or senior scientific staff from the grant, with at least one representative from each core; and
  • Two to three NIH scientific staff.

NOTE: Each voting member noted above will have one vote (including those individuals who may have multiple responsibilities).

All NIH representatives will collectively have one vote.

Additional individuals may be added to the Steering Committee as non-voting members by a decision of the existing voting members. These additional non-voting members may include, for example, other NIH Program Staff members, and/or Program Staff members from other Federal Agencies (e.g., U.S. Food and Drug Administration).

The Steering Committee will meet once per year in person and monthly via phone conference. Applicants should budget for in-person meetings to occur in the Washington, D.C. metropolitan area.

The Steering Committee will have primary responsibility for:

  • Overseeing the overall functioning of the grant;
  • Establishing advisory committees and subcommittees, as needed;
  • Reviewing strategic goals for its evolution as a research resource (in consultation with Scientific Consulting Panel, see below);
  • Ensuring goals for sustainability are prioritized;
  • Ensuring that the grant takes advantage of existing NIH resources and programs and;
  • Ensuring that grant meets the missions/needs of funding agency, to the extent possible.

The Steering Committee will also form subcommittees as appropriate and necessary, each with a specific functional area of oversight as defined in the application.

Scientific Consulting Panel: A Scientific Consulting Panel will operate as a subcommittee to the Steering Committee, advising the Steering Committee and providing technical expertise to the entire project. This panel will comprise scientific experts not affiliated with the grantee institution(s) and may include scientists from academic and other research institutions as well as from NIH, clinicians and clinical staff, and relevant stakeholders. Members of the Scientific Consulting Panel will be selected by the Steering Committee in consultation with the NIH Project Scientist. The Scientific Consulting Panel will be charged with the following activities:

  • Reviewing the overall progress and making appropriate recommendations to strengthen activities in certain areas once per year;
  • Participating in the annual Steering Committee Meeting;
  • Providing additional consultations to the Steering Committee and investigators as needed (responding to inquiries via e-mail and/or telephone); and
  • Providing linkages to investigators not funded by the grant but conducting, or interested in conducting, related research.

Members of the Scientific Consulting Panel should NOT be appointed prior to the submission of the application and should not be listed in the application as this will limit the potential pool of reviewers unnecessarily.

Dispute Resolution:

Any disagreements that may arise in scientific or programmatic matters (within the scope of the award) between award recipients and the NIH may be brought to Dispute Resolution. A Dispute Resolution Panel composed of three members will be convened. It will have three members: a designee of the Steering Committee chosen without NIH staff voting, one NIH designee, and a third designee with expertise in the relevant area who is chosen by the other two; in the case of individual disagreement, the first member may be chosen by the individual awardee. This special dispute resolution procedure does not alter the awardee's right to appeal an adverse action that is otherwise appealable in accordance with PHS regulation 42 CFR Part 50, Subpart D and DHHS regulation 45 CFR Part 16.

3. Reporting

When multiple years are involved, awardees will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.

A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. See the NIH Grants Policy Statement for additional information on this reporting requirement.

In accordance with the regulatory requirements provided at 45 CFR 75.113 and Appendix XII to 45 CFR Part 75, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313). As required by section 3010 of Public Law 111-212, all information posted in the designated integrity and performance system on or after April 15, 2011, except past performance reviews required for Federal procurement contracts, will be publicly available.
