Section I. Funding Opportunity Description

The purpose of the Immigrant health initiative is to support research to design and implement effective interventions to enhance health advantages and reduce health disparities among U.S. immigrant populations (particularly migrant workers, recent and 1st generation immigrants) and address factors related to immigration experiences that affect health. For the purposes of this funding announcement, the term "1st generation" refers to people who were born in their native country and relocated to the U.S. The term "2nd generation" refers to the U.S. born children of 1st generation immigrants.

In 2017, the immigrant population in the U.S. was 44.4 million accounting for 13.6% of the U.S. population and the immigrant population is expected to increase over the next decade. While point-in-time data show that approximately 30% of immigrant families have incomes below the federal poverty level and 23% of today's new immigrants tend to be less educated, these static data do not reflect the changing situation for most immigrants as they settle into a range of communities, acculturate to different social and cultural values, food choices, employment and educational opportunities, and health challenges.

Factors associated with immigration processes prior to and while deciding to immigrate, during the migration experience, and throughout the course of becoming accustomed to a new living environment can translate into higher risk for diseases in the face of multi-level challenges. Immigrants also face multiple ongoing challenges, such as low health literacy, lack of health insurance, limited English proficiency, barriers to effective patient-clinician communication, other limitations in accessing health care as well as maintenance of traditional health practices. Discrimination and prejudice are common and affect daily experiences with individuals and organizations within the host society. Structural factors such as local and national laws and policies, the availability of affordable housing, suitable employment and employment benefits, schools for children, local infrastructure that hinders or facilitates mobility to jobs, schools, medical attention, among other things may undermine well-being and increase stress on new arrivals and long-time immigrants with severe health effects.

Health disparities among some immigrant populations are well documented. However, immigrants also have better health outcomes than U.S. born populations, despite the adversities they encounter. Despite the numerous challenges facing first generation immigrants, research on the mental and physical health of these populations has demonstrated a paradoxical pattern. Recent immigrants have reported better health status than U.S. born populations, a status that is thought to deteriorate with increasing length of U.S. residence and in subsequent generations. More research is needed to understand the drivers of the immigrant paradox, the protective factors and the health advantages and why later generations may have poor health outcomes and how to promote these protective factors.

Importantly, risk factors and disease outcomes also vary by sub-populations among immigrants based on the country of origin; however, many studies consider immigrants as a monolithic group according to world region of origin (e.g., Latin America, Asia, Africa). For example, U.S. immigrants from approximately 20 Latin American countries are treated as one Hispanic/Latino group and not separated into nationality sub-populations, Asians (from more than 30 countries) or Africans (from over 20 countries) are treated as one immigrant population when the health outcomes are often different among different sub-populations. More research is needed to better understand the risk and protective factors unique to each at immigrant sub-population levels.

Most research on immigrant health does not consider the factors and processes that pushed migration of members from particular groups or subsets of groups from one country to another. Little attention has been given to the pre-existing experiences, (for example, starvation resulting from rapid environmental changes, changing food sources, violent outbreaks, exposures to toxic chemicals and pesticides), and how the experience of migration itself, or how the process of adjustment and adaptation to a new cultural, social, political and ecological environment may affect health outcomes. It is important to consider the period(s) in the life course when immigration was experienced and how that experience may affect health outcomes.

Given current understanding of the determinants of immigrant health, (e.g. social, environmental and behavioral) and the mechanisms that these determinants act on to influence health status, a well-developed intervention research framework and multilevel approach are needed to address immigrant health disparities. Singularly or interactively, these multiple factors at various levels may contribute to health disparities observed in these immigrant populations.

Therefore, to improve health outcomes in this vulnerable population, there is a need to build an evidence base for effective multilevel interventions that could enhance health advantages and reduce health disparities among U.S. immigrant populations.

Research Objectives:

Given the scientific literature documenting health inequities among immigrant populations, this announcement calls for multidisciplinary/multilevel research focusing on the design and implementation of effective interventions that will address immigrant-specific factors to reduce health disparities, particularly among migrant workers, recent and 1st generation immigrants.

The intervention research under this FOA should be aimed at improving the health outcome among immigrant groups by targeting the complex causes or consequences of health disparities. Multi-level interventions that include a combination of individual, group (such as peers, family members, etc.), and/or community-level intervention components have been shown to be effective in improving health outcomes. Therefore, this FOA strongly encourages multi-level interventions (i.e., ranging from individuals to societies) in addressing immigrant health disparities.

Many potential individual, contextual and structural factors influence the health of immigrants; therefore, this FOA invites applications that include multidisciplinary research to target multiple factors that cause health disparities among underserved immigrant populations. A life-course perspective is encouraged with interventions focusing attention on transition points across the lifespan and associated risk and protective factors for immigrant populations. Such an approach emphasizes the fact that early life disadvantage need not lead to later negative health outcomes, provided there are compensatory experiences in the intervening years. Attention to the pre-existing immigration experiences, cultural values and related health practices, to how the experience of migration itself, or adjustment, adaptation and assimilation/ long term residence to a new cultural, social, political and ecological environment may affect health outcomes, is encouraged. Intervention should focus on improving and or maintaining the protective factors in the lives of immigrants that may buffer the effects of adversity.

Applicants are encouraged to consider multilevel interventions to improve the health outcomes of immigrants, put into motion by the immigration process throughout the life course. These may include interventions to foster positive interactions and more inclusive, social climates in schools, workplaces, and other organizations/institution level or strategies to improve access to healthy foods, or safe recreational spaces to improve immigrant health.

Projects should involve collaborations among relevant stakeholders in U.S. immigrant population groups, such as researchers, community organizations, healthcare providers, public health organizations, consumer advocacy groups, and faith-based organizations. As appropriate for the research questions posed, inclusion of key immigrant community members in the conceptualization, planning and implementation of the research is encouraged to generate better-informed hypotheses and enhance the translation of the research results into practice.

Interventions that are culturally appropriate and community-based addressing immigration specific population level factors to overcome barriers to improve overall health and reduce health disparities among immigrants are encouraged. Research applications are encouraged to utilize rigorous innovative multidisciplinary approaches addressing social, behavioral factors combined with biological assessments to show whether they may serve as measurable indicators for excess risk of health outcomes. Interventions that include health information technology applications (e.g. behavior monitoring tools, decision aids, health information portals, etc.) and/or social media elements to improve immigrant health are encouraged. Studies to conduct only needs assessments or interventions designed to increase knowledge as a sole outcome will not be supported under this FOA.

Projects must include a focus on immigrants from one or more NIH-designated populations that experience health disparities in the United States, which include racial and ethnic minorities (Blacks or African Americans, Hispanics/Latinos, Asian Americans, and Pacific Islanders). For this FOA, residents of U.S. territories (Guam, Puerto Rico, American Samoa, Commonwealth of the Northern Mariana Islands, and US Virgin Islands) who migrate to the U.S. mainland are also considered to be immigrants. Research on refugees is not supported under this FOA.

Please note that exclusion of non-English speaking participants without compelling scientific justification (e.g., studies focused on second-generation immigrant youth) is discouraged and that appropriate translation services should be provided for in the research plan and the budget.

Research is encouraged among distinct immigrant sub-populations based on the country of origin, rather than larger racial/minority populations when feasible (e.g., Koreans, Vietnamese, Cambodian, etc., rather than Asian Americans). For projects involving comparisons across populations, these comparisons should illuminate immigrant-specific phenomena rather than representing more global comparisons between immigrants with non-Hispanic Whites or the U.S. general population. Examples of appropriate comparisons include but are not limited to:

  • immigrant sub-populations within the same racial/ethnic minority group (e.g., Nicaraguan immigrants vs. Mexican immigrants vs. Puerto Rican immigrants);
  • immigrants across racial/ethnic minority populations that share similar experiences in the U.S. (e.g., Asian immigrant garment workers vs. Latino immigrant garment workers) or from the same racial/ethnic minority population that work in different labor markets (e.g., Mexican agricultural workers vs. Mexican construction workers vs. Mexican meat processing workers);
  • immigrants with their U.S. born counterparts (Chinese immigrants vs. US-born Chinese Americans).

Specific Areas of Research Interest

Research topics of specific interest on interventions may include but are not limited to:

  • Improve health care access and utilization among newly arrived immigrant populations or migrant workers and 1st generation immigrants;
  • Develop multi-level interventions to address complex structural factors that influence the living conditions and result in health advantages or health disparities in the U.S.;
  • Address stress related to stigma, discrimination, social isolation, acculturation, and other experiences faced by immigrants that affect health;
  • Address exposures to adversities and chronic stress during the immigration process that result in worsened health outcomes;
  • Address social and cultural factors at individual, family and/or community levels that may support resilience through the immigration experience among immigrant sub-populations that result in health advantages;
  • Develop community level culturally appropriate health promotion strategies e.g., access to healthy foods, safe recreational spaces to promote immigrant health;
  • Address culture specific beliefs and practices for health promotion and disease prevention among various immigrant sub-populations, specifically where traditional health practices may be the preferred and/or used with western medicine/healthcare systems;
  • Develop culturally appropriate ways of treating and/or preventing the spread of infectious diseases that increase health risks among recent immigrants or migrant workers or immigrants who travel back and forth between the U.S. and the native country;
  • Utilize technology to address language and other barriers in accessing health care and health information among immigrant populations with limited English proficiency;
  • Design and implement culturally appropriate interventions to address immigrant specific risk factors to improve the health and quality of life of immigrant populations;
  • Develop individual, family and/or community levels interventions that may support resiliency through the immigration experience among immigrant sub-populations that result in health advantages;
  • Develop strategies to improve mental and physical health by fostering positive interactions and more inclusive, social climates in schools, workplaces for immigrant populations;
  • Address health care team or organizational-level factors (systems) that facilitate the treatment delivery and follow up care of medically underserved immigrants;
  • Address systematic barriers to care and accommodate cultural and individual diversity for multiple immigrant sub-populations;
  • Adapt evidence-based interventions to ensure cultural relevance for those with limited-English proficiency and low health literacy in low-resource settings to improve quality of care and health outcomes among immigrants;
  • Develop targeted prevention strategies for different immigrant sub-populations to address screening disparities for preventable diseases or conditions;
  • Develop intervention strategies focusing on key transition points across the lifespan and associated risk and protective factors for immigrant populations to improve overall health;
  • Develop strategies to address structural factors and institutional discrimination/racism that affect the health of immigrant populations;
  • Examine how multilevel intervention components may address social determinants (such as housing, employment and educational systems) to increase reach/access to prevention and care in low-resource settings for immigrant (sub)populations.

Design, Analysis, and Sample Size for Studies to Evaluate Group-Based Interventions: Investigators who wish to evaluate the effect of an intervention on a health-related biomedical or behavioral outcome may propose a study in which (1) groups or clusters are assigned to study arms and individual observations are analyzed to evaluate the effect of the intervention, or (2) participants are assigned individually to study arms but receive at least some of their intervention in a real or virtual group or through a shared facilitator. Such studies may propose a parallel group- or cluster-randomized trial, an individually randomized group-treatment trial, a stepped-wedge design, or a quasi-experimental version of one of these designs. In these studies, special methods may be warranted for analysis and sample size estimation. Applicants should show that their methods are appropriate given their plans for assignment of participants and delivery of interventions. Specific Areas of Research Interest for NEI:

The mission of the National Eye Institute (www.nei.nih.gov) is to eliminate vision loss and improve quality of life through vision research. The NEI supports basic and clinical research into diseases and disorders of the visual system and the special needs of people with impaired vision or who are blind. Research topics may include but are not limited to telemedicine, screening and automated diagnosis, medication adherence, quality of life, and rehabilitation strategies for people who are visually impaired or blind.

The National Eye Institute will not support clinical trials that are greater than minimal risk on this funding opportunity. Minimal risk is defined as the probability and magnitude of harm or discomfort anticipated in the research are not greater in and of themselves than those ordinarily encountered in daily life or during the performance of routine physical or psychological examinations or tests.

4. Submission Dates and Times

Part I. Overview Information contains information about Key Dates and times. Applicants are encouraged to submit applications before the due date to ensure they have time to make any application corrections that might be necessary for successful submission. When a submission date falls on a weekend or Federal holiday, the application deadline is automatically extended to the next business day.

Organizations must submit applications to Grants.gov (the online portal to find and apply for grants across all Federal agencies). Applicants must then complete the submission process by tracking the status of the application in the eRA Commons, NIH’s electronic system for grants administration. NIH and Grants.gov systems check the application against many of the application instructions upon submission. Errors must be corrected and a changed/corrected application must be submitted to Grants.gov on or before the application due date and time. If a Changed/Corrected application is submitted after the deadline, the application will be considered late. Section V. Application Review Information

1. Criteria

Only the review criteria described below will be considered in the review process. Applications submitted to the NIH in support of the NIH mission are evaluated for scientific and technical merit through the NIH peer review system.

In addition, for applications involving clinical trials: A proposed Clinical Trial application may include study design, methods, and intervention that are not by themselves innovative but address important questions or unmet needs. Additionally, the results of the clinical trial may indicate that further clinical development of the intervention is unwarranted or lead to new avenues of scientific investigation.

Overall Impact

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).

Scored Review Criteria

Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.

Significance

Does the project address an important problem or a critical barrier to progress in the field? Is the prior research that serves as the key support for the proposed project rigorous? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?

In addition, for applications involving clinical trials

Are the scientific rationale and need for a clinical trial to test the proposed hypothesis or intervention well supported by preliminary data, clinical and/or preclinical studies, or information in the literature or knowledge of biological mechanisms? For trials focusing on clinical or public health endpoints, is this clinical trial necessary for testing the safety, efficacy or effectiveness of an intervention that could lead to a change in clinical practice, community behaviors or health care policy? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed? Are potential ethical issues adequately addressed? Is the process for obtaining informed consent or assent appropriate? Is the eligible population available? Are the plans for recruitment outreach, enrollment, retention, handling dropouts, missed visits, and losses to follow-up appropriate to ensure robust data collection? Are the planned recruitment timelines feasible and is the plan to monitor accrual adequate? Has the need for randomization (or not), masking (if appropriate), controls, and inclusion/exclusion criteria been addressed? Are differences addressed, if applicable, in the intervention effect due to sex/gender and race/ethnicity?

Are the plans to standardize, assure quality of, and monitor adherence to, the trial protocol and data collection or distribution guidelines appropriate? Is there a plan to obtain required study agent(s)? Does the application propose to use existing available resources, as applicable?

Data Management and Statistical Analysis

Are planned analyses and statistical approach appropriate for the proposed study design and methods used to assign participants and deliver interventions? Are the procedures for data management and quality control of data adequate at clinical site(s) or at center laboratories, as applicable? Have the methods for standardization of procedures for data management to assess the effect of the intervention and quality control been addressed? Is there a plan to complete data analysis within the proposed period of the award?

If the project involves human subjects and/or NIH-defined clinical research, are the plans to address 1) the protection of human subjects from research risks, and 2) inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion or exclusion of individuals of all ages (including children and older adults), justified in terms of the scientific goals and research strategy proposed?

Study Timeline

Specific to applications involving clinical trials

Is the study timeline described in detail, taking into account start-up activities, the anticipated rate of enrollment, and planned follow-up assessment? Is the projected timeline feasible and well justified? Does the project incorporate efficiencies and utilize existing resources (e.g., CTSAs, practice-based research networks, electronic medical records, administrative database, or patient registries) to increase the efficiency of participant enrollment and data collection, as appropriate? Are potential challenges and corresponding solutions discussed (e.g., strategies that can be implemented in the event of enrollment shortfalls)?

Protections for Human Subjects

For research that involves human subjects but does not involve one of the categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.

For research that involves human subjects and meets the criteria for one or more of the categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. Inclusion of Women, Minorities, and Individuals Across the Lifespan

When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of individuals of all ages (including children and older adults) to determine if it is justified in terms of the scientific goals and research strategy proposed. Vertebrate Animals

The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. Reviewers will assess the use of chimpanzees as they would any other application proposing the use of vertebrate animals. For additional information on review of the Vertebrate Animals section, please refer to the Worksheet for Review of the Vertebrate Animal Section.

Biohazards

Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.

Resubmissions

For Resubmissions, the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.

Renewals

For Renewals, the committee will consider the progress made in the last funding period.

Revisions

For Revisions, the committee will consider the appropriateness of the proposed expansion of the scope of the project. If the Revision application relates to a specific line of investigation presented in the original application that was not recommended for approval by the committee, then the committee will consider whether the responses to comments from the previous scientific review group are adequate and whether substantial changes are clearly evident.

Resource Sharing Plans

Reviewers will comment on whether the following Resource Sharing Plans, or the rationale for not sharing the following types of resources, are reasonable: (1) Data Sharing Plan. (2) Sharing Model Organisms; and (3) Genomic Data Sharing Plan (GDS)

2. Review and Selection Process

Applications will be evaluated for scientific and technical merit by (an) appropriate Scientific Review Group(s) convened by the Center for Scientific Review, in accordance with NIH peer review policy and procedures, using the stated review criteria. Assignment to a Scientific Review Group will be shown in the eRA Commons.

As part of the scientific peer review, all applications will receive a written critique.

Applications may undergo a selection process in which only those applications deemed to have the highest scientific and technical merit (generally the top half of applications under review) will be discussed and assigned an overall impact score.

Applications will be assigned on the basis of established PHS referral guidelines to the appropriate NIH Institute or Center. Applications will compete for available funds with all other recommended applications submitted in response to this FOA. Following initial peer review, recommended applications will receive a second level of review by the appropriate national Advisory Council or Board. The following will be considered in making funding decisions:
  • Scientific and technical merit of the proposed project as determined by scientific peer review.
  • Availability of funds.
  • Relevance of the proposed project to program priorities.

Section VI. Award Administration Information

1. Award Notices

If the application is under consideration for funding, NIH will request "just-in-time" information from the applicant as described in the NIH Grants Policy Statement.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the grantee’s business official.

Awardees must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

Any application awarded in response to this FOA will be subject to terms and conditions found on the Award Conditions and Information for NIH Grants website. This includes any recent legislation and policy applicable to awards that is highlighted on this website.

Individual awards are based on the application submitted to, and as approved by, the NIH and are subject to the IC-specific terms and conditions identified in the NoA.

Institutional Review Board or Independent Ethics Committee Approval: Grantee institutions must ensure that all protocols are reviewed by their IRB or IEC. To help ensure the safety of participants enrolled in NIH-funded studies, the awardee must provide NIH copies of documents related to all major changes in the status of ongoing protocols.

Data and Safety Monitoring Requirements: The NIH policy for data and safety monitoring requires oversight and monitoring of all NIH-conducted or -supported human biomedical and behavioral intervention studies (clinical trials) to ensure the safety of participants and the validity and integrity of the data. Investigational New Drug or Investigational Device Exemption Requirements: Consistent with federal regulations, clinical research projects involving the use of investigational therapeutics, vaccines, or other medical interventions (including licensed products and devices for a purpose other than that for which they were licensed) in humans under a research protocol must be performed under a Food and Drug Administration (FDA) investigational new drug (IND) or investigational device exemption (IDE).

3. Reporting

When multiple years are involved, awardees will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.

A final RPPR, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. In accordance with the regulatory requirements provided at 45 CFR 75.113 and Appendix XII to 45 CFR Part 75, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period.
