SERVICE AWARD (NRSA) APPLICATIONS AND RELATED FORMS

National Institutes of Health

The Application for an Individual National Research Service Award 
(PHS 416-1) and Continuation Application (416-9) currently in use 
were revised August 31, 1998 and approved for use through August 31, 
2001.  The forms have again been revised and submission has been made 
to the Office of Management and Budget (OMB).  In the interim, until 
approval is granted from the OMB and until further notice, the 
current versions of the PHS 416-1 and 416-9 should be used.

The NIH will publish an announcement in the NIH Guide for Grants and 
Contracts when the new forms are available for use.  There will be a 
transition period when the new or old forms will be accepted.
