EXTENSION OF CURRENT PHS 398, 2590 AND RELATED FORMS

The Application for a Public Health Service Grant (PHS 398) and 
Application for Continuation of a Public Health Service Grant (PHS 
2590) currently in use were revised February 28, 1998 and approved for 
use through February 28, 2001.  The forms have again been revised and 
action has been taken for submission to the Office of Management and 
Budget (OMB).  In the interim, until approval is granted from the OMB 
and until further notice, the current versions of the PHS 398 and 2590 
should be used.

The NIH will publish an announcement in the NIH Guide for Grants and 
Contracts when the new forms are available for use.  There will be a 
transition period when the new or old forms will be accepted.
