Purpose

• Correction: Section IV (Application and Submission Information) Subsection 6 “Other Submission Requirements” under “General Submission Requirements” the first sentence should read “Health Promotion/Disease Prevention” instead of “Health Disparities/Disease Prevention.”
  
  
• Correction: Section IV (Application and Submission Information) Subsection 6.2 “Research Projects” indicates that the page limit for each Project is five pages.  This should indicate that the page limit for each Project is a total of 10 pages for the Research Plan, Sections a. through d.
  
  
• Correction: Where stated “Research Plan, Sections a. through d.” should read “Research Plan, Items 2 through 5” to follow new 398 form.
