This Notice is to provide two corrections to RFA-HS-11-001, (CHIPRA Pediatric Healthcare Quality Measures Program Centers of Excellence (U18), released on April 22, 2010, which can be found at: http://grants.nih.gov/grants/guide/rfa-files/RFA-HS-11-001.html.  The corrections are listed below:

• Grant project periods for the Centers of Excellence awarded under this FOA will be from Fiscal Year 2011 to Fiscal Year 2014.
• Section III.1.a (Eligible Institutions) of the FOA indicates ‘Consortia of up to 3 institutions/organizations are strong encouraged to apply’.  However, this statement should be corrected to indicate that there is NO limit to the number of institutions/organizations listed in the application.  Thus, there is no lower or an upper limit to the number of institutions/organizations that comprise the application.

Additionally, this Notice alerts potential applicants that AHRQ has posted on its website a set of Frequently Asked Questions (FAQs) and Answers related to this FOA.  The FAQs can be found at:   http://www.ahrq.gov/fund/hs11001qa.htm.   All interested potential applicants are strongly encouraged to review the FAQs prior to submitting an application.
