This addendum is to inform potential applicants of two clarifications in 
PAR-O4-039, https://grants.nih.gov/grants/guide/pa-files/par-04-039.html.

1. APPLICATION RECEIPT DATES (Pages 1 and 14) are as follows:
 
January 15, 2004
June 15, 2004
October 15, 2004
Thereafter, February 17, June 15, and October 15 annually for 2005 and 2006.
 
2. SUPPLEMENTARY INSTRUCTIONS (Page 6):  The following two sentences should 
replace the last two sentences under 1.

Certification that the defense has been successfully completed needs to be 
received by the Division of Scientific Review, AHRQ, (as noted above).  If 
not received within 60 days, the application will be returned without 
review.
