NOTICE OF GRANT AWARD LETTER:  NEW FORMAT

Agency for Healthcare Research and Quality (formerly AHCPR)

Effective September 2000, the Agency for Healthcare Research and 
Quality (AHRQ), formerly the Agency for Health Care Policy and Research 
(AHCPR), will begin the transition from the current Notice of Grant 
Award (NGA) Form PHS-5152-5 to a letter format NGA.  The change to 
letter format will apply to all AHRQ grant, cooperative agreement, and 
fellowship awards.  All information currently provided in the NGA   
Form PHS-5152-5 will continue to be provided in the new letter format.

Should you have questions concerning the change to letter format, 
please contact the AHRQ Grants Management Specialist identified in your 
NGA.
