Purpose

The purpose of this Notice is to inform potential applicants that effective immediately the National Heart, Lung, and Blood Institute (NHLBI) will participate in PAR-24-091 "Multidisciplinary Studies of HIV/AIDS and Aging (R01 Clinical Trial Optional)".

Interests of Specific Institutes/Centers/Offices

National Heart, Lung, and Blood Institute (NHLBI)

Applications that would be considered responsive to this NOFO include those that seek to elucidate how aging-related mechanisms and pathways contribute to HIV-associated heart, lung, blood and sleep (HLBS) comorbidities, co-infections, complications, and tissue reservoirs located in anatomical sites relevant to the mission of NHLBI. Topics of interest at the intersection of HIV, aging, and HLBS conditions and risk factors include, but are not limited to, the following. /strong>

  • Elucidating the combined effects of aging, HIV, and antiretroviral therapy on the pathogenesis and progression of HIV-associated cardiovascular (e.g., coronary artery disease), lung diseases (e.g., chronic obstructive pulmonary disease (COPD), pulmonary hypertension, and pulmonary fibrosis), and hematologic conditions such as anemia and thrombosis
  • Understanding the changes in hematopoiesis that occur with aging and HIV infection, including changes in the hematopoietic stem cells and the stem cell niche
  • Understanding the impact of sleep deficiency, sleep-disordered breathing, and/or sleep health disparities on the pathogenesis of HIV-associated cardiopulmonary and cardiometabolic diseases among older people living with HIV PLWH
  • Leveraging existing epidemiological cohort data (e.g., MWCCS: MACS/WIHS Combined Cohort Study) to evaluate the effects of aging on HLBS risk factors (e.g., nutrition, blood lipids, diabetes, hypertension, physical inactivity, sleep deficiency), outcomes, and mortality among PLWH
  • Mechanistic studies to reduce HIV-related heart, lung, blood diseases by modifying or eliminating more traditional risk factors such as smoking, metabolic disturbances, hypertension, and coagulation abnormalities
  • Identifying novel biomarkers or other measures to detect subclinical HIV-related heart, lung, and blood diseases and strategies to mitigate the manifestation of clinical disease
  • Effectiveness research on programs aimed at addressing social determinants of health impacting co-occurring HLBS conditions
  • Developing validated animal and in vitro models to understand mechanisms and target interventions to reduce the combined effects of HIV and aging on HLBS comorbidities
  • Elucidating the interrelationship between sleep deficiency, antiretroviral therapy, and vascular contributions to cognitive impairment and dementia (VCID) among older PLWH
  • Addressing the interplay of aging with HIV infection, disparities and lifespan issues as highlighted in the Updated Notice of Information for HIV Research Priorities at the National Heart, Lung, and Blood Institute (https://grants.nih.gov/grants/guide/notice-files/NOT-HL-23-088.html)

Note: Per NOT-HL-19-690, NHLBI will only accept mechanistic studies that meet NIH's definition of a clinical trial in response to PAR-24-091 Multidisciplinary Studies of HIV/AIDS and Aging (R01 Clinical Trial Optional) and its reissues.
