Purpose

The purpose of this notice is to modify the applicant instructions for Data Tables under Section IV.2 PHS 398 Research Training Program Plan. The modifications require that new (Type 1) applications include Data Tables 2, 3, and 4 as indicated below. Renewal applications must include the Data Tables specified originally in the FOA. In addition, general guidance is added.

 

Part 2. Section IV.2 Content and Form of Application Submission

Original Instructions per RFA-HD-18-011:

Follow all instructions provided in the SF424 (R&R) Application Guide with the following additional modifications:
Data Tables: Particular attention must be given to the required Training Data Tables. For this FOA, applicants should use the following Data Tables only (Data for Scholars should be entered into the Postdoctoral sections of the Data Tables):

Table 2 (Renewal applications only)

Table 3 (Renewal applications only)

Table 4 (Renewal applications only)

Table 5B (Renewal applications only)

Table 7 (Renewal applications only)

Table 8C, Part I only (Renewal applications only)
