Purpose
The purpose of this Notice is to clarify the instructions for the PHS 398 Research Plan form of the Overall component for applications that involve Human Subjects research.
Part 2. Section IV.2. Application and Submission Information
New Language inserted into:
PHS 398 Research Plan (Overall)
Protection of Human Subjects: If Human Subjects research is proposed, upload an attachment indicating that human subject data is included in the Protection of Human Subjects section of the Administrative Core component of the application.
Inclusion of Women and Minorities: If Human Subjects research is proposed and exemption 4 does not apply, upload an attachment indicating that Inclusion of Women and Minorities information is included in the Protection of Human Subjects section of the Administrative Core component of the application.
Inclusion of Children: If Human Subjects research is proposed and exemption 4 does not apply, upload an attachment indicating that Inclusion of Children information is included in the Protection of Human Subjects section of the Administrative Core component of the application.
Vertebrate Animals: If Vertebrate Animals research is proposed, upload an attachment indicating that the Vertebrate Animals information is included within the appropriate Core component of the application.
All other aspects of this FOA remain unchanged.
