Annual Reports

At least once every 12 months the IACUC, through the Institutional Official, must report to OLAW (see PHS Policy, IV.F.1.2., and 4.).  Information to be reported consists of any change in the institution’s program of animal care and use, in the Institutional Official and in the IACUC membership, the dates that the IACUC conducted its semiannual evaluations of the program and facilities, and any minority IACUC member views.

Reporting Periods

OLAW strongly encourages institutions to use the calendar year (January 1-December 31) as the reporting period.   Institutions that prefer to use a different 12-month reporting period may do so by indicating the preferred reporting period in their Assurance, or by submitting a letter or email to OLAW indicating the institution’s preferred reporting period.  If institutions do not specify a reporting period, OLAW will consider that the institution is defaulting to using the calendar year as its reporting period.

Due Dates

Annual reports are due to OLAW no later than the last day of the month immediately following the end of the institution’s reporting period.  Institutions using the calendar year as the reporting period must submit an annual report no later than January 31 for the preceding calendar year. 

Sample Annual Report Format

This sample format is provided to aid institutions in meeting the annual reporting requirements.

While this format is not mandated, its use will facilitate the processing of your annual report. It need not be accompanied by a cover letter or any documentation other than that described/requested in the sample format.

There are two options for using this sample.

1. Complete the form on-line, then print and mail it
2. Print the form, complete it manually and mail it (MS Word - 85 KB, RTF - 149 KB).

In either case, it must be printed and mailed to OLAW. It may not be submitted on-line at this time. Once signed by the institutional official and the chairperson of the Institutional Animal Care and Use Committee, it should be submitted to:
