Section I. Funding Opportunity Description


Purpose

The goal of this FOA is to establish functional genotype-phenotype relationships of genes known to cause Alzheimer's disease (AD), genes or genetic variants suspected of altering the risk of AD, and genetic and biological modifiers that contribute to the disease process in neural cells using human pluripotent stem cells (hPSC) and genome editing approaches. Determining the function of AD candidate risk genes and genetic variants, identified in GWAS and other studies, in hPSC derived human neurons and glial cells is expected to identify new gene or cellular networks and molecular targets underlying the etiology of AD.

Background

Alzheimer's disease (AD) is an irreversible, and currently incurable, brain disorder that affects as many as 5 million Americans. Animal models of AD have been critical to producing insights into disease mechanisms and for discovery and development of drugs for disease treatment. However, animal models are often imperfect at recapitulating the human disease pathophysiology or phenotype. Some of the limitations of animal models potentially could be overcome by developing human cell models of AD. The discovery that human adult fibroblasts and other cell types can be reprogrammed into induced pluripotent stem cells (iPSCs) offers enormous potential for generating disease-specific cells for analyses of neurodegenerative disease mechanisms in human cells. The NIA issued an FOA on human cell reprogramming for aging and AD research in 2011. The intent of the FOA was to support exploratory research projects that would demonstrate proof-of-principle evidence that human reprogrammed cells can be valid models of aging and AD. Funded grants are defining the cellular and molecular phenotypes in neural cells derived from AD iPS cells. Recent studies have reported human cell models of AD using iPS or transdifferentiated cells prepared from patients with sporadic and familial AD. These studies are quite promising and demonstrate the feasibility of capturing relevant disease phenotypes in neurons derived from AD patient cells. The challenge now is to use human pluripotent stem cells to evaluate the contribution of genetic variation, mutations and risk factors in the development of AD pathophysiology and to more fully understand the cellular and molecular underpinnings of AD.

Human pluripotent stem cells (hPSCs) are amenable to the genetic and molecular approaches that have been used in model organisms, from worms to mice, to study and characterize disease mechanisms. hPSCs can be genetically modified using genome editing technologies. Engineered nucleases such as zinc finger nucleases (ZFNs), transcription activator-like effector nucleases (TALENs), and clustered regularly interspaced short palindromic repeat associated systems (CRISPR/Cas) are new tools to facilitate precise modification of the genome in hPSCs. The use of iPSCs derived from diseased patients and healthy control individuals to examine disease-associated genetic mutations has the limitation that the genetic background of disease-specific and control iPSC lines (and neurons derived from them) are not matched. This could contribute to variability in the observed phenotype, even masking a subtle disease-specific phenotype, and make it difficult to attribute an observed phenotype to a specific genetic variant. Using genome editing approaches, mutations can be introduced into wild-type cells or mutations in diseased cells can be corrected, creating isogenic lines in which mutant and normal genes are expressed in an identical genetic background. Causality in genotype-phenotype relationships in human brain cells could be established for known AD risk genes and examined for suspected AD susceptibility gene variants.

Genome-wide association studies (GWAS) and other genetic approaches have identified a number of genes and common genetic variants (SNPs) that contribute to the risk of late-onset AD. Functional evaluation of the GWAS identified risk loci is required to confirm their role in AD. SNPs can be in the protein coding region of genes, which would identify them as potential disease causal genes. SNPs can also be in non-coding, intergenic, or regulatory regions in which case the SNP variant loci could cause functional genomic or molecular effects and, thereby, contribute to the associated phenotype. The impact genetic variation has on gene expression is in general unknown. Studying whether SNPs affect genome-wide gene expression may be one approach to test whether SNPs associated with increased or decreased risk of AD are functional variants or tags of a relevant gene. A direct approach to study causal genotype-phenotype relationships in AD could be the use hPSCs to characterize the functional (e.g. gene expression and epigenetic) changes associated with potential risk genes or SNP variants in the appropriate brain cell type. Neurons or glial cells differentiated from iPSCs made from genotyped individuals carrying a SNP(s) associated with AD risk could be examined for genome-wide gene expression, epigenetic and other molecular changes. Correction of the DNA coding or non-coding risk variant by genome editing in the iPSCs could causally link the risk variant or gene to a functional phenotype(s). Genome editing could be used to engineer isogenic hPSC lines with and without a suspected AD genetic variant to identify the function of that variant or gene mutation in neurons or glial cells. An FOA is proposed to establish the function of genes or gene variants that cause AD, are suspected of altering the risk of AD, or modify the disease process.

Research Objectives

Human pluripotent stem cells may hold the key to study human AD processes in relevant human brain cells. The linkage of disease-associated genetic variants identified in GWAS to molecular and biological disease phenotypes and the identification of genetic and biological modifiers of disease processes in hPSC derived cells may suggest new mechanistic hypotheses on disease causation and therapeutic interventions. The goal of this FOA is to establish functional genotype-phenotype relationships of genes and/or genetic variants causing, modifying, or altering the risk of AD using hPSC and genomic editing approaches. Functional evaluation of AD risk genes and genetic variants in human neurons and glial cells derived from hPSCs may confirm the role in AD of the genes identified in GWAS, DNA sequencing and other studies. Studies on the contribution of multiple genes or genetic variants in producing molecular disease phenotypes using hPSCs will help define the role of common genetic variants in pathways underlying AD. Examining non-cell autonomous contributions to cellular phenotypes via multi-cell culture systems will also be informative. Applicants are expected to assess the transcriptome, epigenome, and biological phenotypes in hPSC-derived neural cells. This data should provide an integrated cell-based model of the genetic and molecular pathways affected by AD risk genes and genetic variants, and potentially identify new molecular targets in the etiology of AD.

Areas of research interest include, but are not limited to:

hPS cells developed under this FOA are considered unique research resources and as such NIH considers the sharing of such unique research resources an important means to enhance the value of NIH-sponsored research. It is expected that hPS cells developed under this FOA will be made readily available for research purposes to qualified individuals within the scientific community.

Overall Impact

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).

Scored Review Criteria

Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.

Significance

Does the project address an important problem or a critical barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?

Investigator(s)

Are the PD(s)/PI(s), collaborators, and other researchers well suited to the project? Protections for Human Subjects

For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.

Awardees must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

2. Administrative and National Policy Requirements

3. Reporting

When multiple years are involved, awardees will be required to submit the annual Non-Competing Progress Report (PHS 2590 or RPPR) and financial statements as required in the NIH Grants Policy Statement.

A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.

The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000. See the NIH Grants Policy Statement for additional information on this reporting requirement.
