The purpose of this Funding Opportunity Announcement (FOA) is to elucidate the etiology and pathogenesis of the neurocognitive complications associated with type 2 diabetes (T2D) with the ultimate goal of informing future strategies to mitigate the risk of these complications. Applications should propose expansions of ongoing human studies of well characterized T2D cohorts or cohorts comparing T2D with non-diabetic populations. Such expansions might include the addition of comprehensive neurocognitive measures (e.g., cognitive testing, neuroimaging, and biomarkers), clinical measures (e.g., insulin resistance, HbA1c), and/or collection of data on other risk factors (e.g., diet, obesity, micro- and macro-vascular disease, inflammation). It must be clearly explained how the collection of additional data will contribute to elucidation of the basis of neurocognitive sequelae of T2D.
Background: Emerging data have established links between T2D and neurocognitive impairment, including dementia. The current epidemic of dementia is driven, at least in part, by the concurrent epidemics of obesity, insulin resistance, T2D, and metabolic syndrome. Early research sought to elucidate the cause(s) for the apparent role of metabolic dysfunction in the increased prevalence of neurocognitive dysfunction and dementia, tentatively attributed to vascular contributions to cognitive impairment and dementia (VCID) and Alzheimer’s disease (AD). Recent neuropathological research indicates that less than half of the variability in cognitive impairment in the years prior to death may be attributable to AD and other common pathologies, and it is unclear which pathologies are the primary drivers of the association between T2D and neurocognitive impairment and dementia.
Unlike other complications of T2D, neurocognitive complications of T2D have not clearly been demonstrated to correlate with measures of peripheral glycemia (with the exception of very poor glycemic control, HbA1c > 10%) and there is even less evidence for an association with other measures of peripheral glucose regulation (e.g., insulin concentration, insulin action, insulin resistance). The dearth of large-scale studies involving longitudinal assessment of T2D and associated risk factors for complications, neurocognition, and disease management has limited our knowledge of how specific parameters associated with T2D may lead to changes in brain structure and function and deficits in cognition, and how susceptibility to these brain changes may vary across the lifespan. Moreover, it can be difficult to distinguish the etiology of cognitive impairment since individuals with T2D are at increased risk of other associated complications (e.g., obesity, micro- and macro-vascular disease) that can affect cognition. Recent advances in neuroimaging, computerized neurocognitive assessment, and the availability of large, well-characterized cohorts with T2D could lead to improvements in characterizing brain structure/function, cognition, and clinical parameters and other risk factors associated with T2D. If specific parameters and other risk factors for adverse neurocognitive outcomes associated with T2D could be defined, treatment protocols could be developed to limit neurocognitive complications associated with T2D.
Types of ancillary studies might include, but are not limited to:
Addition of metabolic and clinical phenotyping in individuals with T2D (e.g., measurement of insulin resistance, HbA1c), including associated factors of relevance to neurocognitive complications of T2D (e.g., diet, obesity, micro- and macro-vascular disease, inflammation), to studies that are already obtaining detailed measurement of neurocognition.
Addition of cohorts with T2D and associated metabolic disease (insulin resistance, prediabetes) to studies that are already obtaining detailed measurement of neurocognition.
Addition of comprehensive neurocognitive measures (e.g., cognitive testing, neuroimaging, and fluid-based biomarkers) to existing studies of T2D cohorts with well-characterized clinical correlates and course of disease.
Regardless of the type of study design, there must be a plan to measure the variables of interest in the same cohort at a minimum of two well justified time points. Cross sectional and purely correlative research is not consistent with the goals of this initiative.
Research that can examine how genetic and environmental factors confer risk or protect against the neurocognitive complications of T2D is encouraged. Research that addresses how neurocognitive complications of T2D impact academic and occupational function (where relevant) and activities of daily living, including the relationship to management of disease and/or treatment adherence is also encouraged.
Research across the lifespan and healthspan is appropriate for this FOA; however, it is possible that due to aging and disease-related factors, isolating mechanisms may be more complex and clinical targets less modifiable as individuals advance in age and disease burden increases. Therefore, there will be a special emphasis on research earlier in the developmental spectrum, especially prior to the development of complex disease (e.g., severe complications of T2D, CVD, dementia).
Strong expertise in T2D and associated disease processes (obesity, insulin resistance, prediabetes) and neuroscience is important for the research teams that will carry out the research supported by this FOA. It is anticipated that many of the funded research studies will involve the analysis and interpretation of complex, high-dimensional datasets. Therefore, it is strongly encouraged to include data scientists with relevant expertise on the research team. Collaborations between basic and clinical scientists are also encouraged but not required.
This FOA is NOT intended to fund the following:
Applicants can propose ancillary studies to ongoing R01 or equivalent studies as well as major ongoing clinical trials or longitudinal observational epidemiological studies. While new clinical trials to test efficacy of interventions for neurocognitive complications of T2D are not appropriate, ancillary studies to ongoing clinical trials may be appropriate if they have the potential to elucidate the etiology of neurocognitive function in T2D.
Proposed ancillary studies should provide an opportunity to explore new ideas and novel mechanisms rather than simply enhance protocol-specified ongoing measures in the parent study i.e., the ancillary study must have aims that are different from the parent study. It is strongly recommended that investigators contact NIDDK staff prior to submitting an ancillary study grant application to determine if the parent study is appropriate for this FOA and whether the proposed study is appropriate for this FOA.
If needed, the Data and Safety Monitoring Board (DSMB) or Observational Study Monitoring Board (OSMB) of the parent study could be used for the proposed project.

Applicants who are using an ongoing clinical study can request funds for selected costs specifically related to the proposed study and outside of normal activities of the clinical study. No costs should be requested for staff salary at the consortium's clinical sites. Instead, the additional time or supplies required should be requested on a capitated basis. For example, if a study requires an additional collection at the next scheduled monitoring visit, there will be no need to budget for costs related to the scheduled visit, but the costs of obtaining the additional samples should be requested (i.e., payment for a blood draw on a capitated basis, cost for a specific collection tube, and cost of shipping samples to the assay lab). Studies that require additional visits outside of those normally scheduled will need to budget for the costs associated with additional visits, including patient travel. If the proposed study will require subjects to visit a specific location for a specific test (for example, an imaging study), patient travel should be included in the request. Costs associated with data collection (case report forms) for data types currently collected do not need to be requested, but if the data or sample collection is new, some costs should be requested. Costs budgeted for Coordinating Center activities are only estimates.
Budget requests should include funds to support travel of the PDs/PIs to Bethesda, MD for one meeting of the neurocognitive complications of T2D and related metabolic factors investigators to be convened by NIDDK.

Research Strategy: The Research Strategy should include the study inclusion criteria for type 2 diabetes and related metabolic dysfunction (e.g., obesity) and the methods used to identify subjects with this diagnosis. In the Approach section when applicable, the measure (e.g., neurocognitive battery, neuroimaging, fluid-based biomarkers, metabolic assays) details and methodology, including amounts and types of measures and samples (applicants should request the minimum volume required for their assay); and reliability, validity, and quality control (measure "robustness") should be included. A statistical plan that shows the adequacy of the sample size to answer the research question and a detailed plan for data analysis, including the personnel involved in the biostatistical work, should also be included.
Letters of Support: Applications to this FOA for ancillary studies must include a letter from the appropriate committee (e.g., Ancillary Study Committee) or individual (e.g., Chair of the Steering Committee) representing the parent study indicating that the parent study is willing to provide the ancillary study investigators with access to participants, samples and data, as needed. A letter of support must document access to the subjects and/or availability of the samples from the clinical study or biorepository (e.g. the NIDDK Biorepository). The letter of support must state clearly the approval for the type and number of measures and samples and/or the number of subjects and clinical protocol. When applicable, the letter of support should acknowledge the burden to patients of the proposed clinical protocol. Approval for the measure and samples or subject access may be contingent on funding. This documentation must be included in the grant application or the application will be considered incomplete and will not be reviewed.

Reviewers will provide an overall impact score to reflect their assessment of the likelihood for the project to exert a sustained, powerful influence on the research field(s) involved, in consideration of the following review criteria and additional review criteria (as applicable for the project proposed).
Reviewers will consider each of the review criteria below in the determination of scientific merit, and give a separate score for each. An application does not need to be strong in all categories to be judged likely to have major scientific impact. For example, a project that by its nature is not innovative may be essential to advance a field.
Does the project address an important problem or a critical barrier to progress in the field? Is there a strong scientific premise for the project? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field? Will this research advance understanding of the neurocognitive complications of T2D and associated disorders (e.g., obesity)?
Are the PD(s)/PI(s), collaborators, and other researchers well suited to the project? If Early Stage Investigators or those in the early stages of independent careers, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?
Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?
Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project? Have the investigators presented strategies to ensure a robust and unbiased approach, as appropriate for the work proposed? Are potential problems, alternative strategies, and benchmarks for success presented? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed? Have the investigators presented adequate plans to address relevant biological variables, such as sex, for studies in vertebrate animals or human subjects?
If this is an ancillary study, has the applicant clearly demonstrated that they have the approval and cooperation of the parent grant for the activities proposed for the funding opportunity announcement?
Is there a plan to measure the variables of interest in the same cohort over a minimum of two time points? If so, are those time points well justified as supporting the study hypotheses?
If the project involves human subjects and/or NIH-defined clinical research, are the plans to address 1) the protection of human subjects from research risks, and 2) inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion or exclusion of children, justified in terms of the scientific goals and research strategy proposed? Is there a clear justification for the number of subjects and/or samples being requested? Is the statistical plan adequate to demonstrate that the number of subjects and/or samples will be sufficient to answer the research question? If applicable, did applicants justify the use of limited, irreplaceable, and valuable clinical samples, as opposed to other possible sources of samples? Did the study take full advantage of the unique characteristics of these particular subjects and/or samples?
Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?
As applicable for the project proposed, reviewers will evaluate the following additional items while determining scientific and technical merit, and in providing an overall impact score, but will not give separate scores for these items.
For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials. For additional information on review of the Human Subjects section, please refer to the Guidelines for the Review of Human Subjects.
When the proposed project involves human subjects and/or NIH-defined clinical research, the committee will evaluate the proposed plans for the inclusion (or exclusion) of individuals on the basis of sex/gender, race, and ethnicity, as well as the inclusion (or exclusion) of children to determine if it is justified in terms of the scientific goals and research strategy proposed. For additional information on review of the Inclusion section, please refer to the Guidelines for the Review of Inclusion in Clinical Research.
The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following criteria: (1) description of proposed procedures involving animals, including species, strains, ages, sex, and total number to be used; (2) justifications for the use of animals versus alternative models and for the appropriateness of the species proposed; (3) interventions to minimize discomfort, distress, pain and injury; and (4) justification for euthanasia method if NOT consistent with the AVMA Guidelines for the Euthanasia of Animals. Reviewers will assess the use of chimpanzees as they would any other application proposing the use of vertebrate animals. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.
For Resubmissions, the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.
For Renewals, the committee will consider the progress made in the last funding period.

A formal notification in the form of a Notice of Award (NoA) will be provided to the applicant organization for successful applications. The NoA signed by the grants management officer is the authorizing document and will be sent via email to the grantee’s business official.
Awardees must comply with any funding restrictions described in Section IV.5. Funding Restrictions. Selection of an application for award is not an authorization to begin performance. Any costs incurred before receipt of the NoA are at the recipient's risk. These costs may be reimbursed only to the extent considered allowable pre-award costs.

Cooperative Agreement Terms and Conditions of Award

When multiple years are involved, awardees will be required to submit the Research Performance Progress Report (RPPR) annually and financial statements as required in the NIH Grants Policy Statement.
A final progress report, invention statement, and the expenditure data portion of the Federal Financial Report are required for closeout of an award, as described in the NIH Grants Policy Statement.
The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for awardees of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All awardees of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at www.fsrs.gov on all subawards over $25,000.
