Section I. Funding Opportunity Description
1. Research Objectives
The National Institute on Alcohol Abuse and
Alcoholism (NIAAA) currently supports a complex array of basic behavioral
research on alcohol use that informs the prevention of HIV and AIDS. A
consensus of participants at a 2005 NIAAA-supported HIV/AIDS workshop, which
brought together funded prevention interventionists and basic behavioral
investigators in AIDS-related areas, cited the need for greater integration of
the theory, methods, knowledge, and insights derived from the basic behavioral
sciences into the development of innovative HIV prevention interventions.
Indeed, it seems likely that the next generation of HIV/AIDS prevention
interventions which are likely to include an array of emerging
behavioral and biomedically-based prevention approaches, such as topical
microbicides, pre- and post-exposure prophylaxis, circumcision, and,
potentially, vaccineswill need to build systematically upon the
advances that have been made in the basic behavioral research arena,
particularly the rich findings from epidemiological, clinical, as well as human
laboratory (experimental) studies in which alcohol is administered to human
subjects under laboratory conditions and behavior is carefully observed.
In the alcohol and HIV/AIDS fields, prevention interventions and basic prevention-oriented behavioral researchwhich is defined here as non-applied research to develop understanding of the determinants and processes affecting risk and protective behaviors, and including observational and qualitative studies, behavioral epidemiology, and experimental researchhave developed relatively independently. Thus, for example, basic behavioral research investigating the impact of alcohol use on disinhibition, expectancies, and social scriptsor situational factors such as alcohol-induced cognitive disruption or other in-the-moment decision-making behaviorshave not been explicitly incorporated into HIV preventive interventions that primarily focus on rational models of behavior change and skills training.
This FOA encourages Research Project Grant (R01) applications from institutions/ organizations that propose to integrate basic alcohol behavioral research and prevention-oriented interventions aimed at reducing alcohol-related HIV/AIDS risk. These interventions are intended to focus on high-risk (for HIV) individuals in their environments in a way that both individual and social/structural level determinants of risk behavior are measured and modified. Successful applications will propose both basic alcohol-specific behavioral (or biobehavioral, e.g. stress response) research and efficacy/effectiveness interventions that address at least one of the following overarching challenges facing the field of alcohol-related HIV/AIDS prevention:
It is also expected that proposals will be appropriate for transdisiplinary and translational research goals. In addition, the intervention component of the proposed application must focus on alcohol-using populations (domestic and/or international) at high-risk for HIV/AIDS: adolescents/young adults; members of racial minority groups; street youth/ homeless; detained youth; commercial sex workers; men who have sex with men; migrant workers; the severely mentally ill; women. The intervention(s) may focus on specific communities or venues within those communities (such as STD clinics, bars, etc.) for implementation of multilevel interventions.
All applications to this FOA must demonstrate that a research partnership between basic behavioral scientists and prevention interventionists exists or will be developed and nurtured. For purposes of this FOA, a basic scientist is defined as one who has demonstrable expertise in one or more lines of basic behavioral or biobehavioral researchdefined (as above) as observational and qualitative studies, epidemiological surveys, and experimental studies (including stress, arousal, other psychophysiological research domains)and for whom a preponderance of his or her recent research effort has involved primarily alcohol-use/non-injecting substance use populations, as measured by publications, grant support, presentations, or other scholarly work. Similarly, for purposes of this FOA, an interventionist is defined as one who has demonstrable and recent expertise in the conduct of behavior change interventions (not necessarily HIV risk behavior) with human populations.
Collaborative arrangements involving at least two investigators are encouraged. Importantly, the NIH now allows applicants and their institutions to identify more than one Principal Investigator (PI) (see NOT-OD-07-017) on RO1 and R21 applications. Research teams are to be conceptualized by the nature of the research goals and need not be limited by geographical or department boundaries. While collaborations among different institutions are encouraged, such applications must show how communication will occur across these boundaries so that fully developed collaborative research partnerships can occur.
RO1 applications submitted with a single principal investigator will be expected to include one or more additional investigators forming a collaborative team that includes basic and intervention expertise, clearly defining the roles of all investigators in the research. The principal investigator of a single-R01 project may be an individual with a primary research background in either basic behavioral or intervention areas. For investigators at multiple institutions, the collaborative arrangements will typically be indicated via one or more consortium/subcontract arrangements.

Background
In 2005, approximately 4.9 million people worldwide became newly infected with HIV. By the close of 2005, an estimated 40.3 million people were living with HIV/AIDS worldwide. Meanwhile, by the end of 2004, an estimated 462,792 persons were living with HIV/AIDS in the 35 areas in the U.S. with confidential name-based HIV infection reporting, and 71% of the estimated 123,405 female adults and adolescents living with HIV/AIDS had been exposed through heterosexual contact.
Like other sexually transmitted diseases (STDs), the spread of HIV/AIDS is facilitated by inconsistent use of barrier methods, such as condoms, and by involvement in high-risk sexual behaviors. Involvement in sex while under the influence of alcohol can cloud judgment aboutand negotiations concerningthe use of condoms or other emerging prevention strategies, such as microbicides and female condoms. A large body of evidence suggests that the relationship between drinking and risky sex likely is the result of a complex interaction among individual and situational factors, as well as partner characteristics such as the sexual experience of the partners. Other research has emphasized the role of alcohol expectancies as moderators of the relationship between alcohol use and risky sex .
Nevertheless, the apparent complexity of the relationship between alcohol use and HIV risk behavior involvement must not be a deterrent to the adoption of a broad prevention orientation in regards to this topic. The relative availability of both legal and illegally brewed alcohol in most parts of the world, its widespread use in conjunction with sex in or around bars and clubs as well as other illegal drinking settings, and the specific behavioral and biological effects that may increase transmission rates are all identified factors in the broader AIDS epidemic and likely to interact in sustaining the epidemic.
Particularly in the absence of a viable HIV vaccineat least in the short termit has become clear that prevention must play an increasingly prominent role in the worldwide response to HIV/AIDS. Indeed, several meta-analyses have determined that HIV/AIDS prevention interventions generally have been effective in reducing sexual risk behaviors in a variety of target populations.
As noted above, the global methodological approach specified by this FOA is integrative prevention research aimed at reducing alcohol-related HIV/AIDS risk. Additionally, at least one component of each project funded under this FOA must address at least one of the following four topical areas (see Objectives below), which represent some of the foremost overarching challenges facing the field of alcohol-related HIV/AIDS risk prevention (as identified at the 2005 NIAAA HIV Prevention Workshop). It is anticipated that successful resolution of these issues will greatly advance the field of alcohol-related HIV risk prevention. Although the original formulation of these themes emphasized primary prevention (for HIV), it should be noted that applications proposing HIV prevention with HIV-seropositive populations are eligible under this FOA provided that they address at least one of the four topical areas described below.
Objectives
1. Model Development, Integration, and Testing
A major overarching challenge facing the field concerns the development or integration of models to fully measure the effects of alcohol use for HIV risk behaviors at an individual, dyadic, and group level. According to one school of thought, alcohols effects on cognitive and attentional abilities are seen as affecting communication, raising the likelihood of misinterpretation, perception of a threat, and interpersonal violence as a defensive reaction. Another model is based on the assumption that individuals hold a priori beliefs that alcohol use will lead to involvement in risky behaviors. Other models of the effects of alcohol use emphasize physiological arousal, disinhibition, drinking prohibition violation, and other psychophysiological factors.
While great strides have been made over the past decade in testing the alcohol attentional and expectancy modelsand othersof the alcohol-risky decision-making relationship, inconsistencies in the results of various investigations utilizing these models suggest that the field remains in need of a convincing overall model to explain how changes in drinking might relate to altered HIV risk. Elements that may potentially find inclusion in an enhanced model have appeared in the research literature in recent years, but many still require careful testing in the context of modifiable HIV risk behaviors. For instance, substantial long-term investment in behavioral research by a number of NIH institutes has resulted in the development of a large body of knowledge in such areas as emotion, motivation, self-concept, personality, cognition, memory, social cognition, family processes, and social networks.
In summary, it appears important to begin consideration of how the various traditional models, both of alcohol-related behaviors and HIV-risky behaviorssuch as the Information-Motivation-Behavioral Skills (IMB) modelmay be integrated, streamlined, and/or otherwise altered to more fully explain risky sexual behaviors under the influence of alcohol. Indeed, one might suggest that relatively few of the popular HIV prevention interventions currently in use have been directly informed by findings from human laboratory experiments in which subjects in the moment behaviors and cognitions are evaluated.
The following are examples of broad research topics that might be the focus of integrative research to enhance model development under this FOA. This list is not meant to be comprehensive, nor are the examples meant to be exclusive of other topics. For each example listed here, applications are expected to propose efficacy or effectiveness interventions that test the newly developed models. It is understood that pilot testing and fine-tuning of intervention components, rethinking of model elements, and retesting of revised interventionse.g., reciprocal contributions from both the basic behavioral and intervention study investigatorsare essential elements of the scientific enterprise.

2. Clarifying the Mechanisms of Action in Prevention Interventions
After years of work, a real handle on the active ingredients in successful behavioral treatments for alcohol misusesuch as cognitive behavioral therapy, couples intervention, and 12 step facilitation approachesremains elusive, while the mechanisms of action in sexual risk reduction interventions appear to be equally unknown. It may be that factors such as, for example, a resilient affective style and dispositional moodand their underlying neural regulationare key factors in behavior change, thus provoking questions about how alcohol use might impact these factors in real time. Moreover, the behavioral change processes among individuals with suppressed executive functioning or other neuropsychological deficits associated with long-term alcohol abuse remain unclear, but may impact such things as adherence to medication usage.
Central to the illumination of the underlying mechanisms of behavior change in the prevention interventions is greater attention paid to the cognitive status of intervention participants. Thus, investigators must take into account the effects on participants of prior alcohol use, including neurocognitive and memory deficits, alcohol-related attentional bias, and impairment of facial expression recognition and information processing. It should also be noted that alcohol use appears to affect executive cognitive functioning differentially on the ascending and descending limbs of the blood alcohol concentration curve. Importantly, for interventionists, some research suggests that deficits in neurocognitive and emotional regulatory functions moderate the ability to respond to preventive intervention materials and enact protective behavioral schemas when necessary.

3. Recognition of Alcohol-Related Sexual/Physical Violence as a Factor in HIV Spread
The U.S. Presidents 5-year Emergency Plan for AIDS Relief, which aims to implement a combination of strategies to prevent HIV infection and to provide treatment and care for individuals infected with, or affected by HIV, acknowledges that intimate partner violence (IPV), coercive sex, and gender inequality fuel the spread of HIV/AIDS. There appear to be at least four mechanisms through which HIV and violence overlap in womens lives:
Alcohol use is linked to violent behavior by way of interacting pharmacological, physiological, endocrinological, genetic, situational, psychological, environmental, and sociocultural determinants. A number of studies have clarified the enormity of the risk for IPV that is conferred by alcohol use. For instance, in a national sample of couples 18 years of age and older who were interviewed in 1995 and again in 2000, the perpetration of male to female partner violence was found to increase significantly with the frequency of drinking 5+ drinks on occasion. Abbey and colleagues (2003) found that, in data collected from 113 college men who reported perpetration of a sexual assault since age 14, the quantity of alcohol consumed by perpetrators during the assault was linearly related to how much aggression they used. Moreover, studies of sexual and nonsexual assaults have found that a high proportion of victims had been drinking prior to the attack. Hence, it seems apparent that the HIV prevention community must acknowledge the role of sexual violence, and its sequelae, as contributing factors to HIV spread. Future HIV prevention interventions might profitably incorporate some of the insights from NIAAAs existing program of experimental research into the effects of alcohol use on violent and aggressive behaviors.

4. Assessment of the implications of alcohol use and abuse for emerging biomedical approaches to HIV prevention
The next generation of biomedically-based HIV/AIDS prevention interventions will need to build systematically upon the advances made in the basic research arena. To date, however, relatively little is known about how alcohol use and abuse may impact the use and long-term effectiveness of these emerging HIV prevention measures.

Multiple PD/PI Leadership Plan: For applications designating multiple PDs/PIs, a new section of the research plan, entitled Multiple PD/PI Leadership Plan, must be included. A rationale for choosing a multiple PD/PI approach should be described. The governance and organizational structure of the leadership team and the research project should be described, including communication plans, process for making decisions on scientific direction, and procedures for resolving conflicts. The roles and administrative, technical, and scientific responsibilities for the project or program should be delineated for the PDs/PIs and other collaborators.
If budget allocation is planned, the distribution of resources to specific components of the project or the individual PDs/PIs should be delineated in the Leadership Plan. In the event of an award, the requested allocations may be reflected in a footnote on the Notice of Award.
Applications Involving a Single Institution
When all PDs/PIs are within a single institution, follow the instructions contained in the SF424 (R&R) Application Guide.
Applications Involving Multiple Institutions
When multiple institutions are involved, one institution must be designated as the prime institution and funding for the other institution(s) must be requested via a subcontract to be administered by the prime institution. When submitting a detailed budget, the prime institution should submit its budget using the Research & Related Budget component. All other institutions should have their individual budgets attached separately to the Research & Related Subaward Budget Attachment(s) Form. allowable. A grantee may, at its own risk and without NIH prior approval, incur
obligations and expenditures to cover costs up to 90 days before the beginning
date of the initial budget period of a new or competing renewal (formerly
competing continuation) award if such costs: are necessary to conduct the
project, and would be allowable under the grant, if awarded, without NIH prior
approval. If specific expenditures would otherwise require prior approval, the
grantee must obtain NIH approval before incurring the cost. NIH prior approval
is required for any costs to be incurred more than 90 days before the beginning

The incurrence of pre-award costs in
anticipation of a competing or non-competing award imposes no obligation on NIH
either to make the award or to increase the amount of the approved budget if an

Plan for Sharing Research Data
The
precise content of the data-sharing plan will vary, depending on the data being
collected and how the investigator is planning to share the data. Applicants

for data sharing, the format of the final dataset, the documentation to be
provided, whether or not any analytic tools also will be provided, whether or
not a data-sharing agreement will be required and, if so, a brief description
of such an agreement (including the criteria for deciding who can receive the
data and whether or not any conditions will be placed on their use), and the
mode of data sharing (e.g., under their own auspices by mailing a disk or
posting data on their institutional or personal Web site, through a data

Investigators responding to this funding opportunity should include a sharing

Core Review Criteria. Reviewers will consider each of the five review criteria below in the determination of scientific and technical merit, and give a separate score for each. For example, a project that by its nature is not innovative may be essential to advance a field.
Significance: Does the project address an important problem or a critical barrier to progress in the field? If the aims of the project are achieved, how will scientific knowledge, technical capability, and/or clinical practice be improved? How will successful completion of the aims change the concepts, methods, technologies, treatments, services, or preventative interventions that drive this field?
Investigator(s): Are the PD/PIs, collaborators, and other researchers well suited to the project? If Early Stage Investigators or New Investigators, do they have appropriate experience and training? If established, have they demonstrated an ongoing record of accomplishments that have advanced their field(s)? If the project is collaborative or multi-PD/PI, do the investigators have complementary and integrated expertise; are their leadership approach, governance and organizational structure appropriate for the project?
Innovation: Does the application challenge and seek to shift current research or clinical practice paradigms by utilizing novel theoretical concepts, approaches or methodologies, instrumentation, or interventions? Are the concepts, approaches or methodologies, instrumentation, or interventions novel to one field of research or novel in a broad sense? Is a refinement, improvement, or new application of theoretical concepts, approaches or methodologies, instrumentation, or interventions proposed?
Approach: Are the overall strategy, methodology, and analyses well-reasoned and appropriate to accomplish the specific aims of the project? Are potential problems, alternative strategies, and benchmarks for success presented? If the project is in the early stages of development, will the strategy establish feasibility and will particularly risky aspects be managed?
If the project involves clinical research, are the plans for 1) protection of human subjects from research risks, and 2) inclusion of minorities and members of both sexes/genders, as well as the inclusion of children, justified in terms of the scientific goals and research strategy proposed?
Environment: Will the scientific environment in which the work will be done contribute to the probability of success? Are the institutional support, equipment and other physical resources available to the investigators adequate for the project proposed? Will the project benefit from unique features of the scientific environment, subject populations, or collaborative arrangements?
2.A. Additional Review Criteria
IAs applicable for the project proposed, reviewers will consider the following additional items in the determination of scientific and technical merit, but will not give separate scores for these items.
Protections for Human Subjects. For research that involves human subjects but does not involve one of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate the justification for involvement of human subjects and the proposed protections from research risk relating to their participation according to the following five review criteria: 1) risk to subjects, 2) adequacy of protection against risks, 3) potential benefits to the subjects and others, 4) importance of the knowledge to be gained, and 5) data and safety monitoring for clinical trials.
For research that involves human subjects and meets the criteria for one or more of the six categories of research that are exempt under 45 CFR Part 46, the committee will evaluate: 1) the justification for the exemption, 2) human subjects involvement and characteristics, and 3) sources of materials.
Inclusion of Women, Minorities, and Children. When the proposed project involves clinical research, the committee will evaluate the proposed plans for inclusion of minorities and members of both genders, as well as the inclusion of children.
Vertebrate Animals. The committee will evaluate the involvement of live vertebrate animals as part of the scientific assessment according to the following five points: 1) proposed use of the animals, and species, strains, ages, sex, and numbers to be used; 2) justifications for the use of animals and for the appropriateness of the species and numbers proposed; 3) adequacy of veterinary care; 4) procedures for limiting discomfort, distress, pain and injury to that which is unavoidable in the conduct of scientifically sound research including the use of analgesic, anesthetic, and tranquilizing drugs and/or comfortable restraining devices; and 5) methods of euthanasia and reason for selection if not consistent with the AVMA Guidelines on Euthanasia.
Resubmission Applications. When reviewing a Resubmission application (formerly called an amended application), the committee will evaluate the application as now presented, taking into consideration the responses to comments from the previous scientific review group and changes made to the project.
Renewal Applications. When reviewing a Renewal application (formerly called a competing continuation application), the committee will consider the progress made in the last funding period.
Revision Applications. When reviewing a Revision application (formerly called a competing supplement application), the committee will consider the appropriateness of the proposed expansion of the scope of the project. If the Revision application relates to a specific line of investigation presented in the original application that was not recommended for approval by the committee, then the committee will consider whether the responses to comments from the previous scientific review group are adequate and whether substantial changes are clearly evident.
Biohazards. Reviewers will assess whether materials or procedures proposed are potentially hazardous to research personnel and/or the environment, and if needed, determine whether adequate protection is proposed.
Additional Review Considerations
As applicable for the project proposed, reviewers will address each of the following items, but will not give scores for these items and should not consider them in providing an overall impact/priority score.
Budget and Period Support. Reviewers will consider whether the budget and the requested period of support are fully justified and reasonable in relation to the proposed research.
Select Agents Research. The amended policy incorporates: the use of an NIH definition of clinical
research; updated racial and ethnic categories in compliance with the new OMB
standards; clarification of language governing NIH-defined Phase III clinical
trials consistent with the SF424 (R&R) application; and updated roles and
responsibilities of NIH staff and the extramural community. The policy
continues to require for all NIH-defined Phase III clinical trials that: a) all
applications or proposals and/or protocols must provide a description of plans
to conduct analyses, as appropriate, to address differences by sex/gender and/or
racial/ethnic groups, including subgroups if applicable; and b) investigators
must report annual accrual and progress in conducting analyses, as appropriate,
by sex/gender and/or racial/ethnic group differences.
Inclusion of
Children as Participants in Clinical Research:
The NIH
maintains a policy that children (i.e., individuals under the age of 21) must
be included in all clinical research, conducted or supported by the NIH, unless
there are scientific and ethical reasons not to include them.
All investigators
proposing research involving human subjects should read the "NIH Policy
and Guidelines" on the inclusion of children as participants in research

Required
Education on the Protection of Human Subject Participants:
NIH policy
requires education on the protection of human subject participants for all
investigators submitting NIH applications for research involving human subjects

NIH Public Access Policy:
NIH-funded
investigators are requested to submit to the NIH manuscript submission (NIHMS)
system (http://www.nihms.nih.gov/) at
PubMed Central (PMC) an electronic version of the author's final manuscript
upon acceptance for publication, resulting from research supported in whole or
in part with direct costs from NIH. The author's final manuscript is defined as
the final version accepted for journal publication, and includes all
modifications from the publishing peer review process.
NIH is
requesting that authors submit manuscripts resulting from 1) currently funded
NIH research projects or 2) previously supported NIH research projects if they
are accepted for publication on or after May 2, 2005. The NIH Public Access Policy applies to all research grant and career development award mechanisms,
cooperative agreements, contracts, Institutional and Individual Ruth L.
Kirschstein National Research Service Awards, as well as NIH intramural
research studies. The Policy applies to peer-reviewed, original research
publications that have been supported in whole or in part with direct costs
from NIH, but it does not apply to book chapters, editorials, reviews, or
conference proceedings. Publications resulting from non-NIH-supported research
projects should not be submitted.

Standards for
Privacy of Individually Identifiable Health Information:
The Department
of Health and Human Services (HHS) issued final modification to the
"Standards for Privacy of Individually Identifiable Health
Information", the "Privacy Rule", on August 14, 2002. The Privacy Rule is a federal regulation under the Health Insurance Portability and
Accountability Act (HIPAA) of 1996 that governs the protection of individually
identifiable health information, and is administered and enforced by the HHS
Office for Civil Rights (OCR).
Decisions about
applicability and implementation of the Privacy Rule reside with the researcher
and his/her institution. The OCR website (http://www.hhs.gov/ocr/)
provides information on the Privacy Rule, including a complete Regulation Text
and a set of decision tools on "Am I a covered entity?" Information

Grant Applications or Appendices:
All applications and
proposals for NIH funding must be self-contained within specified page
limitations. For publications listed in the appendix and/or Progress report,
Internet addresses (URLs) or PubMed Central (PMC) submission identification
numbers must be used for publicly accessible on-line journal
articles. Publicly accessible on-line journal articles or PMC
articles/manuscripts accepted for publication that are directly relevant to the
project may be included only as URLs or PMC submission
identification numbers accompanying the full reference in either the
Bibliography & References Cited section, the Progress Report Publication
List section, or the Biographical Sketch section of the NIH grant application.
A URL or PMC submission identification number citation may be repeated in each
of these sections as appropriate. There is no limit to the number of URLs or
PMC submission identification numbers that can be cited.

The PHS strongly
encourages all grant recipients to provide a smoke-free workplace and
discourage the use of all tobacco products. In addition, Public Law 103-227,
the Pro-Children Act of 1994, prohibits smoking in certain facilities (or in
some cases, any portion of a facility) in which regular or routine education,
library, day care, health care, or early childhood development services are
provided to children. This is consistent with the PHS mission to protect and
advance the physical and mental health of the American people.
Loan
Repayment Programs:
NIH encourages
applications for educational loan repayment from qualified health professionals
who have made a commitment to pursue a research career involving clinical,
pediatric, contraception, infertility, and health disparities related areas.
The LRP is an important component of NIH's efforts to recruit and retain the
next generation of researchers by providing the means for developing a research
career unfettered by the burden of student loan debt. Note that an NIH grant is
not required for eligibility and concurrent career award and LRP applications
are encouraged. The periods of career award and LRP award may overlap providing
the LRP recipient with the required commitment of time and effort, as LRP
awardees must commit at least 50% of their time (at least 20 hours per week
based on a 40 hour week) for two years to the research. For further
