APPLICATION FOR CONTINUATION OF A PUBLIC HEALTH SERVICE GRANT (PHS 2590) FACE PAGES Release Date: February 16, 2000 NOTICE: OD-00-021 National Institutes of Health NIH has experienced an unanticipated depleted supply of the form used to produce the computer-generated Application for Continuation of a Public Health Service Grant (PHS 2590) face page. As a result, there has been a delay in mailing the pre-printed 2590 face pages and mailing labels. A partial mailing of face pages was completed for those applications due to NIH on March 1 (for budget periods beginning May 1). We have not yet mailed pre-printed 2590 face pages and mailing labels for those applications due to NIH on April 1 (for budget periods beginning June1). Grantees with noncompeting applications due to NIH in March for award budget periods beginning in May are strongly encouraged to complete a blank 2590 face page to submit with your applications. Blank 2590 face pages (Form Page A) are available in PDF format from the NIH website at https://grants.nih.gov/grants/funding/2590/2590.htm. To avoid processing delays, applications to continue support of an NIH grant must be submitted to the grants management office of the awarding NIH Institute or Center two months prior to the budget period start date. A list of mailing addresses for the 2590 application receipt points for the NIH Institutes and Centers is located on the NIH website at https://grants.nih.gov/grants/type5_mailing_addresses.htm NIH expects to immediately resume the printing and mailing of face pages for those applications due to NIH on April 1.
