Purpose

The Agency for Healthcare Research and Quality's (AHRQ) new Notice of Intent signals the Agency's upcoming plan to fund grants that use innovative approaches to data analytics to support chronic disease prevention and management by engaging patient navigators to improve population health.

AHRQ intends to publish a Request for Applications (RFA) in fiscal year 2019 for investigator-initiated projects that would use data analytics to assess community patterns of chronic illness, modifiable risk factors, and clinical and community resources to support chronic disease prevention and management. Data would be used to support patient navigators embedded in primary care practices to identify patients at high risk for preventable disease or disease progression and connect them to clinical and community services in order to improve health outcomes.
