Purpose

This Notice informs the community of an application form issue with RFA-HL-17-004 titled ‘Pediatric Heart Network Clinical Research Centers (UG1)’. The budget form originally posted with this funding opportunity announcement did not accommodate the full 7 years of support allowed for this opportunity. On December 15, a new application forms package with the appropriate budget form was posted in Grants.gov to address this issue. The new application form package has a Competition ID of ‘FORMS-C-REVISED’ and a Competition Title of ‘Revised to include appropriate budget form'.

The FORMS-C-REVISED application forms package with the updated budget form must be used when applying to RFA-HL-17-004.

Actions required (based on submission method)

Applicants using ASSIST: No action needed. We will update your application forms and data for you. The Competition ID ‘FORMS-C-REVISED’ will be displayed in the FOA information section of your application Summary tab once updated.

Applicants using Grants.gov’s downloadable forms: You must return to RFA-HL-17-004, download a fresh copy of the application package and manually move your application data to the new forms. To verify you have the correct forms, check for the Competition ID ‘FORMS-C-REVISED’ at the top of the Grant Application Package forms management screen.

Applicants using system-to-system solutions: Please check with your solution provider to determine what action, if any, is needed to update your application forms.
